| 6 | Activity and safety of pegylated liposomal doxorubicin,5-fluorouracil and folinic acid in inoperable hepatocellular carcinoma:A phase Ⅱ study显示文摘AIM: To improve the results of New therapeutic strategies in hepatocellular carcinoma (HCC). We have conducted a phase Ⅱ study with pegylated liposomal doxorubicin (PLD), 5-fluorouracil (5FU) and folinic acid (FA). METHODS: Thirty-one patients with hystologically- confirmed, inoperable HCC, received combination chemotherapy with PLD 25 mg/mq on d 1, 5FU 1200 mg/mq in 48 h continuous infusion, and oral FA 30 mg on d 1 and 2 every 3 wk until disease progression or intolerable toxicity. RESULTS: The median age was 65 years (range 41-82) and 28 patients were hepatitis C virus seropositive (90%). The majority of patients were Child-Pugh Class B (55%). Two patients showed a partial response (PR), and 16 had stable disease (SD). With a median follow-up of 14 mo, the median time to progression of all evaluable patients was 4 mo (95% CI 1.7-7). Median overall survival was 9 mo (95% CI 3-24 mo). After 1 year, 9 of 18 PR/SD patients were alive. Chemotherapy was well tolerated. CONCLUSION: PLD/FU/FA combination seems capable of achieving durable stabilization of HCC. | 11 | Importance of clinical suspicion and multidisciplinary management for early diagnosis of a cardiac laminopathy patient:A case report显示文摘BACKGROUND Laminopathies are rare diseases,whose cardiac manifestations are heterogeneous and,especially in their initial stage,similar to those of more common conditions,such as ischemic heart disease.Early diagnosis is essential,as these conditions can first manifest themselves with sudden cardiac death.Electrical complications usually appear before structural complications;therefore,it is important to take into consideration these rare genetic disorders for the differential diagnosis of brady and tachyarrhythmias,even when left ventricle systolic function is still preserved.CASE SUMMARY A 60-year-old man,without history of previous disorders,presented in September 2019 to the emergency department because of the onset of syncope associated with hypotension.The patient was diagnosed with a high-grade atrioventricular block.A dual chamber pacemaker was implanted,but after the onset of a sustained ventricular tachycardia during physical exertion,a drug eluting stent was implanted on an intermediate stenosis on the left anterior descending artery,which had previously been considered non-haemodynamically significant.During the follow-up,the treating cardiologist,suspicious of the overall clinical picture,recommended a genetic test for the diagnosis of cardiomyopathies,which tested positive for a pathogenetic mutation of the lamin A/C gene.While awaiting the result of the genetic test and,later,the pacemaker to be upgraded to a biventricular defibrillator,a remote monitoring device was given to the patient in 2019-related lockdown.CONCLUSIONThis case aims to raise awareness of the cardiological manifestations of laminopathies,which can be dangerously misdiagnosed as other, more common conditions. | Vincenzo Ezio Santobuono Andrea Igoren Guaricci Eugenio Carulli Nicola Bozza Martino Pepe Alfredo Ranauro Carlotta Ranieri Maria Cristina Carella Francesco Loizzi Nicoletta Resta Stefano Favale Cinzia Forleo | 2021 | World Journal of Clinical Cases2021，9，25： | 0 |
| 12 | Bevacizumab maintenance in metastatic colorectal cancer: How long?显示文摘The management of patients with non-progressive metastatic colorectal cancer after six months of treatment has not yet been codified. The most relevant concerns are the effectiveness of maintenance versus discontinuation, and the tolerability of prolonged treatment. Here we report the case of a 72-year-old man affected by colorectal cancer with lung metastases who achieved a complete response after receiving capecitabine, oxaliplatin and bevacizumab for six months, and bevacizumab alone for six months. Bevacizumab was continued as maintenance regimen for more than three years. It was discontinued because of an arthroplasty. Fifty-eight months after beginning first-line treatment, the patientremains free from relapse.
