1Review of 500 single incision laparoscopic colorectalsurgery cases-Lessons learned显示文摘Single incision laparoscopic surgery(SILS) is a minimally invasive platform with specific benefits over traditional multiport laparoscopic surgery. The safety and feasibility of SILS has been proven, and the applications continue to grow with experience. After 500 cases at a high-volume, single-institution, we were able to standardize instrumentation and operative steps, as well as develop adaptations in technique to help overcome technical and ergonomic challenges. These technical adaptations have allowed the successful application of SILS to technically difficult patient populations, such as pelvic cases, inflammatory bowel disease cases, and high body mass index patients. 2Release of interleukin-10 and neurotrophic factors in the choroid plexus:possible inductors of neurogenesis following copolymer-1 immunization after cerebral ischemia显示文摘Copolymer-1(Cop-1) is a peptide with immunomodulatory properties, approved by the Food and Drug Administration of United States in the treatment of multiple sclerosis. Cop-1 has been shown to exert neuroprotective effects and induce neurogenesis in cerebral ischemia models. Nevertheless, the mechanism involved in the neurogenic action of this compound remains unknown. The choroid plexus(CP) is a network of cells that constitute the interphase between the immune and central nervous systems, with the ability to mediate neurogenesis through the release of cytokines and growth factors. Therefore, the CP could play a role in Cop-1-induced neurogenesis. In order to determine the participation of the CP in the induction of neurogenesis after Cop-1 immunization, we evaluated the gene expression of various growth factors(brain-derived neurotrophic factor, insulin-like growth factor 1, neurotrophin-3) and cytokines(tumor necrosis factor alpha, interferon-gamma, interleukin-4(IL-4), IL-10 and IL-17), in the CP at 14 days after ischemia. Furthermore, we analyzed the correlation between the expression of these genes and neurogenesis. Our results showed that Cop-1 was capable of stimulating an upregulation in the expression of the genes encoding for brain-derived neurotrophic factor, insulin-like growth factor 1, neurotrophin-3 and IL-10 in the CP, which correlated with an increase in neurogenesis in the subventricular and subgranular zone. As well, we observed a downregulation of IL-17 gene expression. 4Use of a combination strategy to improve neuroprotection and neuroregeneration in a rat model of acute spinal cord injury显示文摘Spinal cord injury is a very common pathological event that has devastating functional consequences in patients. In recent years, several research groups are trying to find an effective therapy that could be applied in clinical practice. In this study, we analyzed the combination of different strategies as a potential therapy for spinal cord injury. Immunization with neural derived peptides(INDP), inhibition of glial scar formation(dipyridyl: DPY), as well as the use of biocompatible matrix(fibrin glue: FG) impregnated with bone marrow mesenchymal stem cells(MSCs) were combined and then its beneficial effects were evaluated in the induction of neuroprotection and neuroregeneration after acute SCI. Sprague-Dawley female rats were subjected to a moderate spinal cord injury and then randomly allocated into five groups: 1) phosphate buffered saline; 2) DPY; 3) INDP + DPY; 4) DPY+ FG; 5) INDP + DPY + FG + MSCs. In all rats, intervention was performed 72 hours after spinal cord injury. Locomotor and sensibility recovery was assessed in all rats. At 60 days after treatment, histological examinations of the spinal cord(hematoxylin-eosin and Bielschowsky staining) were performed. Our results showed that the combination therapy(DPY+ INDP + FG+ MSCs) was the best strategy to promote motor and sensibility recovery. In addition, significant increases in tissue preservation and axonal density were observed in the combination therapy group. Findings from this study suggest that the combination theapy(DPY+ INDP + FG + MSCs) exhibits potential effects on the protection and regeneration of neural tissue after acute spinal cord injury. 5Antimicrobial susceptibility testing before first-line treatment for Helicobacter pylori infection in patients with dual or triple antibiotic resistance显示文摘AIM To evaluate the efficacy of antimicrobial susceptibilityguided therapy before first-line treatment for infection in patients with dual or triple antibiotic resistance.METHODS A total of 1034 patients infected by Helicobacter pylori(H. pylori) during 2013-2014 were tested for antimicrobial susceptibility. 157 of 1034(15%) patients showed resistance to two(127/1034; 12%) and to three(30/1034; 3%) antibiotics. Sixty-eight patients with dual H. pylori-resistance(clarithromycin, metronidazole or levofloxacin) were treated for 10 d with triple therapies: OAL(omeprazole 20 mg b.i.d., amoxicillin 1 g b.i.d., and levofloxacin 500 mg b.i.d.) 43cases, OAM(omeprazole 20 mg b.i.d., amoxicillin 1 g b.i.d., and metronidazole 500 mg b.i.d.) 12 cases and OAC(omeprazole 20 mg b.id., amoxicillin 1 g b.i.d., and clarithromycin 500 mg b.i.d.) 13 cases based on the antimicrobial susceptibility testing. Twelve patients showed triple H. pylori-resistance(clarithromycin, metronidazole and levofloxacin) and received for 10 d triple therapy with OAR(omeprazole 20 mg b.id., amoxicillin 1 g b.i.d., and rifabutin 150 mg b.i.d.). Eradication was confirmed by 13C-urea breath test. Adverse effects and compliance were assessed by a questionnaire. RESULTS Intention-to-treat eradication rates were: OAL(97.6%), OAM(91.6%), OAC(92.3%) and OAR(58.3%). Cure rate was significantly higher in na?ve patients treated with OAR-10 compared to patients who had two or three previous treatment failures(83% vs 33%). Adverse events rates for OAL, OAM, OAC and OAR were 22%, 25%, 23% and 17%, respectively, all of them mild-moderate. 6Simulation and animation model to boost mining efficiency and enviro-friendly in multi-pit operations显示文摘A discrete-event system simulation and animation program was developed to enhance the efficiency of a truck-excavator operation and reduce the environmental impact of haulage in an open-cut coal mine with multiple-pit operations. In any mine, a key objective is to have sufficient equipment for production and not to have excess to where it becomes counterproductive. Due to the advent of responsible mining,environmental regulations, and eco-friendly practices, these factors must also be considered in the analysis. Simulation studies can be financially advantageous for both the optimization of existing mine operations and new development phases in a mine. This study is a new approach to use discrete-event system simulation for mine systems, in order to investigate and possibly reduce environmental impact considering mining haulage performance and production target. A hypothetical layout of a surface coal mine with two pit operations was used for the simulation and animation model. The simulation model includes the animation of the operation. Animation is helpful to enhance the benefit of a mine simulation model. 8Spinal cord injury-induced cognitive impairment: a narrative review显示文摘Spinal cord injury is a serious damage to the spinal cord that can lead to life-long disability.Based on its etiology,spinal cord injury can be classified as traumatic or non-traumatic spinal cord injury.Furthermore,the pathology of spinal cord injury can be divided into two phases,a primary injury phase,and a secondary injury phase.The primary spinal cord injury phase involves the initial mechanical injury in which the physical force of impact is directly imparted to the spinal cord,disrupting blood vessels,axons,and neural cell membranes.After the primary injury,a cascade of secondary events begins,expanding the zone of neural tissue damage,and exacerbating neurological deficits.Secondary injury is a progressive condition characterized by pro-inflammatory cytokines,reactive oxygen species,oxidative damage,excitatory amino acids such as glutamate,loss of ionic homeostasis,mitochondrial dysfunction,and cell death.This secondary phase lasts for several weeks or months and can be further subdivided into acute,subacute,and chronic.One of the most frequent and devastating complications developed among the spinal cord injury population is cognitive impairment.The risk of cognitive decline after spinal cord injury has been reported to be 13 times higher than in healthy individuals.The exact etiology of this neurological complication remains unclear,however,many factors have been proposed as potential contributors to the development of this disorder,such as concomitant traumatic brain injury,hypoxia,anoxia,autonomic dysfunction,sleep disorders such as obstructive sleep apnea,body temperature dysregulation,alcohol abuse,and certain drugs.This review focuses on a deep understanding of the pathophysiology of spinal cord injury and its relationship to cognitive impairment.We highlight the main mechanisms that lead to the development of this neurological complication in patients with spinal cord injury.Oscar V.Alcántar-Garibay Diego Incontri-Abraham Antonio Ibarra 2022Neural Regeneration Research2022,17,12:3
9Erosion problem in tool steel using cold box core-making process显示文摘This paper presents the erosion results of the AISI H13 steel impinged by resin-bonded silica sand, using a testing rig that closely simulates the real blowing conditions during industrial core-making. Steel specimens were heat treated to obtain hardness of 294, 445 and 595 HV200 (29, 45 and 55 HRC). Erosion tests were carried out at impingement angles from 20° to 90° and air drag pressures of 1.38, 2.07 and 2.76 bar (20, 30 and 40 psi). The main results are summarized as follows:(i) The harder material, the lower erosion;(ii) the maximum erosion rate is at 30°;(iii) Little difference in erosion rate at impact angle of 60° and 90° for a constant pressure tested regardless of the hardness level;(iv) As the pressure increases, so does the erosion rate, being more sensitive for low impact angles. 11The influence of elevation on soil properties and forest litter in the Siliceous Moncayo Massif,SW Europe显示文摘Understanding the effects of elevation and related factors(climate,vegetation) on the physical and chemical soil properties can help to predict changes in response to future climate or afforestation forcings.This work aims to contribute to the knowledge of soil evolution and the classification of forest soils in relation to elevation in the montane stage,with special attention to podzolization and humus forms.The northern flank of the Moncayo Massif(Iberian Range,SW Europe) provides a unique opportunity to study a forest soils catena within a consistent quartzitic parent material over a relatively steep elevation gradient.With increasing elevation,pH,base saturation,exchangeable potassium,and fine silt-sized particles decrease significantly,while organic matter,the C/N ratio,soil aggregate stability,water repellency and coarse sand-sized particles increase significantly.The soil profiles shared a set of properties in all horizons:loamy-skeletal particle-size,extreme acidity(pH-H_2O<5.6) and low base saturation(<50%).The most prevalent soil forming processes in the catena include topsoil organic matter accumulation and even podzolization,which increases with elevation.From the upper to lower landscape positions of wooded montane stage of the Moncayo Massif,mull-moder-mor humus and an UmbrisolCambisol-Podzol soil unit sequences were found.BADIA David RUIZ Alberto GIRONA Antonio MARTI Clara CASANOVA Jose IBARRA Paloma ZUFIAURRE Raquel 2016Journal of Mountain Science2016,13,12:2
12Politicizing Responsible Innovation:Responsibility as Inclusive Governance显示文摘This paper aims to provide the historical and conceptual bases underlying the inclusionary transition of European innovation policy,and critical analysis of the difficulties relating to the political nature of this transition.In the 50s and 60s of last century,linear innovation models operated on the basis of a clear division of roles among the different actors in innovation and fundamentally economistic‐based strategies.The following decades saw innovation policies progressively recognize the multi‐dimensional and complex nature of innovation and the need to make adjustments,but always in explicit response to the competitiveness imperative.More recent RRI(Responsible Research and Innovation)strategy within the European Union,in contrast,demands opening up the whole innovation process(including values and motivations)to collective decision,i.e.,approaching responsible innovation as inclusive innovation.This paper appraises this important development primarily on the basis of in‐depth analysis of the main policy literature on innovation,and also on the grounds of related academic literature.As a result,we conclude that the bid for collaboration models cohabits constitutively with another set of dynamics aimed at strengthening centralized and prescriptive forms of innovation.In other words,that inclusionary or political eagerness represented through RRI must grapple with the strategic imperative of competitiveness and economic development.Hence,fundamental tension exists,which should be elucidated in light of the objectives,demands and considerations that are integrated,and cease to be integrated,in innovation dynamics and trajectories.Andoni Eizagirre Hannot Rodríguez Andoni Ibarra 2017International Journal of Innovation Studies2017,1,1:2

15Qigong in the treatment of children with autism spectrum disorder:A systematic review显示文摘Background: Autism spectrum disorder is a condition that affects all races, ethnic and socioeconomic groups. With a high incidenee ratio of one in every 68, it has become one of the most discussed psychiatric disorders. For this reason, the need for investigating novel treatments has been emerging. Qigong, a traditional Chinese mind-body technique, has already proven to be able to reduce symptoms of several physical and psychological illnesses. Objective: The purpose of this systematic review is to examine and categorize the current scientific evidence regarding the efficacy of Qigong on children suffering from autism spectrum disorders. Search strategy: A systematic literature search of the electronic scientific databases PubMed, Clinical Trials.gov, BioMed Central, PubMed Central and Google Scholar was performed to identify studies of Qigong in the treatment of children with autism spectrum disorder. Inclusion criteria: This review included randomized controlled trials, replication studies, retrospective studies and observational follow-up studies of Qigong on children with autism spectrum disorder. Case reports and case series were excluded. Data extraction and analysis: Two researchers independently evaluated the methodological quality of all included studies. Any discrepancies were solved by discussion until consensus was achieved. Results: Our literature search identified 157 publications, and 10 additional publications from hand search of references. After duplicate removal, 103 records remained. After the title/abstract screening, 19 publications were obtained for detailed evaluation. After detailed evaluation, 10 studies were included. Seven studies were conducted with small children with 2-6 years old employing Qigong massage, and three studies were con ducted with older children aged 7-17 years old applying both Qigong massage (one study) and Neigong (two studies). Conclusion: Studies demonstrated that Qigong has interesting and promising applicability and effect on children with autism spectrum disorder and should be tested further. Despite the need for more rigorous controlled studies, Qigong seems to be able to decrease severity of individual sensory, behavioural, and language components of autism, and improve self-control, sociability, sensory and cognitive awareness as well as healthy-physical behaviour. Besides positive effect on children and adolescents, benefits seem to extend to parents and caregivers as well. However, quality of methodology seems to be insufficient to state that Qigong is an alternative to common behavioural therapies.
