| 1 | Management of postoperative spinal infections显示文摘Postoperative surgical site infection(SSI) is a common complication after posterior lumbar spine surgery. This review details an approach to the prevention,diagnosis and treatment of SSIs. Factors contributing to the development of a SSI can be split into three categories:(1) microbiological factors;(2) factors related to the patient and their spinal pathology; and(3) factors relating to the surgical procedure. SSI is most commonly caused by Staphylococcus aureus. The virulence of the organism causing the SSI can affect its presentation. SSI can be prevented by careful adherence to aseptic technique,prophylactic antibiotics,avoiding myonecrosis by frequently releasing retractors and preoperatively optimizing modifiable patient factors. Increasing pain is commonly the only symptom of a SSI and can lead to a delay in diagnosis. C-reactive protein and magnetic resonance imaging can help establish the diagnosis. Treatment requires acquiring intra-operative cultures to guide future antibiotic therapy and surgical debridement of all necrotic tissue. A SSI can usually be adequately treated without removing spinalinstrumentation. A multidisciplinary approach to SSIs is important. It is useful to involve an infectious disease specialist and use minimum serial bactericidal titers to enhance the effectiveness of antibiotic therapy. | 2 | Abdominal applications of diffusion-weighted magnetic resonance imaging: Where do we stand?显示文摘Diffusion-weighted imaging (DWI) is one of the magnetic resonance imaging (MRI) sequences providing qualitative as well as quantitative information at a cellular level. It has been widely used for various applications in the central nervous system. Over the past decade, various extracranial applications of DWI have been increasingly explored, as it may detect changes even before signal alterations or morphological abnormalities become apparent on other pulse sequences. Initial results from abdominal MRI applications are promising, particularly in oncological settings and for the detection of abscesses. | 3 | 腺苷激发与运动^(201)Tl心肌灌注断层显像对冠心病诊断的比较显示文摘对静息心电图正常的81例冠心病患者和10例正常对照者进行腺苷激发与117例冠心病患者和16例正常对照者进行运动201Tl单光子发射型心肌灌注断层(心肌SPECT)显像。结果:(1)腺苷激发与运动201Tl心肌SPECT显像对冠心病心肌缺血诊断的敏感性分别为79%与81%,特异性各为80%与81%;腺苷激发与运动心肌SPECT显像在1支、2支、3支血管狭窄三亚组对心肌缺血诊断的敏感性分别为74%与89%,80%与71%,82%与88%;两种方法比较差异均无显著性(P>0.05)。(2)腺苷试验在冠心病患者中的药物副作用占89%,绝大多数症状轻微,于停药后迅速消失。提示,腺苷激发心肌SPECT显像是一安全而有效的冠心病心肌缺血诊断方法,其诊断价值与运动心肌SPECT显像相似。 | 王雄 Joseph K Russell Jaekyeong Heo | 1997 | 中华心血管病杂志1997，25，6： | 6 |
| 4 | Novel susceptibility genes in inflammatory bowel disease显示文摘The inflammatory bowel disease, Crohn's disease and ulcerative colitis, are polygenic disorders with important environmental interactions. To date, the most widely adopted approach to identifying susceptibility genes in complex diseases has involved genome wide linkage studies followed by studies of positional candidate genes in loci of interest. This review encompasses data from studies into novel candidate genes implicated in the pathogenesis of inflammatory bowel disease. | 5 | Preoperative immunonutrition in patients undergoing liver resection:A prospective randomized trial显示文摘BACKGROUND Preoperative supplementation with immunonutrients, including arginine and n-3 fatty acids, has been shown in a number of systematic reviews to reduce infectious complications in patients who have undergone gastrointestinal surgery. Limited information, however, is available on the benefits of nutritional supplementation enriched with arginine and n-3 fatty acids in patients undergoing liver resection.AIM To evaluate the effects of preoperative nutritional supplementation enriched with arginine and n-3 fatty acids on inflammatory and immunologic markers and clinical outcome in patients undergoing liver resection.METHODS Thirty-four patients undergoing liver resection were randomized to either five days of preoperative Impact? [1020 kcal/d, immunonutrition(IMN) group], or standard care [no supplementation, standard care(STD) group]. Nutritional status was measured at study entry by subjective global assessment(SGA).Functional assessments(grip strength, fatigue and performance status) were carried out at study entry, on the day prior to surgery, and on postoperative day(POD) 7 and 30. Inflammatory and immune markers were measured at study entry, on the day prior to surgery, and POD 1, 3, 5, 7, 10 and 30. Postoperative complications were recorded prospectively until POD30.RESULTS A total of 32 patients(17 IMN and 15 STD) were analysed. All except four patients were SGA class A. The plasma ratio of(eicosapentaenoic acid plus docosahexaenoic acid) to arachidonic acid was higher in IMN patients on the day prior to surgery and POD 1, 3, 5 and 7(P < 0.05). Plasma interleukin(IL)-6 concentrations were elevated in the IMN group(P = 0.017 for POD7). No treatment effect was detected for functional measures, immune response(white cell count and total lymphocytes) or markers of inflammation(C-reactive protein,tumour necrosis factor-α, IL-8, IL-10). There were 10 patients with infectious complications in the IMN group and 4 in the STD group(P = 0.087). | 15 | Sarcina ventriculi of the stomach:A case report显示文摘Sarcina ventriculi is a Gram positive organism,which has been reported to be found rarely,in the gastric specimens of patients with gastroparesis.Only eight cases of Sarcina,isolated from gastric specimens have been reported so far.Sarcina has been implicated in the development of gastric ulcers,emphysematous gastritis and gastric perforation.We report a case of 73-year-old male,with history of prior Billroth Ⅱ surgery and truncal vagotomy,who presented for further evaluation of iron deficiency anemia.An upper endoscopy revealed diffuse gastric erythema,along with retained food.Biopsies revealed marked inflammation with ulcer bed formation and presence of Sarcina organisms.The patient was treated with ciprofloxacin and metronidazole for 1 wk,and a repeat endoscopy showed improvement of erythema,along with clearance of Sarcina organisms.Review of reported cases including ours suggests that Sarcina is more frequently an innocent bystander rather than a pathogenic organism.However,given its association with life threatening illness in two reported cases,it may be prudent to treat with antibiotics and anti-ulcer therapy,until further understanding is achieved. | Shiva K Ratuapli Dora M Lam-Himlin Russell I Heigh | 2013 | World Journal of Gastroenterology2013，19，14： | 1 |
