| 1 | Surgical outcome of pancreatic cancer using radical antegrade modular pancreatosplenectomy procedure显示文摘AIM:To evaluate the surgical outcomes following radical antegrade modular pancreatosplenectomy (RAMPS) for pancreatic cancer. METHODS:Twenty-four patients underwent RAMPS with curative intent between January 2005 and June 2009 at the National Cancer Center, South Korea. Clinicopathologic data, including age, sex, operative findings, pathologic results, adjuvant therapy, postop-erative clinical course and follow-up data were retro-spectively collected and analyzed for this study. RESULTS:Twenty-one patients (87.5%) underwent distal pancreatectomy and 3 patients (12.5%) underwent total pancreatectomy using RAMPS. Nine patients (37.5%) underwent combined vessel resection, including 8 superior mesenteric-portal vein resections and 1 celiac axis resection. Two patients (8.3%) underwent combined resection of other organs, including the colon, stomach or duodenum. Negative tangential margins were achieved in 22 patients (91.7%). The mean tumor diameter for all patients was 4.09 ± 2.15 cm. The 2 patients with positive margins had a mean diameter of 7.25 cm. The mean number of retrieved lymph nodes was 20.92 ± 11.24 and the node positivity rate was 70.8%. The median survival of the 24 patients was 18.23 ± 6.02 mo. Patients with negative margins had a median survival of 21.80 ± 5.30 mo and those with positive margins had a median survival of 6.47 mo (P = 0.021). Nine patients (37.5%) had postoperative complications, but there were no postoperative mortalities. Pancreatic fistula occurred in 4 patients (16.7%):2 patients had a grade A fistula and 2 had a grade B fistula. On univariate analysis, histologic grade, positive tangential margin, pancreatic fistula and adjuvant therapy were significant prognostic factors for survival. | 2 | Clinical course of ulcerative colitis patients who develop acute pancreatitis显示文摘AIM To investigate the clinical course of ulcerative colitis(UC)patients who develop acute pancreatitis.METHODS We analyzed 3307 UC patients from the inflammatory bowel disease registry at Asan Medical Center from June 1989 to May 2015.The clinical course of UC patients who developed acute pancreatitis was compared with that of non-pancreatitis UC patients.RESULTS Among 51 patients who developed acute pancreatitis,13(0.40%)had autoimmune,10(0.30%)had aminosalicylate-induced,and 13(1.73%)had thiopurineinduced pancreatitis.All 13 patients with autoimmune pancreatitis(AIP)had type 2 AIP.Two(15.4%)patients had pre-existing AIP,and three(23.1%)patients developed AIP and UC simultaneously.Compared to non-pancreatitis patients,AIP patients had UC diagnosed at a significantly younger age(median,22.9 years vs 36.4 years;P=0.001).AIP and aminosalicylate-induced pancreatitis patients had more extensive UC compared to non-pancreatitis patients.All patients with pancreatitis recovered uneventfully,and there were no recurrences.Biologics were used more frequently in aminosalicylate-and thiopurine-induced pancreatitis patients compared to non-pancreatitis patients[adjusted OR(95%CI),5.16(1.42-18.67)and6.90(1.83-25.98),respectively].Biologic utilization rate was similar among AIP and non-pancreatitis patients[OR(95%CI),0.84(0.11-6.66)].Colectomy rates for autoimmune,aminosalicylate-induced,and thiopurineinduced pancreatitis,and for non-pancreatitis patients were 15.4%(2/13),20%(2/10),15.4%(2/13),and7.3%(239/3256),respectively;the rates were not significantly different after adjusting for baseline disease extent.CONCLUSION Pancreatitis patients show a non-significant increase in colectomy,after adjusting for baseline disease extent. | Jong Wook Kim Sung Wook Hwang Sang Hyoung Park Tae Jun Song Myung-Hwan Kim Ho-Su Lee Byong Duk Ye Dong-Hoon Yang Kyung-Jo Kim Jeong-Sik Byeon Seung-Jae Myung Suk-Kyun Yang | 2017 | World Journal of Gastroenterology2017,23,19: | 3 |
| 3 | Sex-dependent difference in the effect of metformin on colorectal cancer-specific mortality of diabetic colorectal cancer patients显示文摘AIM To assess factors associated with the higher effect of metformin on mortality in diabetic colorectal cancer(CRC) patients,since the factors related to the effectiveness of metformin have not been identified yet.METHODS Between January 2000 and December 2010,413 patients diagnosed with both stage 3/4 CRC and diabetes mellitus were identified. Patients' demographics and clinical characteristics were analyzed. The effect of metformin on CRC-specific mortality and the interactions between metformin and each adjusted factor were evaluated.RESULTS Total follow-up duration was median 50 mo(range: 1-218 mo). There were 85 deaths(45.9%) and 72 CRC-specific deaths(38.9%) among 185 patients who used metformin,compared to 130 total deaths(57.0%) and 107 CRC-specific deaths(46.9%) among 228 patients who did not use metformin. In multivariate analysis,survival benefit associated with metformin administration was identified(HR = 0.985,95%CI: 0.974-0.997,P = 0.012). Interaction test between metformin and sex after adjustment for relevant factors revealed that female CRC patients taking metformin exhibited a significantly lower CRC-specific mortality rate than male CRC patients taking metformin(HR = 0.369,95%CI: 0.155-0.881,P = 0.025). Furthermore,subgroup analysis revealed significant differences in CRC-specific mortality between the metformin and non-metformin groups in female patients(HR = 0.501,95%CI: 0.286-0.879,P = 0.013) but not male patients(HR = 0.848,95%CI: 0.594-1.211,P = 0.365).
