2Wireless esophageal pH capsule for patients with gastroesophageal reflux disease: A multicenter clinical study显示文摘AIM:To investigate the feasibility and safety of pH capsule to monitor pH in patients with gastroesophageal reflux disease(GERD).METHODS:Ninety-one patients with symptoms suggestive of GERD were enrolled in this study,46 of whom were randomized to the pH capsule group;the remaining 45 patients used the conventional catheter and pH capsule simultaneously.The pH data and traceswere recorded via automatic analysis,and capsule detachment was assessed using X-ray images.All of the patients were required to complete a questionnaire regarding tolerance with the capsule.RESULTS:The capsules were successfully attached on the first attempt,and no early detachment of the capsules was observed.Compared to the 24-h pH data recorded with the conventional catheter,the data collected with the pH capsule showed no significant differences in 24-h esophageal acid exposure.The measurements of esophageal acid exposure over 24 h collected with the two devices showed a significant correlation(r2=0.996,P<0.001).Capsule detachment occurred spontaneously in 89 patients,and 2 capsules required endoscopic removal due to chest pain.The capsule was associated with less interference with daily activity.CONCLUSION:The wireless pH capsule provides a feasible and safe method for monitoring gastroesophageal reflux and therefore may serve as an important tool for diagnosing GERD.Xiao-Jun Yang Tian Gan Lei Wang Zhuan Liao Xiao-Hong Tao Wei Shen Xiao-Yan Zhao 2014World Journal of Gastroenterology2014,20,40:9
3Large-eddy simulation of shock-wave/turbulent boundary layer interaction with and without Spark Jet control显示文摘The efficiency and mechanism of an active control device ‘‘Spark Jet' and its application in shock-induced separation control are studied using large-eddy simulation in this paper.The base flow is the interaction of an oblique shock-wave generated by 8° wedge and a spatially-developing Ma = 2.3 turbulent boundary layer.The Reynolds number based on the incoming flow property and the boundary layer displacement thickness at the impinging point without shock-wave is20000.The detailed numerical approaches were presented.The inflow turbulence was generated using the digital filter method to avoid artificial temporal or streamwise periodicity.The numerical results including velocity profile,Reynolds stress profile,skin friction,and wall pressure were systematically validated against the available wind tunnel particle image velocimetry(PIV) measurements of the same flow condition.Further study on the control of flow separation due to the strong shock-viscous interaction using an active control actuator ‘‘Spark Jet' was conducted.The single-pulsed characteristic of the device was obtained and compared with the experiment.Both instantaneous and time-averaged flow fields have shown that the jet flow issuing from the actuator cavity enhances the flow mixing inside the boundary layer,making the boundary layer more resistant to flow separation.Skin friction coefficient distribution shows that the separation bubble length is reduced by about 35% with control exerted.Yang Guang Yao Yufeng Fang Jian Gan Tian Li Qiushi Lu Lipeng 2016Chinese Journal of Aeronautics2016,29,3:8

5Short- and long-term outcomes associated with enhanced recovery after surgery protocol vs conventional management in patients undergoing laparoscopic gastrectomy显示文摘BACKGROUND At present,the enhanced recovery after surgery(ERAS)protocol is widely implemented in the field of gastric surgery.However,the effect of the ERAS protocol on the long-term prognosis of gastric cancer has not been reported.AIM To compare the effects of ERAS and conventional protocols on short-term outcomes and long-term prognosis after laparoscopic gastrectomy.METHODS We retrospectively analyzed the data of 1026 consecutive patients who underwent laparoscopic gastrectomy between 2012 and 2015.The patients were divided into either an ERAS group or a conventional group.The groups were matched in a 1:1 ratio using propensity scores based on covariates that affect cancer survival.The primary outcomes were the 5-year overall and cancer-specific survival rates.The secondary outcomes were the postoperative short-term outcomes and inflammatory indexes.RESULTS The patient demographics and baseline characteristics were similar between the two groups after matching.Compared to the conventional group,the ERAS group had a significantly shorter postoperative hospital day(7.09 d vs 8.67 d,P<0.001),shorter time to first flatus,liquid intake,and ambulation(2.50 d vs 3.40 d,P<0.001;1.02 d vs 3.64 d,P<0.001;1.47 d vs 2.99 d,P<0.001,respectively),and lower medical costs($7621.75 vs$7814.16,P=0.009).There was a significantly higher rate of postoperative complications among patients in the conventional group than among those in the ERAS group(18.1 vs 12.3,P=0.030).Regarding inflammatory indexes,the C-reactive protein and procalcitonin levels on postoperative day 3/4 were significantly different between the two groups(P<0.001 and P=0.025,respectively).The ERAS protocol was associated with significantly improved 5-year overall survival and cancer-specific survival rates compared with conventional protocol(P=0.013 and 0.032,respectively).When stratified by tumour stage,only the survival of patients with stage III disease was significantly different between the two groups(P=0.044).CONCLUSION Adherence to the ERAS protocol improves both the short-term outcomes and the 5-year overall survival and cancer-specific survival of patients after laparoscopic gastrectomy.Yu-Long Tian Shou-Gen Cao Xiao-Dong Liu Ze-Qun Li Gan Liu Xing-Qi Zhang Yu-Qi Sun Xin Zhou Dao-Sheng Wang Yan-Bing Zhou 2020World Journal of Gastroenterology2020,26,37:8

7Effect of a transverse plasma jet on a shock wave induced by a ramp显示文摘We conducted experiments in a wind tunnel with Mach number 2 to explore the evolution of a transverse plasma jet and its modification effect on a shock wave induced by a ramp with an angle of 24°. The transverse plasma jet was created by arc discharge in a small cylindrical cavity with a 2 mm diameter orifice. Three group tests with different actuator arrangements in the spanwise or streamwise direction upstream from the ramp were respectively studied to compare their disturbances to the shock wave. As shown by a time-resolved schlieren system, an unsteady motion of the shock wave by actuation was found: the shock wave was significantly modified by the plasma jet with an upstream motion and a reduced angle. Compared to spanwise actuation, a more intensive impact was obtained with two or three streamwise actuators working together. From shock wave structures, the control effect of the plasma jet on the shock motion based on a thermal effect, a potential cause of shock modification, was discussed. Furthermore, we performed a numerical simulation by using the Improved Delayed Detached Eddy Simulation(IDDES) method to simulate the evolution of the transverse plasma jet plume produced by two streamwise actuators. The results show that flow structures are similar to those identified in schlieren images. 8Timing the termination of the Doushantuo negative carbon isotope excursion: evidence from U-Pb ages from the Dengying and Liuchapo formations,South China显示文摘The Doushantuo negative carbon isotope excursion(DOUNCE) is the largest known marine inorganic carbon isotope anomaly. The origin of this pronounced negative excursion is still an enigmatic issue that attracts geologists. Time constraints on the excursion are the critical information that would provide insight into its genesis. In previous decades, the timing of its termination has been constrained by the widely cited zircon U-Pb age of 550.5 ± 0.8 Ma for the tuff at the top of the Miaohe Member at the Jiuqunao section in the Yangtze Gorges area, South China. However, results of recent studies indicate that the reliability of this time constraint needs to be re-evaluated. Here, a geochronological study was carried out using two K-bentonites from Fanglong in South China. A K-bentonite in the lower Dengying Formation yielded a U-Pb age of 557 ± 3 Ma, while a K-bentonite in the basal Liuchapo Formation yielded an age of 550 ± 3 Ma. Based on regional correlations between the Ediacaran successions in South China,the age(557 ± 3 Ma) for the K-bentonite in the lower Dengying Formation may serve as a second critical timing constraint for the ending of the DOUNCE. 9Effects of interactions between environmental factors and KIF1B genetic variants on the risk of hepatocellular carcinoma in a Chinese cohort显示文摘AIM: To examine the effect of the potential interaction between KIF1 B variants(rs17401966 and rs3748578) and environmental factors on the risk of hepatocellular carcinoma(HCC) in a high-risk region in China.METHODS: Three hundred and six patients with HCC and 306 hospital-based control participants residing in the Shunde region of Guangdong Province, China were enrolled. Clinical characteristics were collected by reviewing the complete medical histories from the patient archives, and epidemiological data were collected using a questionnaire and clinical examination. Two single nucleotide polymorphisms(SNPs) of KIF1B(rs17401966 and rs3748578) were chosen for the current study. All subjects were genotypedusing a Taq Man real-time polymerase chain reaction. Multiplicative and additive logistic regression models were used to evaluate various gene-environment interactions.RESULTS: Smoking, frequent consumption of raw freshwater fish, hepatitis B virus(HBV) infection, and a family history of HCC were important risk factors for HCC in this population. Chronic infection with HBV was the most important environmental risk factor for HCC [odds ratio(OR) = 12.02; 95% confidence interval(95%CI): 6.02-24.00]. No significant association was found between the KIF1 B variants alone and the risk of HCC. Nevertheless, a significant additive effect modification was observed between rs17401966 and alcohol consumption(P for additive interaction = 0.0382). 10A Phytophthora capsid Effector Targets ACD11 Binding Partners that Regulate ROS-Mediated Defense Response in Arabidopsis显示文摘Reactive oxygen species (ROS) play a vital role in plant immune response, but the genes involved in the regulation of ROS are scantily reported. Phytophthora pathogens produce a large number of effectors to promote infection, but the modes of action adopted are largely unknown. Here, we report that RxLR207 could activate ROS-mediated cell death in Nicotiana benthamiana and was essential for virulence of P. capsici. We found that this effector targeted BPA1 (binding partner of ACD11) and four members of BPLs (BPAl-Like proteins)in Arabidopsis, and the bpa1 and bpl mutants had enhanced ROS accumulation and cell death under biotic or abiotic stresses. Furthermore, we showed that BPA1 and several BPLs functioned redundantly in plant immunity to P. capsici. We discovered that BPA1 and all six BPLs interacted with ACD11, and stabilization of ACD11 was impaired in the bpa 1, bpl2, bpl3, and bpl4 mutants. RxLR207 could promote the degradation of BPA1, BPL1, BPL2, and BPL4 to disrupt ACD11 stabilization in a 26S proteasome-dependent manner. 15Effects of Increased Night Temperature on Cellulose Synthesis and the Activity of Sucrose Metabolism Enzymes in Cotton Fiber显示文摘Temperature is one of the key factors that influence cotton fiber synthesis at the late growth stage of cotton. In this paper, using two early-maturing cotton varieties as experimental materials, night temperature increase was stimulated in the field using far-infrared quartz tubes set in semi-mobile incubators and compared with the normal night temperatures (control) in order to investigate the effects of night temperature on the cotton fiber cellulose synthesis during secondary wall thickening. The results showed that the activity of sucrose synthase (SuSy) and sucrose phosphate synthase (SPS) quickly increased and remained constant during the development of cotton fiber, while the activity of acid invertase (AI) and alkaline invertase (NI) decreased, increased night temperatures prompted the rapid transformation of sugar, and all the available sucrose fully converted into cellulose. With night temperature increasing treatment, an increase in SuSy activity and concentration of sucrose indicate more sucrose converted into UDPG (uridin diphosphate-glucose) during the early and late stages of cotton fiber development. Furthermore, SPS activity and the increased concentration of fructose accelerated fructose degradation and reduced the inhibition of fructose to SuSy; maintaining higher value of allocation proportion of invertase and sucrose during the early development stages of cotton fiber, which was propitious to supply a greater carbon source and energy for cellulose synthesis. Therefore, the minimum temperature in the nightime was a major factor correlated with the activity of sucrose metabolism enzymes in cotton fiber. 16Near infrared light triggered nitric oxide releasing platform based on upconversion nanoparticles for synergistic therapy of cancer stem-like cells显示文摘Near infrared(NIR) light-driven nitric oxide(NO) release nano-platform based on upconversion nanoparticles(UCNPs) and light sensitive NO precursor Roussin's black salt(RBS) was fabricated to generate NO upon 808 nm irradiation. The application of 808 nm laser as the excitation source could achieve better penetration depth and avoid overheating problem. The combination of UCNPs and RBS could realize the on-demand release of NO at desired time and location by simply controlling the output of NIR laser.Cellular uptake results showed that more nanoparticles were internalized in cancer stem-like cells(CSCs)rather than non-CSCs. Therefore, a synergistic cancer therapy strategy to eradicate both CSCs and nonCSCs simultaneously was developed. Traditional chemo-drug could inhibit non-CSCs but has low killing efficiency in CSCs. However, we found that the combination of NO and chemotherapy could efficiently inhibit CSCs in bulk cells, including inhibiting mammosphere formation ability, decreasing CD44^+/CD24^- subpopulation and reducing tumorigenic ability. The mechanism studies confirmed that NO could not only induce apoptosis but also increase drug sensitivity by declining drug efflux in CSCs. 17Tissue distribution and tumor uptake of folate receptor–targeted epothilone folate conjugate,BMS-753493,in CD2F1 mice after systemic administration显示文摘To assess targeting of an epothilone folate conjugate(BMS-753493) to the folate receptor(FR)-overexpressed tumor in mice bearing both FRt and FR– tumors,a series of experiments were conducted by quantitative whole-body autoradiography(QWBA) and LC–MS/MS following i.v.administration of BMS-753493 or its active moiety,BMS-748285 in mice bearing FRt(98M109) and FR–(M109) tumors.QWBA showed [3H]BMS-753493–derived radioactivity was extensively distributed to various tissues.The FR over-expressing 98M109 tumors showed consistently higher level of radioactivity than FR-negative tumors(i.e.,M109 tumors) up to 48 h post dose of [3H]BMS-753493,despite the magnitude of difference between the tumors is relatively small(generally 3 5-fold).The radioactivity level in 98M109 tumors was 2 12-fold of normal tissues except intestine/content at 48 h post dose.No selective radioactivity uptake into 98M109 tumors over M109 or normal tissues was observed after i.v.administration of the active epothilone,[3H]BMS-748285.LC–MS/MS measurements demonstrated that the concentrations of BMS-748285,presumably from hydrolysis of the folate conjugate,in 98M109 tumors were greater than those in M109 tumors after i.v.administration of BMS-753493(2–3-fold) whereas no differential uptake in the tumors following BMS-748285 administration.Those data were consistent with radioactivity determinations.Those results demonstrated that the folate conjugation in BMS-753493 enabled moderately preferential distribution of the active epothilone to FR overexpressing 98M109 tumors,thereby supporting targeted delivery of cytotoxics through the folate receptor.Hong Shen Lifei Wang Weiqi Chen Krista Menard Yang Hong Yuan Tian Samuel J.Bonacorsi W.Griffith Humphreys Francis Y.Lee Jinping Gan 2016Acta Pharmaceutica Sinica B2016,6,5:3

19Flow field design and process stability in electrochemical machining of diamond holes显示文摘The metal grille, commonly composed of an amount of diamond holes, has been growingly used as a key structure on stealth aircraft. Electrochemical machining(ECM) promises to be increasingly applied in aircraft manufacturing on the condition that process stability is guaranteed.In this work, a flow field model was designed to improve the process stability. This model is endowed with a variety of flow channel features, together with vibrating feeding modes. The flow field distribution on the bottom surface of the diamond hole was discussed and evaluated as well.The numerical results show that a short arc flow channel could significantly enhance the uniformity of electrolyte velocity distribution and a vibrating feeding of the cathode enables to reduce both fluctuations of the electrolyte velocity and pressure on the bottom surface of the diamond hole.Consequently, the flow field mutations were eliminated. It is verified from the experimental results that a short arc flow channel, when combined with vibrating feeding, is capable of improving machining localization and process stability markedly. 20Preparation of Highly Dispersed Antimony-doped Tin Oxide Nano-powder via Ion-exchange Hydrolysis of SnCl_4 and SbCl_3 and Azeotropic Drying显示文摘Antimony-doped tin hydroxide colloid precipitates have been synthesized by hydrolysis of SnCl4 and SbCl3 using: (1) an ion-exchange hydrolysis to remove chlorine ions, and (2) isoamyl acetate as an azeotropic solvent to obviate water. The obtained dried powder is of high dispersivity without any need for further grinding. The size and dispersivity of the final particles are investigated with the aid of TG-DTA, BET, XRD and TEM.
