| 2 | High-resolution microendoscopy for esophageal cancer screening in China: A cost-effectiveness analysis显示文摘AIM:To study the cost-effectiveness of high-resolution microendoscopy(HRME)in an esophageal squamous cell carcinoma(ESCC)screening program in China.METHODS:A decision analytic Markov model of ESCC was developed.Separate model analyses were conducted for cohorts consisting of an averagerisk population or a high-risk population in China.Hypothetical 50-year-old individuals were followed until age 80 or death.We compared three different strategies for both cohorts:(1)no screening;(2)standard endoscopic screening with Lugol’s iodine staining;and(3)endoscopic screening with Lugol’s iodine staining and an HRME.Model parameters were estimated from the literature as well as from GLOBOCAN,the Cancer Incidence and Mortality Worldwide cancer database.Health states in the model included non-neoplasia,mild dysplasia,moderate dysplasia,high-grade dysplasia,intramucosal carcinoma,operable cancer,inoperable cancer,and death.Separate ESCC incidence transition rates were generated for the average-risk and high-risk populations.Costs in Chinese currency were converted to international dollars(I$)and were adjusted to 2012dollars using the Consumer Price Index.RESULTS:The main outcome measurements for this study were quality-adjusted life years(QALYs)and incremental cost-effectiveness ratio(ICER).For the average-risk population,the HRME screening strategy produced 0.043 more QALYs than the no screening strategy at an additional cost of I$646,resulting in an ICER of I$11808 per QALY gained.Standard endoscopic screening was weakly dominated.Among the high-risk population,when the HRME screening strategy was compared with the standard screening strategy,the ICER was I$8173 per QALY.For both the high-risk and average-risk screening populations,the HRME screening strategy appeared to be the most cost-effective strategy,producing ICERs below the willingness-topay threshold,I$23500 per QALY.One-way sensitivity analysis showed that,for the average-risk population,higher specificity of Lugol’s iodine(>40%)and lower specificity of HRME(<70%)could make Lugol’s iodine screening cost-effective.For the high-risk population,the results of the model were not substantially affected by varying the follow-up rate after Lugol’s iodine screening,Lugol’s iodine test characteristics(sensitivity and specificity),or HRME specificity.CONCLUSION:The incorporation of HRME into an ESCC screening program could be cost-effective in China.Larger studies of HRME performance are needed to confirm these findings. | Chin Hur Sung Eun Choi Chung Yin Kong Gui-Qi Wang Hong Xu Alexandros D Polydorides Li-Yan Xue Katherine E Perzan Angela C Tramontano Rebecca R Richards-Kortum Sharmila Anandasabapathy | 2015 | World Journal of Gastroenterology2015,21,18: | 8 |
| 3 | Murine lung eosinophil activation and chemokine production in allergic airway inflammation显示文摘Eosinophils play important roles in asthma and lung infections.Murine models are widely used for assessing the functional significance and mechanistic basis for eosinophil involvements in these diseases.However,little is known about tissue eosinophils in homeostasis.In addition,little data on eosinophil chemokine production during allergic airway inflammation are available.In this study,the properties and functions of homeostatic and activated eosinophils were compared.Eosinophils from normal tissues expressed costimulation and adhesion molecules B7-1,B7-2 and ICAM-1 for Ag presentation but little major histocompatibility complex(MHC)class II,and were found to be poor stimulators of T-cell proliferation.However,these eosinophils expressed high levels of chemokine mRNA including C10,macrophage inflammatory protein(MIP)-1a,MIP-1c,MIP-2,eotaxin and monocyte chemoattractant protein-5(MCP-5),and produced chemokine proteins.Eosinophil intracellular chemokines decreased rapidly with concomitant surface marker downregulation upon in vitro culturing consistent with piecemeal degranulation.Lung eosinophils from mice with induced allergic airway inflammation exhibited increased chemokines mRNA expression and chemokines protein production and upregulated MHC class II and CD11c expression.They were also found to be the predominant producers of the CCR1 ligands CCL6/C10 and CCL9/MIP-1c in inflamed lungs.Eosinophil production of C10 and MIP-1c correlated with the marked influx of CD11bhigh lung dendritic cells during allergic airway inflammation and the high expression of CCR1 on these dendritic cells(DCs).The study provided baseline information on tissue eosinophils,documented the upregulation of activation markers and chemokine production in activated eosinophils,and indicated that eosinophils were a key chemokine-producing cell type in allergic lung inflammation. | C Edward Rose Jr Joanne A Lannigan Paul Kim James J Lee Shu Man Fu Sun-sang J Sung | 2010 | Cellular & Molecular Immunology2010,7,5: | 6 |
| 4 | Role of stem cell therapies in treating chronic wounds:A systematic review显示文摘BACKGROUND The impairment of cutaneous wound healing results in chronic,non-healing wounds that are caused by altered wound environment oxygenation,tissue injury,and permissive microbial growth.Current modalities for the treatment of these wounds inadequately address the complex changes involved in chronic wound pathogenesis.Consequently,stem cell therapies have emerged as a potential therapeutic modality to promote cutaneous regeneration through trophic and paracrine activity.AIM To investigate current literature regarding use of stem cell therapies for the clinical treatment of chronic,non-healing wounds.METHODS PubMed,EMBASE,Cochrane Library,Web of Science,and Scopus were queried with combinations of the search terms“mesenchymal stem cells,”“adult stem cells,”“embryonic stem cells,”“erythroid precursor cells,”“stem cell therapies,”and“chronic wounds”in order to find relevant articles published between the years of 2000 and 2019 to review a 20-year experience.Reference lists from the articles were reviewed to identify additional pertinent articles.Retrieved manuscripts(reviews,case reports/series,retrospective/prospective studies,and clinical trials)were evaluated by the authors for their depiction of clinical stem cell therapy use.Data were extracted from the articles using a standardized collection tool.RESULTS A total of 43 articles describing the use of stem cell therapies for the treatment of chronic wounds were included in this review.While stem cell therapies have been explored in in vitro and in vivo applications in the past,recent efforts are geared towards assessing their clinical role.A review of the literature revealed that adipose-derived stem cells,bone marrow-derived stem cells,bone marrowderived mononuclear cells,epidermally-derived mesenchymal stem cells,fibroblast stem cells,keratinocyte stem cells,placental mesenchymal stem cells,and umbilical cord mesenchymal stem cells have all been employed in the treatment of chronic wounds of various etiologies.Most recently,embryonic stem cells have emerged as a novel stem cell therapy with the capacity for multifaceted germ cell layer differentiation.With the capacity for self-renewal and differentiation,stem cells can enrich existing cell populations in chronic wounds in order to overcome barriers impeding the progression of wound healing.Further,stem cell therapies can be utilized to augment cell engraftment,signaling and activity,and resultant patient outcomes.CONCLUSION Assessing observed clinical outcomes,potential for stem cell use,and relevant therapeutic challenges allows wound care stakeholders to make informed decisions regarding optimal treatment approaches for their patients’chronic wounds. | Anjali C Raghuram Roy P Yu Andrea Y Lo Cynthia J Sung Melissa Bircan Holly J Thompson Alex K Wong | 2020 | World Journal of Stem Cells2020,12,7: | 5 |

| 6 | Evaluation of the string test for the detection of Helicobacter pylori显示文摘AIM: Helicobacter pylori can be diagnosed by invasive ornon-invasive tests but to obtain bacteria for culture andantibiotic susceptibility testing, an upper GI endoscopy isoften required. The string test may be a minimally-invasivealternative method of obtaining H. pylori samples. This studyevaluates the sensitivity and specificity of the string test inthe diagnosis of H. pylori in comparison with endoscopicmeans of diagnosis.METHODS: This was a prospective open comparative studyof patients with dyspepsia with endoscopy-based tests asgold standard (defined as a positive CLO test and antralhistology). Fasting patients swallowed the encapsulated-string(Entero-test Hp), which was withdrawn after 1 hour. Thegastric juice from the string was plated onto H. pylori-selectivemedia for culture. Helicobacter pylori was identified by typicalcolony morphology, gram stain and biochemical test results.RESULTS: Thirty dyspeptic patients were recruited of whom21 (70 %) were positive for H. pylori according to the goldstandard.
