2Asymptomatic lymphangioma involving the spleen and retroperitoneum in adults显示文摘Lymphangioma,a benign neoplasm of the lymphatic system,is common in children but rare in adults.Its clinical manifestations include abdominal pain,nausea, vomiting and a palpable mass.However,abdominal sonography or abdominal computed tomography(CT) scan can also incidentally reveal lymphangioma.A larger or symptomatic lymphangioma is treated with total resection to prevent recurrence,infection,torsion and enlargement.Although lymphangioma rarely becomes malignant,its prognosis is generally good. We report a cystic lymphangioma of the spleen and retroperitoneum,which was incidentally found in a 56-year-old man who was hospitalized due to a colon mass.Physical examination showed no specific findings.Abdominal CT revealed a 5.7 cm,non-enhanced multilobulated cystic mass with multiple septa in the spleen and a 10 cm lobulated cystic mass in the paraaortic area.Splenectomy and retroperitoneal resection of the cystic mass were conducted.The endotheliumof splenic and retroperitoneal cyst was immunohisto- chemically stained with D2-40 antibody.The patient was finally diagnosed with splenic cystic and retroperitoneal cavernous lymphangioma.Sook Hee Chung Young Sook Park Yun Ju Jo Seong Hwan Kim Dae Won Jun Byoung Kwan Son Jun Young Jung Dae Hyun Baek Dong Hee Kim Yoon Young Jung Won Mi Lee 2009World Journal of Gastroenterology2009,15,44:7
3PD-1 blockade-unresponsive human tumor-infiltrating CD8^(+)T cells are marked by loss of CD28 expression and rescued by IL-15显示文摘Blockade of programmed death-1(PD-1)reinvigorates exhausted CD8^(+)T cells,resulting in tumor regression in cancer patients.Recently,reinvigoration of exhausted CD8^(+)T cells following PD-1 blockade was shown to be CD28-dependent in mouse models.Herein,we examined the role of CD28 in anti-PD-1 antibody-induced human T cell reinvigoration using tumor-infiltrating CD8^(+)T cells(CD8^(+)TILs)obtained from non-small-cell lung cancer patients.Single-cell analysis demonstrated a distinct expression pattern of CD28 between mouse and human CD8^(+)TILs.Furthermore,we found that human CD28^(+)CD8^(+)but not CD28^(–)CD8^(+)TILs responded to PD-1 blockade irrespective of B7/CD28 blockade,indicating that CD28 costimulation in human CD8^(+)TILs is dispensable for PD-1 blockade-induced reinvigoration and that loss of CD28 expression serves as a marker of anti-PD-1 antibody-unresponsive CD8^(+)TILs.Transcriptionally and phenotypically,PD-1 blockade-unresponsive human CD28^(–)PD-1^(+)CD8^(+)TILs exhibited characteristics of terminally exhausted CD8^(+)T cells with low TCF1 expression.Notably,CD28^(–)PD-1^(+)CD8^(+)TILs had preserved machinery to respond to IL-15,and IL-15 treatment enhanced the proliferation of CD28^(–)PD-1^(+)CD8^(+)TILs as well as CD28^(+)PD-1^(+)CD8^(+)TILs.Taken together,these results show that loss of CD28 expression is a marker of PD-1 blockade-unresponsive human CD8^(+)TILs with a TCF1–signature and provide mechanistic insights into combining IL-15 with anti-PD-1 antibodies.Kyung Hwan Kim Hong Kwan Kim Hyung-Don Kim Chang Gon Kim Hoyoung Lee Ji Won Han Seong Jin Choi Seongju Jeong Minwoo Jeon Hyunglae Kim Jiae Koh Bo Mi Ku Su-Hyung Park Myung-Ju Ahn Eui-Cheol Shin 2021Cellular & Molecular Immunology2021,18,2:6
4Emerging role of ^(18)F-fluorodeoxyglucose positron emission tomography for guiding management of hepatocellular carcinoma显示文摘Hepatocellular carcinoma(HCC) is one of major causes of cancer mortality worldwide. For decades, ^(18)F-fluorodeoxyglucose(FDG) positron emission tomography(PET) has been widely used for staging, predicting prognosis, and detecting cancer recurrence in various types of malignant diseases. Due to low sensitivity of FDG PET for detecting intrahepatic HCC lesions, the clinical value of FDG PET in HCC patients has been limited. However, recent studies with diverse analytic methods have shown that FDG PET has promising role in aiding management of HCC patients. In this review, we will discuss the clinical role of FDG PET for staging, predicting prognosis, and evaluating treatment response in HCC. 6Treatment for gastric ‘indefinite for neoplasm/dysplasia' lesions based on predictive factors显示文摘BACKGROUND Gastric ‘indefinite for neoplasm/dysplasia'(IFND) is a borderline lesion that is difficult to diagnose as either regenerative or neoplastic. There is a need for guidance in the identification of a subset of patients, who have an IFND lesion with a higher risk of malignant potential, to enable risk stratification and optimal management.AIM To determine the clinical and pathologic factors for the accurate diagnosis of gastric IFND lesions.METHODS In total, 461 gastric lesions diagnosed via biopsy as IFND lesions were retrospectively evaluated. Endoscopic resection(n = 134), surgery(n = 22), and follow-up endoscopic biopsy(n = 305) were performed to confirm the diagnosis.The time interval from initial biopsy to cancer diagnosis was measured, and diagnostic delays were categorized as > 2 wk, > 2 mo, > 6 mo, and > 1 year. The IFND lesions presenting as regenerating atypia(60%) or atypical epithelia(40%)at initial biopsy were adenocarcinomas in 22.6%, adenomas in 8.9%, and gastritis in 68.5% of the cases.RESULTS Four clinical factors [age ≥ 60 years(2.445, 95%CI: 1.305-4.580, P = 0.005),endoscopic size ≥ 10 mm(3.519, 95%CI: 1.891-6.548, P < 0.001), single lesion(5.702, 95%CI: 2.212-14.696, P < 0.001), and spontaneous bleeding(4.056, 95%CI:1.792-9.180, P = 0.001)], and two pathologic factors [atypical epithelium(25.575,95%CI: 11.537-56.695, P < 0.001], and repeated IFND diagnosis [6.022, 95%CI:1.822-19.909, P = 0.003)] were independent risk factors for gastric cancer. With two or more clinical factors, the sensitivity and specificity for carcinoma were91.3% and 54.9%, respectively. Ten undifferentiated carcinomas were initially diagnosed as IFND. In the subgroup analysis, fold change(5.594, 95%CI: 1.458-21.462, P = 0.012) predicted undifferentiated or invasive carcinoma in the submucosal layers or deeper. Diagnostic delays shorter than 1 year were not associated with worse prognoses. 7Improvement of diabetes and hypertension after gastrectomy: A nationwide cohort study显示文摘AIM: To evaluate the effect of gastrectomy on diabetes mellitus(DM) and hypertension(HTN) in non-obese gastric cancer patients. METHODS: A total of 100000 patients, diagnosed with either type 2 DM or HTN, were randomly selected from the 2004 Korean National Health Insurance System claims. Among them, 360 diabetes and 351 hypertensive patients with gastric cancer who had been regularly treated without chemotherapy from January 2005 to December 2010 were selected. They were divided into three groups according to their treatment methods: total gastrectomy(TG), subtotal gastrectomy(STG) and endoscopic resection(ER). RESULTS: The drug discontinuation rate of antidiabetic and anti-hypertensive agents after gastric cancer treatment was 9.7% and 11.1% respectively. DM appeared to be improved more frequently(22.8%) and earlier(mean ± SE 28.6 ± 1.8 mo) in TG group than in the two other groups [improved in 9.5% of ER group(37.4 ± 1.1 mo) and 6.4% of STG group(47.0 ± 0.8 mo)]. The proportion of patients treated with multiple drugs decreased more notably in TG group compared to others(P = 0.001 in DM, and P = 0.035 in HTN). 9Clinical implication of FDG uptake of bone marrow on PET/CT in gastric cancer patients with surgical resection显示文摘AIM To determine the relationship between F-18 fluorodeoxyglucose(FDG) uptake of bone marrow(BM) on positron emission tomography/computed tomography(PET/CT) and clinical factors and to assess the prognostic value of FDG uptake of BM in gastric carcinoma.METHODS We retrospectively enrolled 309 gastric cancer patients who underwent staging FDG PET/CT and curative surgical resection. FDG uptake of primary tumor was visually classified as positive or negative FDG uptake. Mean FDG uptake of BM(BM SUV) and BM-to-liver uptake ratio(BLR) were measured. The relationships of BM SUV or BLR with clinical factors were evaluated. The prognostic values of BM SUV, BLR, and other clinical factors for predicting recurrence-free survival(RFS) and overall survival(OS) were assessed.RESULTS Of 309 patients, 38 patients(12.3%) experienced cancer recurrence and 18 patients(5.8%) died. Patients with advanced gastric cancer, positive FDG uptake, and recurrence had higher values of BM SUV and BLR than those with early gastric cancer, negative FDG uptake, and no recurrence(P < 0.05). BM SUV and BLR were significantly correlated with hemoglobin level, neutrophil-to-lymphocyte ratio, and platelet-tolymphocyte ratio(P < 0.05). On multivariate analysis, multiple tumors, T stage, lymph node metastasis, tumor involvement of resection margin, and BLR were significantly associated with RFS(P < 0.05). T stage, lymph node metastasis, hemoglobin level, and BLR were significantly associated with OS(P < 0.05). 10Crystal Facet Engineering of TiO_(2) Nanostructures for Enhancing Photoelectrochemical Water Splitting with BiVO_(4) Nanodots显示文摘Although bismuth vanadate(BiVO4)has been promising as photoanode material for photoelectrochemical water splitting,its charge recombination issue by short charge diffusion length has led to various studies about heterostructure photoanodes.As a hole blocking layer of BiVO4,titanium dioxide(TiO_(2)) has been considered unsuitable because of its relatively positive valence band edge and low electrical conductivity.Herein,a crystal facet engineering of TiO_(2) nanostructures is proposed to control band structures for the hole blocking layer of BiVO4 nanodots.We design two types of TiO_(2) nanostructures,which are nanorods(NRs)and nanoflowers(NFs)with different(001)and(110)crystal facets,respectively,and fabricate BiVO4/TiO_(2) heterostructure photoanodes.The BiVO4/TiO_(2) NFs showed 4.8 times higher photocurrent density than the BiVO4/TiO_(2) NRs.Transient decay time analysis and time-resolved photoluminescence reveal the enhancement is attributed to the reduced charge recombination,which is originated from the formation of type II band alignment between BiVO4 nanodots and TiO_(2) NFs.This work provides not only new insights into the interplay between crystal facets and band structures but also important steps for the design of highly efficient photoelectrodes.Mi Gyoung Lee Jin Wook Yang Hoonkee Park Cheon Woo Moon Dinsefa M.Andoshe Jongseong Park Chang‑Ki Moon Tae Hyung Lee Kyoung Soon Choi Woo Seok Cheon Jang‑Joo Kim Ho Won Jang 2022Nano-Micro Letters2022,14,3:2

12Catalysts for electrochemical ammonia oxidation:Trend,challenge,and promise显示文摘Developing sustainable and clean energy technology is critial to address the current energy and environmental crisis.As an energy carrier,hydrogen has been getting attention due to its abundance,high energy density,and zero carbon emission.Due to the difficulty of hydrogen storage and transport,the electro-oxidation of ammonia shows great potential for renewable energy since it satisfies both energy supply and environmental protection as a carbon-free fuel with high hydrogen content.However,ammonia electro-oxidation is a sluggish reaction and shows low durability.Understanding the complicated mechanism and intermediates is conducive to developing efficient electrocatalysts with high catalytic activity and durability.This review begins with the ammonia oxidation technologies and their principles.The two typical mechanisms for ammonia electro-oxidation,Oswin-Salomon mechanism and Gerischer-Mauerer mechanism,are discussed,and attention to the poisoning species is included.Then,the electrocatalysts for ammonia reaction are classified into Pt-based catalysts and non-noble metal-based catalysts,and their performances are reviewed with various strategies.Based on the progress on the electrochemical ammonia oxidation catalysts,the challenges and future insight on ammonia oxidation reaction are discussed and proposed.Sol A Lee Mi Gyoung Lee Ho Won Jang 2022Science China Materials2022,65,12:2
13Risk of sedation for diagnostic esophagogastroduodenoscopy in obstructive sleep apnea patients显示文摘AIM: To investigate whether patients with obstructive sleep apnea (OSA) are at risk of sedation-related complications during diagnostic esophagogastroduodenoscopy (EGD). METHODS: A prospective study was performed in consecutive patients with OSA, who were confirmed with full-night polysomnography between July 2010 and April 2011. The occurrence of cardiopulmonary complications related to sedation during diagnostic EGD was compared between OSA and control groups. RESULTS: During the study period, 31 patients with OSA and 65 controls were enrolled. Compared with the control group, a higher dosage of midazolam was administered (P = 0.000) and a higher proportion of deep sedation was performed (P = 0.024) in the OSA group. However, all adverse events, including sedation fail-ure, paradoxical responses, snoring or apnea, hypoxia, hypotension, oxygen or flumazenil administration, and other adverse events were not different between the two groups (all P > 0.1). Patients with OSA were not predisposed to hypoxia with multivariate logistic regression analysis (P = 0.068). 19Newly developed autoimmune cholangitis without relapse of autoimmune pancreatitis after discontinuing prednisolone显示文摘A 57-year-old man presented with a 2-wk history of painless jaundice and weight loss.He had a large illdefined enhancing mass-like lesion in the uncinate process of the pancreas with stricture of the distal common bile duct.Aspiration cytology of the pancreatic mass demonstrated inflammatory cells without evidence of malignancy.Total serum immunoglobulin G level was slightly elevated,but IgG4 level was normal.After the 2-wk 40 mg prednisolone trial,the patient's symptoms and bilirubin level improved significantly.A follow-up computed tomography(CT) scan showed a dramatic resolution of the pancreatic lesion.A low dose steroid was continued.After six months he self-discontinued prednisolone for 3 wk,and was presented with jaundice again.A CT scan showed newly developed intrahepatic biliary dilatation and marked concentric wall thickening of the common hepatic duct and the proximal common bile duct without pancreatic aggravation.The patient' s IgG4 level was elevated to 2.51 g/L.Prednisolone was started again,after which his serum bilirubin level became normal and the thickening of the bile duct was resolved.This case suggests that autoimmune pancreatitis can progress to other organs that are not involved at the initial diagnosis,even with sustained pancreatic remission.Ji Hun Kim Jae Hyuck Chang Sung Min Nam Mi Jeong Lee Il Ho Maeng Jin Young Park Yun Sun Im Tae Ho Kim Chang Whan Kim Sok Won Han 2012World Journal of Gastroenterology2012,18,41:1
