| 3 | 牙支持式全瓷与金属烤瓷固定修复体生存率和并发症发生率的循证医学评价显示文摘目的:本文通过1:3腔循证医学对相关医学证据进行评价分析,比较口腔固定修复体(fixedd ental prostheses,FDPs)中全瓷固定修复体与烤瓷固定修复体的生存率和并发症发生率,确定全瓷固定修复体是否与烤瓷固定修复体相当或者优于烤瓷固定修复体,以此解决口腔修复中常见临床问题以及为临床医师决定修复方案时提供理论依据。材料和方法:用6s检索策略进行相关的检索。检索结果中没有找到相关的支持系统或总结。在Evidence-Based Dentistry杂志中没有检索到相关摘要,Trip数据库中检索到3篇摘要,均舍弃掉,Cochrane数据库中检索到3篇系统评价,也均舍弃掉,在MEDLINEOVID中检索到6篇系统评价,本课题只利用其中1篇,Embase中没有检索到相关系统评价。通过所选择的系统评价对全瓷和烤瓷固定修复体的生存率和并发症发生率进行评估。另外,本文还增加了一个相关的前瞻性队列研究并进行了评价。结果:该系统评价论述了一个备受关注的临床问题,但因其搜索系统性不强,纳入研究的方法以及对影响结果的研究特点不明确,导致了其内部效度降低。外部适用性受限于降低的内部效度,广义的指标定义,不精确的结果和不完善的指标检测对外部适用性造成了一定的影响。然而该结果也能被选择性应用到临床实践中。对并发症发生率和5年生存率设置95%置信区间进行计算,发现烤瓷固定修复体的生存率远高于全瓷固定修复体,全瓷固定修复体有较高的技术失败率。前瞻性队列研究通过选择适当的患者及其相同的基线水平和治疗方式达到良好的内部有效性,从而解决了一个重点突出的临床问题。该研究比较了烤瓷固定修复体在种植治疗引入前和引入后的10年Kaplan—Meier累积生存率,发现烤瓷固定修复体的生存率较高,且因为种植治疗的引入以及结构不良基牙的减少使用而大大提高。结论:系统评价和前瞻性队列研究的结果是互补的:烤瓷固定修复体有较高成活率,其5年生存率远远高于全瓷固定修复体。两种固定修复体在并发症方面的差异还不清楚,但有证据表明,烤瓷固定修复体并发症的发生率要低于全瓷固定修复体。这方面的证据可以直接用于临床,且有助于指导临床决策。 | Danielle Layton 廖维立(译) 李彦(审) | 2014 | 中国口腔医学继续教育杂志2014，，1： | 2 |
