1Pulmonary infection control window in treatment of severe respiratory failure of chronic obstructive pulmonary diseases: a prospective, randomized controlled, multi-centred study显示文摘Background Early withdraw from invasive mechanical ventilation (MV) followed by noninvasive MV is a new strategy for changing modes of treatment. This study was conducted to estimate the feasibility and the efficacy of early extubation and sequential noninvasive MV commenced at beginning of pulmonary infection control window in patients with exacerbated hypercapnic respiratory failure caused by chronic obstructive pulmonary diseases (COPD). Methods A prospective, randomized controlled study was conducted in eleven teaching hospitals' respiratory or medical intensive care units in China. Ninety intubated COPD patients with severe hypercapnic respiratory failure triggered by pulmonary infection (pneumonia or purulent bronchitis) were involved in the study. When the pulmonary infection had been controlled by antibiotics and comprehensive therapy, the 'pulmonary infection control window (PIC window)' has been reached. Each case was randomly assigned to study group (extubation and noninvasive MV via facial mask immediately) or control group (invasive MV was received continuously after PIC window by using conventional weaning technique).Results Study group (n=47) and control group (n=43) had similar clinical characteristics initially and at the time of PIC window. Compared with control group, study group had shorter duration of invasive MV [(6.4±4.4) days vs (11.3±6.2) days, P=0.000], lower rate of ventilator associated pneumonia (VAP) (3/47 vs 12/43, P=0.014), fewer days in ICU [(12±8) days vs (16±11) days, P=0.047] and lower hospital mortality (1/47 vs 7/43, P=0.025).Conclusions In COPD patients requiring intubation and invasive MV for hypercapnic respiratory failure, which is exacerbated by pulmonary infection, early extubation followed by noninvasive MV initiated at the start of PIC window may decrease significantly the duration of invasive MV, the risk of VAP and hospital mortality.Wang, C Zhan, QY Cao, ZX Wei, LQ Cheng, ZZ Liu, S Zhang, JL Chen, RC Luo, Q Niu, SF Zhu, L Wu, DW Fang, BM Wu, TH Wang, CZ Liu, YN 2005Chinese Medical Journal2005,,19:6
2Water limitation and rootstock genotype interact to alter grape berry metabolism through transcriptome reprogramming显示文摘Grapevine is a perennial crop often cultivated by grafting a scion cultivar on a suitable rootstock.Rootstocks influence scions,particularly with regard to water uptake and vigor.Therefore,one of the possibilities to adapt viticulture to the extended drought stress periods is to select rootstocks conferring increased tolerance to drought.However,the molecular mechanisms associated with the ability of rootstock/scion combination to influence grape berry metabolism under drought stress are still poorly understood.The transcriptomic changes induced by drought stress in grape berries(cv.Pinot noir)from vines grafted on either 110R(drought-tolerant)or 125AA(drought-sensitive)rootstock were compared.The experiments were conducted in the vineyard for two years and two grape berry developmental stages(50%and 100%veraison).The genome-wide microarray approach showed that water stress strongly impacts gene expression in the berries,through ontology categories that cover cell wall metabolism,primary and secondary metabolism,signaling,stress,and hormones,and that some of these effects strongly depend on the rootstock genotype.Indeed,under drought stress,berries from vines grafted on 110R displayed a different transcriptional response compared to 125AA-concerning genes related to jasmonate(JA),phenylpropanoid metabolism,and pathogenesis-related proteins.The data also suggest a link between JA and secondary metabolism in water-stressed berries.Overall,genes related to secondary metabolism and JA are more induced and/or less repressed by drought stress in the berries grafted on the drought-sensitive rootstock 125AA.These rootstock-dependent gene expression changes are relevant for berry composition and sensory properties.Mariam Berdeja Philippe Nicolas Christian Kappel Zhan Wu Dai Ghislaine Hilbert Anthony Peccoux Magali Lafontaine Nathalie Ollat Eric Gomès Serge Delrot 2015Horticulture Research2015,2,1:5
3Nonalcoholic fatty liver disease is associated with benign gastrointestinal disorders显示文摘AIM:To explore associations between nonalcoholic fatty liver disease(NAFLD)and benign gastrointestinal and pancreato-biliary disorders.METHODS:Patient demographics,diagnoses,and hospital outcomes from the 2010 Nationwide Inpatient Sample were analyzed.Chronic liver diseases were identified using International Classification of Diseases,the 9thRevision,Clinical Modification codes.Patients with NAFLD were compared to those with other chronic liver diseases for the endpoints of total hospital charges,disease severity,and hospital mortality.Multivariable stepwise logistic regression analyses to assess for the independent association of demographic,comorbidity,and diagnosis variables with the event of NAFLD(vs other chronic liver diseases)were also performed.RESULTS:Of 7800441 discharge records,32347(0.4%)and 271049(3.5%)included diagnoses of NAFLD and other chronic liver diseases,respectively.NAFLD patients were younger(average 52.3 years vs55.3 years),more often female(58.8%vs 41.6%),less often black(9.6%vs 18.6%),and were from higher income areas(23.7%vs 17.7%)compared to counterparts with other chronic liver diseases(all P<0.0001).Diabetes mellitus(43.4%vs 28.9%),hypertension(56.9%vs 47.6%),morbid obesity(36.9%vs 8.0%),dyslipidemia(37.9%vs 15.6%),and the metabolic syndrome(28.75%vs 8.8%)were all more common among NAFLD patients(all P<0.0001).The average total hospital charge($39607 vs$51665),disease severity scores,and intra-hospital mortality(0.9%vs6.0%)were lower among NALFD patients compared to those with other chronic liver diseases(all P<0.0001).Compared with other chronic liver diseases,NAFLD was significantly associated with diverticular disorders[OR=4.26(3.89-4.67)],inflammatory bowel diseases[OR=3.64(3.10-4.28)],gallstone related diseases[OR=3.59(3.40-3.79)],and benign pancreatitis[OR=2.95(2.79-3.12)]on multivariable logistic regression(all P<0.0001)when the latter disorders were the principal diagnoses on hospital discharge.Similar relationships were observed when the latter disorders were associated diagnoses on hospital discharge.CONCLUSION:NAFLD is associated with diverticular,inflammatory bowel,gallstone,and benign pancreatitis disorders.Compared with other liver diseases,patients with NAFLD have lower hospital charges and mortality.Srinevas K Reddy Min Zhan H Richard Alexander Samer S El-Kamary 2013World Journal of Gastroenterology2013,19,45:4
4术中神经电生理监测在单椎板入路选择性神经后根离断术中的作用显示文摘目的单椎板入路选择性神经后根离断术(SL-SDR)治疗痉挛性脑瘫完全依赖术中神经电生理监测结果。本研究旨在讨论SL-SDR术中肌电图判读对神经根选择和离断的影响及作用。方法回顾分析2016年5月至2019年3月在上海交通大学附属儿童医院神经外科采用改良神经电生理监测离断方案指导下SL-SDR治疗的痉挛性脑瘫患儿的临床资料,重点关注术中肌电图判读及其与术前临床表现的相关性,以及比较采用不同神经电生理监测离断方案解读术中肌电图时的神经后根选择差异。结果共纳入318例患儿,男性231例、女性87例,平均年龄5.9岁,32例为偏瘫、161例为双侧瘫、125例为四肢瘫。每例患儿有2~8组目标肌群(术前肌张力≥2级)。术中神经电生理监测结果显示,探测的21728枚神经小根[(68.3±8.2)枚/例]中6272枚(28.9%)与肛门括约肌相关,15456枚[(48.6±7.6)枚/例]与下肢肌群相关,其中,神经后根11009枚[(34.6±7.4)枚/例]。共有3370枚[(10.6±4.7)枚/例]符合改良神经后根离断方案,其中3061枚50%部分离断、309枚75%部分离断。术前粗大运动功能分级(GMFCS)分级Ⅰ~Ⅴ级的离断率(部分离断的神经后根数/下肢肌群相关神经后根数)分别为15.8%、22.3%、33.4%、41.8%和45.7%。75%部分离断的离断率随术前GMFCS分级的递增而增加,分别为1.5%、4.8%、8.5%、14.1%和15.2%。术后3周,2068组目标肌群肌张力显著降低[(1.7±0.5)级对(2.7±0.6)级]。术前GMFCS分级Ⅰ和Ⅱ级患儿采用传统离断方案约有20%的病例无法识别任何1枚约20%需离断的神经后根,而随着患儿术前GMFCS分级的递增,传统与改良方案离断的符合率逐步提高,分别为39.5%、41.3%、52.2%、54.1%和62.8%。结论由于SL-SDR完全依赖于术中肌电图判读,使得术中神经电生理监测显得至关重要。改良神经电生理监测离断方案指导下的SL-SDR可以安全、有效治疗痉挛性脑瘫。SL-SDR术中肌电图及其与临床表现相关的预后,可以使临床医师更好地理解此类患儿脊髓神经元环路的运作模式。Xiao B Constatntini S Browd SR Zhan Q Jiang W Mei R 詹琪佳(编译) 2020中国现代神经疾病杂志2020,20,4:4
5Expressions of p53 and inducible nitric oxid synthase in congenital choledochal cysts显示文摘BACKGROUND: Choledochal cyst, an isolated defect un- restricted to the bile duct, is more appropriately regarded as the sentinel feature of a constellation of anomalies affect- ing the pancreatobiliary system. This study was to assess the relationship between the expression of inducible nitric oxide synthase (iNOS) and the p53 gene as well as the pathoge- nesis of choledochal cysts. METHODS: iNOS and p53 were detected by immunohisto- chemistry staining in 26 patients with congenital choledo- chal cysts. Histopathologically, hyperplasia of the mucosa of the cysts and the amylase level in the bile were also inve- stigated. RESULTS: Patients with a high level of amylase in the bile had higher expression of iNOS than those with a low level of amylase. p53 protein was expressed neither in fusiform type nor in cystic type. The incidence of mucosal hyperpla- sia was significantly higher in the fusiform type than that in the cystic type. CONCLUSIONS: Higher expression of iNOS may partici- pate in hyperplasia and carcinogenesis of the mucosa of choledochal cysts. The regurgitation of pancreatic juice in- to the biliary system might induce mucosal hyperplasia of the biliary tract and inflammatory reaction.
