| 2 | Gastrointestinal sarcoidosis associated with pneumatosis cystoides intestinalis显示文摘A 39-year-old male reported fevers,weight loss,watery loose stools,and decreased visual acuity in his right eye over the prior five years.He was pancytopenic,had an elevated American council on exercise level,total bilirubin,and alkaline phosphatase.Computed tomography revealed massive hepatosplenomegaly and emphysematous lung changes.Liver biopsy showed non caseating granulomas.The patient was diagnosed with extrapulmonary sarcoidosis and was treated with prednisone.The patient symptomatically improved but 5 mo later presented with abdominal pain caused by perforation of the cecum.He underwent a cecectomy and pathology revealed pneumatosis cystoides intestinalis.This represents the first reported association between pneumatosis cystoides intestinalis and sarcoidosis.The etiology of pneumatosis cystoides intestinalis in this case was likely multifactorial and involved both effects of the corticosteroids as well as the advanced nature of the gastrointestinal sarcoidosis.Furthermore this case has the unique features of emphysematous lung changes and pancytopenia which are uncommon with sarcoidosis. | Hussein Rahim Mubashir Khan Jay Hudgins Kevin Lee Lei Du Louis Amorosa | 2013 | World Journal of Gastroenterology2013,19,7: | 3 |
| 3 | DNNBoT: Deep Neural Network-Based Botnet Detection and Classification显示文摘The evolution and expansion of IoT devices reduced human efforts,increased resource utilization, and saved time;however, IoT devices createsignificant challenges such as lack of security and privacy, making them morevulnerable to IoT-based botnet attacks. There is a need to develop efficientand faster models which can work in real-time with efficiency and stability. The present investigation developed two novels, Deep Neural Network(DNN) models, DNNBoT1 and DNNBoT2, to detect and classify well-knownIoT botnet attacks such as Mirai and BASHLITE from nine compromisedindustrial-grade IoT devices. The utilization of PCA was made to featureextraction and improve effectual and accurate Botnet classification in IoTenvironments. The models were designed based on rigorous hyperparameterstuning with GridsearchCV. Early stopping was utilized to avoid the effects ofoverfitting and underfitting for both DNN models. The in-depth assessmentand evaluation of the developed models demonstrated that accuracy andefficiency are some of the best-performed models. The novelty of the presentinvestigation, with developed models, bridge the gaps by using a real datasetwith high accuracy and a significantly lower false alarm rate. | 4 | Development of bis-thiobarbiturates as successful urease inhibitors and their molecular modeling studies显示文摘Bis-thiobarbiturate derivatives 1–15 have been synthesized, characterized by1 HNMR and EI-MS and screened for urease inhibition. All compounds showed various degree of urease inhibitory activity with IC_(50) values ranging 7.45 0.12 74.24 0.81 mmol/L while the standard thiourea behaved normally(IC_(50) = 21.10 0.12). Compounds 1(IC_(50) = 7.45 0.12 μmol/L), 9(IC_(50) = 18.17 1.03 μmol/L) and 13(IC_(50) = 8.61 0.45 μmol/L) showed excellent urease inhibitory activity in the series. Molecular modeling studies were performed to understand the binding site with the bimetallic nickel center of the enzyme.Structure-activity relationship has also been established for these compounds. | 5 | 临床检查可预测下肢周围动脉疾病?显示文摘Context: Lower extremity peripheral arterial disease(PAD) is common and associated with significant increases in morbidity and mortality. Physicians typically depend on the clinical examination to identify patients who need further diagnostic testing. Objective: To systematically review the accuracy and precision of the clinical examination for PAD. Data Sources, Study Selection, and Data Extraction: MEDLINE(January 1966 to March 2005) and Cochrane databases were searched for articles on the diagnosis of PAD based on physical examination published in the English language. Included studies compared an element of the history or physical examination with a reference standard of ankle- brachial index, duplex sonography, or angiogram. Seventeen of the 51 potential articles identified met inclusion criteria. Two of the authors independently extracted data, performed quality review, and used consensus to resolve any discrepancies. Data Synthesis: For asymptomatic patients, the most useful clinical findings to diagnose PAD are the presence of claudication(likelihood ratio [LR], 3.30; 95% confidence interval [CI], 2.30- 4.80), femoral bruit(LR, 4.80; 95% CI, 2.40- 9.50), or any pulse abnormality(LR, 3.10; 95% CI, 1.40- 6.60). While none of the clinical examination features help to lower the likelihood of any degree of PAD, the absence of claudication or the presence of normal pulses decreases the likelihood of moderate to severe disease. When considering patients who are symptomatic with leg complaints, the most useful clinical findings are the presence of cool skin(LR, 5.90; 95% CI, 4.10- 8.60), the presence of at least 1 bruit(LR, 5.60; 95% CI, 4.70- 6.70), or any palpable pulse abnormality(LR, 4.70; 95% CI, 2.20- 9.90). The absence of any bruits(iliac, femoral, or popliteal)(LR, 0.39; 95% CI, 0.34- 0.45) or pulse abnormality(LR, 0.38; 95% CI, 0.23- 0.64) reduces the likelihood of PAD. Combinations of physical examination findings do not increase the likelihood of PAD beyond that of individual clinical findings. However, when combinations of clinical findings are all normal, the likelihood of disease is lower than when individual symptoms or signs are normal. A PAD scoring system, which includes auscultation of arterial components by handheld Doppler, provides greater diagnostic accuracy. Conclusions: Clinical examination findings must be used in the context of the pretest probability because they are not independently sufficient to include or exclude a diagnosis of PAD with certainty. | 14 | First report on molecular characterization of Leishmania species from cutaneous leishmaniasis patients in southern Khyber Pakhtunkhwa province of Pakistan显示文摘Objective: To report presence of Leishmania major in Khyber Pakhtunkhwa of Pakistan, where cutaneous leishmaniasis(CL) is endemic and was thought to be caused by Leishmania tropica only. Methods: Biopsy samples from 432 CL suspected patients were collected from 3 southern districts of Khyber Pakhtunkhwa during years 2011–2016. Microscopy on Giemsa stained slides were done followed by amplification of the ribosomal internal transcribed spacer 1 gene. Results: Leishmania amastigotes were detected by microscopy in 308 of 432 samples(71.3%) while 374 out of 432 samples(86.6%) were positive by ribosomal internal transcribed spacer 1 PCR. Subsequent restriction fragment length polymorphism confirmed Leishmania tropica in 351 and Leishmania major in 6 biopsy samples. Conclusions: This study is the first molecular characterization of Leishmania species in southern Khyber Pakhtunkhwa. It confirmed the previous assumptions that anthroponotic CL is the major CL form present in Khyber Pakhtunkhwa province. | 20 | Machine Learning-Enabled Communication Approach for the Internet of Medical Things显示文摘The Internet ofMedical Things(IoMT)is mainly concernedwith the efficient utilisation of wearable devices in the healthcare domain to manage various processes automatically,whereas machine learning approaches enable these smart systems to make informed decisions.Generally,broadcasting is used for the transmission of frames,whereas congestion,energy efficiency,and excessive load are among the common issues associated with existing approaches.In this paper,a machine learning-enabled shortest path identification scheme is presented to ensure reliable transmission of frames,especially with the minimum possible communication overheads in the IoMT network.For this purpose,the proposed scheme utilises a well-known technique,i.e.,Kruskal’s algorithm,to find an optimal path from source to destination wearable devices.Additionally,other evaluation metrics are used to find a reliable and shortest possible communication path between the two interested parties.Apart from that,every device is bound to hold a supplementary path,preferably a second optimised path,for situations where the current communication path is no longer available,either due to device failure or heavy traffic.Furthermore,the machine learning approach helps enable these devices to update their routing tables simultaneously,and an optimal path could be replaced if a better one is available.The proposed mechanism has been tested using a smart environment developed for the healthcare domain using IoMT networks.Simulation results show that the proposed machine learning-oriented approach performs better than existing approaches where the proposed scheme has achieved the minimum possible ratios,i.e.,17%and 23%,in terms of end to end delay and packet losses,respectively.Moreover,the proposed scheme has achieved an approximately 21%improvement in the average throughput compared to the existing schemes. | Rahim Khan Abdullah Ghani Samia Allaoua Chelloug Mohammed Amin Aamir Saeed Jason Teo | 2023 | Computers, Materials & Continua2023,76,8: | 0 |
