| 2 | New-onset diabetes mellitus after kidney transplantation:Current status and future directions显示文摘A diagnosis of new-onset diabetes after transplantation(NODAT) carries with it a threat to the renal allograft,as well as the same short-and long-term implications of type 2 diabetes seen in the general population.NODAT usually occurs early after transplantation,and is usually diagnosed according to general population guidelines.Non-modifiable risk factors for NODAT include advancing age,African American,Hispanic,or South Asian ethnicity,genetic background,a positive family history for diabetes mellitus,polycystic kidney disease,and previously diagnosed glucose intolerance.Modifiable risk factors for NODAT include obesity and the metabolic syndrome,hepatitis C virus and cytomegalovirus infection,corticosteroids,calcineurin inhibitor drugs(especially tacrolimus),and sirolimus.NODAT affects graft and patient survival,and increases the incidence of post-transplant cardiovascular disease.The incidence and impact of NODAT can be minimized through pre-and post-transplant screening to identify patients at higher risk,including by oral glucose tolerance tests,as well as multi-disciplinary care,lifestyle modification,and the use of modified immunosuppressive regimens coupled with glucose-lowering therapies including oral hypoglycemic agents and insulin.Since NODAT is a major cause of post-transplant morbidity and mortality,measures to reduce its incidence and impact have the potential to greatly improve overall transplant success. | Sneha Palepu G V Ramesh Prasad | 2015 | World Journal of Diabetes2015,6,3: | 8 |
| 3 | Burden of care in caregivers of patients with alcohol use disorder and schizophrenia and its association with anxiety, depression and quality of life显示文摘Background Caregivers play a pivotal role in providing care for mentally ill patients.Increase in caregiver burden can make them vulnerable to mental illness themselves.Aims We assessed the severity of burden of care and its association with depression,anxiety and quality of life among caregivers of patients with alcohol use disorder(AUD)and schizophrenia.Methods This was an observational,cross-sectional,single-centred study of 50 consecutive caregivers of patients with AUD and schizophrenia.Participants were recruited from the psychiatry outpatient department of a tertiary care hospital between January and June 2017.The caregivers were further assessed by demographic details,Hospital Anxiety Depression Scale for assessment of depression and anxiety,Zarit Burden Interview for assessment of caregiver burden and WHO Quality Of Life-BREF for assessment of quality of life.Statistical data were analysed using GraphPad InStat V.3.06(California).Multiple linear regression analysis was applied to identify the predictors of caregiver burden.Results Burden of care experienced by caregivers of patients with AUD is as high as that of caregivers of patients with schizophrenia(U=1142.5,p=0.46).Caregivers experiencing high burden of care are likely to have symptoms of anxiety(U=22,p<0.001),depression(U=32,p<0.001)and poor quality of life(U=84.5,p<0.001).Female caregivers are likely to experience higher burden of care(U=819.5,p=0.006).For caregivers of patients with schizophrenia,anxiety,environmental health,socioeconomic status and patients,occupation can predict burden of care,while for caregivers of patients with AUD,depression and environmental health can predict burden of care.Conclusion Our study suggests that caregivers of patients with AUD experience burden of care as high as that of caregivers of patients with schizophrenia.Caregivers with high burden of care are more likely to have depression,anxiety and poor quality of life.Trial registration number CTRI/2017/03/008224. | Sneha Vadher Rishi Desai Bharat Pancha Ashok Vala Imran Jahangirali Ratnani Mihir P Rupani Kinjal Vasava | 2020 | General Psychiatry2020,33,4: | 7 |
| 4 | Cellulose based polymers in development of amorphous solid dispersions显示文摘Cellulose derivatives have gained immense popularity as stabilizers for amorphous solid dispersion owing to their diverse physicochemical properties. More than 20 amorphous solid dispersion-based products that have been approved for marketing consist of cellulose derivatives as stabilizers, thus highlighting their importance in generation of amorphous solid dispersions. These polymers offer numerous advantages like drug solubilization, crystallization inhibition and improvement in release patterns of drugs. Exploring their potential and exploiting their chemistry and p H responsive behaviour have led to the synthesis of new derivatives that has broadened the scope of the use of cellulose derivatives in amorphous formulation development. The present review aims to provide an overview of different mechanisms by which these cellulose derivatives inhibit the crystallization of drugs in the solid state and from supersaturated solution. A summary of different categories of cellulose derivatives along with the newly explored polymers has been provided. Humongous progress in the treatment modalities available and the increasing need to provide functional information in addition to the morphological data; has led to leaping progress in the field of imaging. Magnetic resonance imaging has undergone tremendous progress with various newer MR techniques providing vital functional information and is becoming the cornerstone of 'radiomics/radiogenomics'. Diffusionweighted imaging is one such technique which capitalizes on the tendency of water protons to diffuse randomly in a given system. This technique has revolutionized oncological imaging, by giving vital qualitative and quantitative information regarding tumor biology which helps in detection, characterization and post treatment surveillance of the lesions and challenging the notion that 'one size fits all'. It has been applied at various sites with different clinical experience. | 7 | Is colonic lavage a suitable alternative for left-sided colonic emergencies?显示文摘BACKGROUND The use of intra-operative colonic lavage(IOCL)with primary anastomosis remains controversial in the emergency left-sided large bowel pathologies,with alternatives including Hartmann’s procedure,manual decompression and subtotal colectomy.AIM To compare the peri-operative outcomes of IOCL to other procedures.METHODS Electronic databases were searched for articles employing IOCL from inception till July 13,2020.Odds ratio and weighted mean differences(WMD)were estimated for dichotomous and continuous outcomes respectively.Single-arm meta-analysis was conducted using DerSimonian and Laird random effects.RESULTS Of 28 studies were included in this meta-analysis,involving 1142 undergoing IOCL,and 634 other interventions.IOCL leads to comparable rates of wound infection when compared to Hartmann’s procedure,and anastomotic leak and wound infection when compared to manual decompression.There was a decreased length of hospital stay(WMD=-7.750;95%CI:-13.504 to-1.996;P=0.008)compared to manual decompression and an increased operating time.Single-arm meta-analysis found that overall mortality rates with IOCL was 4%(CI:0.03-0.05).Rates of anastomotic leak and wound infection were 3%(CI:0.02-0.04)and 12%(CI:0.09-0.16)respectively.CONCLUSION IOCL leads to similar rates of post-operative complications compared to other procedures.More extensive studies are needed to assess the outcomes of IOCL for emergency left-sided colonic surgeries. | Hui Yu Tham Wen Hui Lim Sneha Rajiv Jain Cheng Han Mg Snow Yunni Lin Jie Ling Xiao Fung Joon Foo Kar Yong Wong Choon Seng Chong | 2021 | World Journal of Gastrointestinal Surgery2021,13,4: | 3 |
| 8 | High Risk Factors of Pancreatic Carcinoma显示文摘Over the past decades, cancer has become one of the toughest challenges for health professionals. The epidemiologists are increasingly directing their research efforts on various malignant tumor worldwide. Of note, incidence of cancers is on the rise more quickly in developed countries. Indeed, great endeavors have to be made in the control of the life-threatening disease. As we know it, pancreatic cancer(PC) is a malignant disease with the worst prognosis. While little is known about the etiology of the PC and measures to prevent the condition, so far, a number of risk factors have been identified. Genetic factors, pre-malignant lesions, predisposing diseases and exogenous factors have been found to be linked to PC. Genetic susceptibility was observed in 10% of PC cases, including inherited PC syndromes and familial PC. However, in the remaining 90%, their PC might be caused by genetic factors in combination with environmental factors. Nonetheless, the exact mechanism of the two kinds of factors, endogenous and exogenous, working together to cause PC remains poorly understood. The fact that most pancreatic neoplasms are diagnosed at an incurable stage of the disease highlights the need to identify risk factors and to understand their contribution to carcinogenesis. | 10 | Study of cyanide removal from contaminated water using zinc peroxide nanomaterial显示文摘The present study highlights the potential application of zinc peroxide(ZnO_2)nanomaterial as an efficient material for the decontamination of cyanide from contaminated water. A process patent for ZnO_2 synthesis has been granted in United States of America(US Patent number 8,715,612; May 2014),South Africa,Bangladesh,and India. The ZnO_2 nanomaterial was capped with polyvinylpyrrolidone(PVP)to control the particle size. The PVP capped ZnO_2nanomaterial(PVP-ZnO_2)before and after adsorption of cyanide was characterized by scanning electron microscope,transmission electron microscope,X-ray diffractometer,Fourier transform infrared spectroscopy and time of flight-secondary ion mass spectrometry. The remaining concentration of cyanide after adsorption by PVP-ZnO_2 was determined using ion chromatograph. The adsorption of cyanide over PVP-ZnO_2 was also studied as a function of p H,adsorbent dose,time and concentration of cyanide. The maximum removal of cyanide was observed in p H range 5.8–7.8 within 15 min. The adsorption data was fitted to Langmuir and Fruendlich isotherm and it has been observed that data follows both the isotherms and also follows second order kinetics. | Himani Uppal S. | 14 | Preparation of NiFe_2O_4 nanopowder via EDTA precursor and study of its properties显示文摘An ethylene diamine tetra acetic acid (EDTA) precursor-based chemical method is reported for preparing single-phase nickel ferrite (NiFe 2 O 4 ) nanopowder. The synthesized powder was characterized by thermogravimetric analysis, differential scanning calorimetric analysis, X-ray diffraction, particle size analysis and scanning electron microscopy. DC electrical resistivity and magnetic property of the syn- thesized NiFe 2 O 4 were measured by using a two-probe method and a vibrating sample magnetometer respectively. The DC resistivity behaviour of the NiFe 2 O 4 nanopowder was correlated with the change of microstructure, during sintering.
