| 1 | Comparison of neutron energy spectrum unfolding methods and evaluation of rationality criteria显示文摘The neutron energy spectrum was measured using a Bonner sphere spectrometer at six locations inside the containment vessel of a nuclear reactor at the Qinshan nuclear power plant. The structures of the neutron spectra obtained by the maximum entropy, iteration, and genetic algorithm methods were consistent with one another and could be interpreted as the spectral superposition of different energy regions. The characteristic parameters of the neutron spectrum, including the fluence rate,average energy, and neutron ambient dose equivalent rate H^(*)(10), were in good agreement among the three methods. In addition, an LB6411 neutron ambient dose equivalent meter was employed to obtain the H^(*)(10) directly for comparison.These findings indicate that neutron spectrum unfolding methods can be used to overcome the problems associated with the response functions of dosimeters to provide more accurate H^(*)(10) values. | 2 | Clinicopathologic characteristics and therapeutic responses of Chinese patients with non-small cell lung cancer who harbor an anaplastic lymphoma kinase rearrangement显示文摘Introduction: The rearrangement of the anaplastic lymphoma kinase(ALK) gene accounts for approximately 1%–6% of lung adenocarcinoma cases and deines a molecular subgroup of tumors characterized by clinical sensitivity to ALK inhibitors such as crizotinib. This study aimed to identify the relationship between ALK rearrangement and the clinico?pathologic characteristics of non?small cell lung cancer(NSCLC) and to analyze the therapeutic responses of crizotinib and conventional chemotherapy to ALK rearrangement in NSCLC patients.Methods: A total of 487 lung cancer patients who underwent testing for ALK rearrangement in our department were included in this study. ALK rearrangement was examined by using fluorescence in situ hybridization(FISH) assay.Results: Among the 487 patients, 44(9.0%) were diagnosed with ALK rearrangement by using FISH assay. In 123 patients with adenocarcinoma who were non?smokers and of a young age(≤58 years old), the frequency of ALK rearrangement was 20.3%(25/123). Short overall survival(OS) was associated with non?adenocarcinoma tumor type(P = 0.006), poorly diferentiated tumors(P al growth factor rece= 0.001), advanced?stage tumors(P < 0.001), smoking history(P ptor(EGFR)(P = 0.008), and wild?type epidermrter time to cancer p= 0.008). Moreover, patients with poorly diferentiated and advanced?stage tumors had a shorogression compared with those with well diferentiated(P = 0.023) and early?stage tumors(P = 0.001), respectively.Conclusions: ALK?rearranged NSCLC tends to occur in younger individuals who are either non?smokers or light smokers with adenocarcinoma. | 3 | Intervention effect of Zhuangfang Rugan Huaxian Granules on CCl_(4)compound factor‑induced liver fibrosis rat model and its effect on the expression ofα‑SMA and E‑Cadherin显示文摘Objective:To study the intervention effect of Zhuangfang Soft Liver and Fibrous Granules on CCl_(4)compound factor-induced liver fibrosis rat model and its effect onα-SMA and E-Cadherin protein expression.Methods:Forty-eight male Wistar rats were randomly divided into a blank group,a pathological model group and a group of 16 rats in each group.The rats in the pathological model group and the pellet group were injected subcutaneously with 40%carbon tetrachloride(CCl_(4))oil combined with peanut oil to establish an animal model of rat liver fibrosis,and the pellets were administered by gavage three times a week for 8 weeks from the 5th week.The expression of oxidative stress oxidase dismutase(SOD)and malondialdehyde(MDA)was detected by enzyme-linked immunoassay(ELISA),and the expression ofα-smooth muscle actin(α-SMA)and E-calmodulin(E-Cadherin)was detected by RT-PCR andWestern Blot.expression.Results:Compared with the blank group,the serum ALT and AST levels of the rats in the pathological model group were significantly increased,and the serum ALT and AST levels of the rats in the soft liver and fibrous pellet group were significantly decreased(P<0.001).In the pathological model group,the lobular structure of liver tissue was mutilated and numerous liver fibrous tissues were abnormally proliferated,and the collagen fibers formed by the proliferation were confluent into dense fibrous intervals,thus turning into pseudolobular structure,and a large number of fatty degenerated hepatocytes of different shapes and sizes appeared inside the pseudolobular structure,and the diffuse infiltration of inflammatory cells was significant.Compared with the pathological model group,the degree of destruction of hepatic lobular structure was significantly reduced in the LBP group,and there were different degrees of proliferation of collagen fibrous tissue concentrated in the area from the central vein to the confluent area,and the fibrous septum was relatively loose,with a small amount of inflammatory cell infiltration,and the degree of infiltration was reduced compared with the pathological model group.Compared with the pathological model group,the SOD level was significantly higher in the group of liver softener and fibrous pellets compared with the pathological model group and the blank group(P<0.001),the MDA level was significantly lower in the group of liver softener and fibrous pellets compared with the pathological model group(P<0.001),the expression level ofα-SMA was significantly lower in the group of liver softener and fibrous pellets(P<0.001),and the expression level of E-Cadherin was significantly higher in the group of liver softener and fibrous pellets(P<0.001).Conclusion:Soft Liver Fibro Pellets can effectively inhibit liver fibrous tissue proliferation and reduce the serum ALT and AST levels in rats with CCl_(4)combined with peanut oil compound solution induced liver fibrosis,and its anti-liver fibrosis mechanism may be through protecting liver function,reducing the level of oxidative stress,down-regulating the expression ofα-SMA protein,up-regulating the expression of E-cadherin protein,and inhibiting epithelial-mesenchymal transition epithelial mesenchymal transformation(EMT)and thus exerting therapeutic effects on CCl_(4)compound-induced hepatic fibrosis in rats. | ZHANG Wen‑fu WU Shan‑shan DAI Ming LV Jian‑lin HUANG Jing‑jing LI Xiao‑long WANG Zhen‑chang | 2023 | Journal of Hainan Medical University2023,29,5: | 0 |

| 7 | Prognostic values of the integrated model incorporating the volume of metastatic regional cervical lymph node and pretreatment serum Epstein-Barr virus DNA copy number in predicting distant metastasis in patients with N1 nasopharyngeal carcinoma显示文摘Background: According to the 7 th edition of the American Joint Committee on Cancer(AJCC) staging system, over50% of patients with nasopharyngeal carcinoma(NPC) have N1 disease at initial diagnosis. However, patients with N1 NPC are relatively under-researched, and the metastasis risk of this group is not well-stratified. This study aimed to evaluate the prognostic values of gross tumor volume of metastatic regional lymph node(GTVnd) and pretreatment serum copy number of Epstein-Barr virus(EBV) DNA in predicting distant metastasis of patients with N1 NPC, and to develop an integrated prognostic model that incorporates GTVnd and EBV DNA copy number for this group of patients.Methods: The medical records of 787 newly diagnosed patients with nonmetastatic, histologically proven N1 NPC who were treated at Sun Yat-sen University Cancer Center between November 2009 and February 2012 were analyzed. Computed tomography-derived GTVnd was measured using the summation-of-area technique. Blood samples were collected before treatment to quantify plasma EBV DNA. The receiver operating characteristic(ROC) curve analysis was used to evaluate the cut-off point for GTVnd, and the area under the ROC curve was used to assess the predicted validity of GTVnd. The survival rates were assessed by Kaplan-Meier analysis, and the survival curves were compared using a log-rank test. Multivariate analysis was conducted using the Cox proportional hazard regression model.Results: The 5-year distant metastasis-free survival(DMFS) rates for patients with GTVnd > 18.9 vs.≤ 18.9 mL were82.2% vs. 93.2%(P < 0.001), and for patients with EBV DNA copy number > 4000 vs. < 4000 copies/mL were 83.5% vs.93.9%(P < 0.001). After adjusting for GTVnd, EBV DNA copy number, and T category in the Cox regression model, both GTVnd > 18.9 mL and EBV DNA copy number > 4000 copies/mL were significantly associated with poor prognosis(both P < 0.05). According to combination of GTVnd and EBV DNA copy number, all patients were divided into low-,moderate-, and high-risk groups, with the 5-year DMFS rates of 96.1,87.4, and 73.8%, respectively(P < 0.001). Multivariate analysis confirmed the prognostic value of this model for distant metastatic risk stratification(hazard ratio [HR],4.17; 95% confidence interval [CI] 2.34-7.59; P < 0.001).Conclusions: GTVnd and serum EBV DNA copy number are independent prognostic factors for predicting distant metastasis in NPC patients with N1 disease.
