| 1 | A matched-pair analysis of laparoscopic versus open pancreaticoduodenectomy: oncological outcomes using Leeds Pathology Protocol显示文摘BACKGROUND: Laparoscopic pancreaticoduodenectomy(LPD)is a safe procedure. Oncological safety of LPD is still a matter for debate. This study aimed to compare the oncological outcomes,in terms of adequacy of resection and recurrence rate following LPD and open pancreaticoduodenectomy(OPD).METHODS: Between November 2005 and April 2009, 12LPDs(9 ampullary and 3 distal common bile duct tumors)were performed. A cohort of 12 OPDs were matched for age,gender, body mass index(BMI) and American Society of Anesthesiologists(ASA) score and tumor site.RESULTS: Mean tumor size LPD vs OPD(19.8 vs 19.2 mm,P=0.870). R0 resection was achieved in 9 LPD vs 8 OPD(P=1.000). The mean number of metastatic lymph nodes and total number resected for LPD vs OPD were 1.1 vs 2.1(P=0.140)and 20.7 vs 18.5(P=0.534) respectively. Clavien complications grade I/II(5 vs 8), III/IV(2 vs 6) and pancreatic leak(2 vs 1)were statistically not significant(LPD vs OPD). The mean high dependency unit(HDU) stay was longer in OPD(3.7 vs 1.4 days,P<0.001). There were 2 recurrences each in LPD and OPD(logrank,P=0.983). Overall mortality for LPD vs OPD was 3 vs 6(log-rank, P=0.283) and recurrence-related mortality was 2 vs 1.There was one death within 30 days in the OPD group secondary to severe sepsis and none in the LPD group.CONCLUSIONS: Compared to open procedure, LPD achieved a similar rate of R0 resection, lymph node harvest and longterm recurrence for tumors less than 2 cm. | 2 | Inguinodynia following Lichtenstein tension-free hernia repair:A review显示文摘Chronic Groin Pain (Inguinodynia) following inguinal hernia repair is a significant,though under-reported problem. Mild pain lasting for a few days is common following mesh inguinal hernia repair. However,moderate to severe pain persisting more than 3 mo after inguinal herniorrhaphy should be considered as pathological. The major reasons for chronic groin pain have been identified as neuropathic cause due to inguinal nerve(s) damage or non-neuropathic cause due to mesh or other related factors. The symptom complex of chronic groin pain varies from a dull ache to sharp shooting pain along the distribution of inguinal nerves. Thorough history and meticulous clinical examination should be performed to identify the exact cause of chronic groin pain,as there is no single test to confirm the aetiology behind the pain or to point out the exact nerve involved. Various studies have been performed to look at the difference in chronic groin pain rates with the use of mesh vs non-mesh repair,use of heavyweight vs lightweight mesh and mesh fixation with sutures vs glue. Though there is no convincing evidence favouring one over the other,lightweight meshes are generally preferred because of their lesser foreign body reaction and better tolerance by the patients. Identification of all three nerves has been shown to be an important factor in reducing chronic groin pain,though there are no well conducted randomised studies to recommend the benefits of nerve excision vs preservation. Both nonsurgical and surgical options have been tried for chronic groin pain,with their consequent risks of analgesic sideeffects,recurrent pain,recurrent hernia and significant sensory loss. | 7 | Effect of alkaline metal ions on the photoluminescence properties of Eu^(3+)-doped Ca_3Al_2O_6 phosphors显示文摘Ca_3–xAl_2O_6:x Eu^(3+)(0.02≤x≤0.10) and Ca_(2.92)Al_2O_6:0.04Eu^(3+),0.04M^+(M^+=Li^+, Na^+, and K^+) phosphors were prepared by sol-gel method and their photoluminescence properties were investigated. The crystal structure of the annealed Ca_3–xAl_2O_6:x Eu^(3+) and Ca_(2.92)Al_2O_6:0.04Eu^(3+),0.04M^+ phosphors was analyzed with X-ray diffraction(XRD). The size and morphological characteristics of the annealed phosphors were investigated with a field emission scanning electron microscope(FE-SEM). The photoluminescence properties were evaluated with a spectrofluorometer. The phosphors were effectively excited by the light of 396 nm and showed intense red emission at 616 nm, which originated from the 5D_0→7F_2 transition of Eu^(3+). Among the prepared phosphors, the Ca_(2.96)Al_2O_6:0.04Eu^(3+) phosphor showed the strongest emission intensity and the highest color purity. | 9 | Effects of space and temperature dependent internal heat generation/absorption on MHD flow of a nanofluid over a stretching sheet显示文摘In this paper we analyzed the effects of space and temperature dependent internal heat generation/absorption(non-uniform heat source/sink) on magnetohydrodynamic boundary layer flow of water based nanofluid over a stretching sheet with different nanoparticles. The flow is generated due to linear stretching of the sheet and influenced by uniform magnetic field, which is applied normally to the stretching sheet. A scaling group of transformation is used to reduce the governing momentum and energy equations into non-linear ordinary differential equations. The resulting differential equations are solved analytically using hypergeometric functions and numerically by the fourth order Runge-Kutta method with shooting technique. The influence of nanoparticle volume fraction, magnetic field, Prandtl number, non uniform heat source/sink, local skin friction coefficient and reduced Nusselt number are investigated for different nanoparticles. | GANGA B. | 12 | Simulation of snowmelt-runoff under climate change scenarios in a data-scarce mountain environment显示文摘Pakistan is an agriculture-based economy and major proportion of irrigation water for its cultivated lands is abstracted from the Upper Indus Basin(UIB).UIB water supplies are mostly contributed from the high-altitude snow and glacier fields situated in the Hindukush–Karakoram–Himalayan ranges.Any change in the flows of these river catchments due to climate variability may result in the form of catastrophic events like floods and droughts and hence will adversely affect the economy of Pakistan.This study aims to simulate snowmelt runoff in a mountainous sub-catchment(Shyok River basin)of the UIB under climate change scenarios.Snowmelt Runoff Model(SRM)coupled with remotely sensed snow cover product(MOD10A2)is used to simulate the snowmelt runoff under current and future climate scenarios in the study area.The results indicate that(a)SRM has efficiently simulated the flow in Shyok River with average Nash–Sutcliff coefficient value(R2)of 0.8(0.63–0.93)for all six years(2000–2006)of basin-wide and zone-wise simulations,(b)an increase of 10%(by 2050)and 20%(by 2075)in SCA will result in a flow rise of∼11%and∼20%,respectively,and(c)an increase of 1℃(by 2025),2℃(by 2050),3℃(by 2075)and 4℃(by 2100)in mean temperature will result in a flow rise of∼26%,∼54%,∼81%and∼118%,respectively.This study suggests that SRM equipped with remotely sensed snow cover data is an effective tool to estimate snowmelt runoff in high mountain data-scarce environments. | Adnan Ahmad Tahir Samreen Abdul Hakeem Tiesong Hu Huma Hayat Muhammad Yasir | 2019 | International Journal of Digital Earth2019,12,8: | 1 |
| 13 | Long-term outcomes of pediatric liver transplantation in acute liver failure vs end-stage chronic liver disease:A retrospective observational study显示文摘BACKGROUND Children with acute liver failure(ALF)who meet the criteria are eligible for super-urgent transplantation,whereas children with end-stage chronic liver disease(ESCLD)are usually transplanted electively.Pediatric liver transplantation(PLT)in ALF and ESCLD settings has been well described in the literature,but there are no studies comparing the outcomes in these two groups.AIM To determine if there is a difference in post-operative complications and survival outcomes between ALF and ESCLD in PLT.METHODS This was a retrospective observational study of all primary PLTs performed at a single center between 2000 and 2019.ALF and ESCLD groups were compared for pretransplant recipient,donor and operative parameters,and post-operative outcomes including graft and patient survival.RESULTS Over a 20-year study period,232 primary PLTs were performed at our center;195 were transplanted for ESCLD and 37 were transplanted for ALF.The ALF recipients were significantly older(median 8 years vs 5.4 years;P=0.031)and heavier(31 kg vs 21 kg;P=0.011).Living donor grafts were used more in the ESCLD group(34 vs 0;P=0.006).There was no difference between the two groups concerning vascular complications and rejection,but there were more bile leaks in the ESCLD group.Post-transplant patient survival was significantly higher in the ESCLD group:1-,5-,and 10-year survival rates were 97.9%,93.9%,and 89.4%,respectively,compared to 78.3%,78.3%,and 78.3%in the ALF group(P=0.007).However,there was no difference in 1-,5-,and 10-year graft survival between the ESCLD and ALF groups(90.7%,82.9%,77.3%vs 75.6%,72.4%,and 66.9%;P=0.119).CONCLUSION Patient survival is inferior in ALF compared to ESCLD recipients;the main reason is death in the 1st year post-PLT in ALF group.Once the ALF children overcome the 1st year after transplant,their survival stabilizes,and they have good long-term outcomes. | Amr M Alnagar Abdul R Hakeem Khaled Daradka Eirini Kyrana Marumbo Methga KarthikeyanPalaniswamy Sanjay Rajwal Jamila Mulla Moira O'meara Vivek Upasani Dhakshinamoorthy Vijayanand Raj Prasad Magdy S Attia | 2023 | World Journal of Transplantation2023,13,3: | 1 |
