| 9 | 中国社区卫生服务该怎样满足患者需求:一项全国性的代表性调查显示文摘目的中国自2009年起开展社区卫生服务,截至2014年已建成社区卫生服务中心8 669家,拥有社区卫生服务工作者30万人。然而,至社区卫生服务中心就诊的患者数并不多,因为人们对全科医生缺乏信任,但对专科医生过度依赖。方法于2015年9—12月,采用分层随机抽样法,按社区卫生服务中心在城市和郊区的分布比例(2∶1)在中国东、中、西部地区抽取样本社区卫生服务中心,并采用不同问卷对在其工作的资深临床医生和全科医生进行调查。两套问卷内容均涉及所在社区卫生服务中心的服务人口特征、开办的临床和健康教育活动情况。158例资深临床医生和3 580例全科医生对调查进行了有效应答,问卷有效应答率分别为84%和86%。结果社区卫生服务中心的中位医生数为8例、中位护士数为13例,但仅半数医生注册为全科医生,极少数护士接受过专业的全科培训。98%的社区卫生服务中心配有听诊器,97%配有血压计,但仅43%配有检眼镜,64%配有妇科检查仪器。社区卫生服务中心倾向于有选择地向部分慢性病患者提供医疗服务。全科医生每天接诊的中位患者数为12.5例。多因素分析结果显示,在举办主体为私立医院的社区卫生服务中心、配有药剂师和护士的社区卫生服务中心,全科医生的每日接诊患者数较多。结论中国拥有规模较大的、以年轻人为主的全科医疗服务团队,并配有持续性专业培训;社区卫生服务中心配有基础医疗设备,但仅少数可为患者提供多种常见病的综合性医疗服务;社区卫生服务中心就诊患者数仍较少。 | WONG W C W JIANG S ONG J J 本刊编辑部 | 2017 | 中国全科医学2017,20,19: | 6 |
| 10 | Multiphase computed tomography radiomics of pancreatic intraductal papillary mucinous neoplasms to predict malignancy显示文摘BACKGROUND Intraductal papillary mucinous neoplasms(IPMNs)are non-invasive pancreatic precursor lesions that can potentially develop into invasive pancreatic ductal adenocarcinoma.Currently,the International Consensus Guidelines(ICG)for IPMNs provides the basis for evaluating suspected IPMNs on computed tomography(CT)imaging.Despite using the ICG,it remains challenging to accurately predict whether IPMNs harbor high grade or invasive disease which would warrant surgical resection.A supplementary quantitative radiological tool,radiomics,may improve diagnostic accuracy of radiological evaluation of IPMNs.We hypothesized that using CT whole lesion radiomics features in conjunction with the ICG could improve the diagnostic accuracy of predicting IPMN histology.AIM To evaluate whole lesion CT radiomic analysis of IPMNs for predicting malignant histology compared to International Consensus Guidelines.METHODS Fifty-one subjects who had pancreatic surgical resection at our institution with histology demonstrating IPMN and available preoperative CT imaging were included in this retrospective cohort.Whole lesion semi-automated segmentation was performed on each preoperative CT using Healthmyne software(Healthmyne,Madison,WI).Thirty-nine relevant radiomic features were extracted from each lesion on each available contrast phase.Univariate analysis of the 39 radiomics features was performed for each contrast phase and values were compared between malignant and benign IPMN groups using logistic regression.Conventional quantitative and qualitative CT measurements were also compared between groups,viaχ2(categorical)and Mann Whitney U(continuous)variables.RESULTS Twenty-nine subjects(15 males,age 71±9 years)with high grade or invasive tumor histology comprised the'malignant'cohort,while 22 subjects(11 males,age 70±7 years)with low grade tumor histology were included in the'benign'cohort.Radiomic analysis showed 18/39 precontrast,19/39 arterial phase,and 21/39 venous phase features differentiated malignant from benign IPMNs(P<0.05).Multivariate analysis including only ICG criteria yielded two significant variables:thickened and enhancing cyst wall and enhancing mural nodule<5 mm with an AUC(95%CI)of 0.817(0.709-0.926).Multivariable post contrast radiomics achieved an AUC(95%CI)of 0.87(0.767-0.974)for a model including arterial phase radiomics features and 0.834(0.716-0.953)for a model including venous phase radiomics features.Combined multivariable model including conventional variables and arterial phase radiomics features achieved an AUC(95%CI)of 0.93(0.85-1.0)with a 5-fold cross validation AUC of 0.90.CONCLUSION Multi-phase CT radiomics evaluation could play a role in improving predictive capability in diagnosing malignancy in IPMNs.Future larger studies may help determine the clinical significance of our findings. | Stuart L Polk Jung W Choi Melissa J McGettigan Trevor Rose Abraham Ahmed Jongphil Kim Kun Jiang Yoganand Balagurunathan Jin Qi Paola T Farah Alisha Rathi Jennifer B Permuth Daniel Jeong | 2020 | World Journal of Gastroenterology2020,26,24: | 5 |

| 13 | 术中神经电生理监测在单椎板入路选择性神经后根离断术中的作用显示文摘目的单椎板入路选择性神经后根离断术(SL-SDR)治疗痉挛性脑瘫完全依赖术中神经电生理监测结果。本研究旨在讨论SL-SDR术中肌电图判读对神经根选择和离断的影响及作用。方法回顾分析2016年5月至2019年3月在上海交通大学附属儿童医院神经外科采用改良神经电生理监测离断方案指导下SL-SDR治疗的痉挛性脑瘫患儿的临床资料,重点关注术中肌电图判读及其与术前临床表现的相关性,以及比较采用不同神经电生理监测离断方案解读术中肌电图时的神经后根选择差异。结果共纳入318例患儿,男性231例、女性87例,平均年龄5.9岁,32例为偏瘫、161例为双侧瘫、125例为四肢瘫。每例患儿有2~8组目标肌群(术前肌张力≥2级)。术中神经电生理监测结果显示,探测的21728枚神经小根[(68.3±8.2)枚/例]中6272枚(28.9%)与肛门括约肌相关,15456枚[(48.6±7.6)枚/例]与下肢肌群相关,其中,神经后根11009枚[(34.6±7.4)枚/例]。共有3370枚[(10.6±4.7)枚/例]符合改良神经后根离断方案,其中3061枚50%部分离断、309枚75%部分离断。术前粗大运动功能分级(GMFCS)分级Ⅰ~Ⅴ级的离断率(部分离断的神经后根数/下肢肌群相关神经后根数)分别为15.8%、22.3%、33.4%、41.8%和45.7%。75%部分离断的离断率随术前GMFCS分级的递增而增加,分别为1.5%、4.8%、8.5%、14.1%和15.2%。术后3周,2068组目标肌群肌张力显著降低[(1.7±0.5)级对(2.7±0.6)级]。术前GMFCS分级Ⅰ和Ⅱ级患儿采用传统离断方案约有20%的病例无法识别任何1枚约20%需离断的神经后根,而随着患儿术前GMFCS分级的递增,传统与改良方案离断的符合率逐步提高,分别为39.5%、41.3%、52.2%、54.1%和62.8%。结论由于SL-SDR完全依赖于术中肌电图判读,使得术中神经电生理监测显得至关重要。改良神经电生理监测离断方案指导下的SL-SDR可以安全、有效治疗痉挛性脑瘫。SL-SDR术中肌电图及其与临床表现相关的预后,可以使临床医师更好地理解此类患儿脊髓神经元环路的运作模式。 | Xiao B Constatntini S Browd SR Zhan Q Jiang W Mei R 詹琪佳(编译) | 2020 | 中国现代神经疾病杂志2020,20,4: | 4 |

| 18 | Influence of Fe Addition on Products of Self-Propagating High-Temperature Synthesis Reaction in 3Ti-Si-2C System显示文摘Influence of Fe addition on products of self-propagating high-temperature synthesis (SHS) reaction in 3Ti-Si-2C system was investigated in the present study. Without Fe addition, Ti5Si3 and TiC are the dominant phases along with a small amount of Ti3SiC2 phase and unreacted C left in the final products. As Fe content ranges from 10% to 30%, the products consist of TiC, Ti5Si3, Fe2Ti and unreacted C, but no trace of Ti3SiC2 phase is detected. Furthermore, the amounts of both Fe2Ti and C phases increase with Fe content increasing. Addition of Fe has a great effect on the reaction route and significantly restrains the formation of Ti3SiC2 during the combustion synthesis process, and therefore, the SHS is not an effective fabrication technique to synthesize the ternary Ti3SiC2 ceramic in either 3Ti-Si-2C or Fe-3Ti-Si-2C system. Besides, without Fe addition, Ti5Si3 presents as the coarse irregular appearance with an obviously sintered morphology. In contrast, the shape of Ti5Si3 exhibits more and more spherical (cobblestone-like) and the surface becomes increasingly smooth, because the amount of liquids formed during the SHS reaction increases with the increase of Fe content.
