| 2 | Outcomes of severe sepsis and septic shock patients after stratifi cation by initial lactate value显示文摘BACKGROUND: In the setting of severe sepsis and septic shock, mortality increases when lactate levels are ≥ 4 mmol/L. However, the consequences of lower lactate levels in this population are not well understood. The study aimed to determine the in-hospital mortality associated with severe sepsis and septic shock when initial lactate levels are < 4 mmol/L.METHODS: This is a retrospective cohort study of septic patients admitted over a 40-month period. Totally 338 patients were divided into three groups based on initial lactate values. Group 1 had lactate levels < 2 mmol/L; group 2: 2–4 mmol/L; and group 3: ≥ 4 mmol/L. The primary outcome was in-hospital mortality.RESULTS: There were 111 patients in group 1, 96 patients in group 2, and 131 in group 3. The mortality rates were 21.6%, 35.4%, and 51.9% respectively. Univariate analysis revealed the mortality differences to be statistically significant. Multivariate logistic regression demonstrated higher odds of death with higher lactate tier group, however the findings did not reach statistical significance.CONCLUSION: This study found that only assignment to group 3, initial lactic acid level of ≥ 4 mmol/L, was independently associated with increased mortality after correcting for underlying severity of illness and organ dysfunction. | 4 | Forecasting of Software Reliability Using Neighborhood Fuzzy Particle Swarm Optimization Based Novel Neural Network显示文摘This paper proposes an artificial neural network(ANN) based software reliability model trained by novel particle swarm optimization(PSO) algorithm for enhanced forecasting of the reliability of software. The proposed ANN is developed considering the fault generation phenomenon during software testing with the fault complexity of different levels. We demonstrate the proposed model considering three types of faults residing in the software. We propose a neighborhood based fuzzy PSO algorithm for competent learning of the proposed ANN using software failure data. Fitting and prediction performances of the neighborhood fuzzy PSO based proposed neural network model are compared with the standard PSO based proposed neural network model and existing ANN based software reliability models in the literature through three real software failure data sets. We also compare the performance of the proposed PSO algorithm with the standard PSO algorithm through learning of the proposed ANN. Statistical analysis shows that the neighborhood fuzzy PSO based proposed neural network model has comparatively better fitting and predictive ability than the standard PSO based proposed neural network model and other ANN based software reliability models. | 7 | Rapid Biosynthesis of Silver Nanoparticles Using Cymbopogan Citratus (Lemongrass) and its Antimicrobial Activity显示文摘The present study deals with the rapid green synthesis of silver nanoparticles using fresh leaves of Cymbopogan Citratus(Lemongrass). Silver nanoparticles were formed within 8~10 minutes by microwave irradiation using aqueous solution of Ag NO3(1 m M) with fresh leaves extract of Cymbopogan Citratus. The synthesized silver nanoparticles were characterized by using UV-visible spectrophotometer analysis, nanoparticle tracking analyzer, transmission electron microscope and energy dispersive X-ray spectra. The antibacterial activity of these nanoparticles was studied against multiple drug resistant hospital isolates of E.coli, S.aureus,P.mirabilis and hospital isolates of S. typhi, K.pnuemoniae. Also, the antifungal activity of these nanoparticles was studied against C.albicans(hospital isolate) and A.niger(NCIM 616). The synergistic effect of silver nanoparticles along with antibiotics was also studied against multiple drug resistant hospital isolates and found to be effective. | 9 | Effect of Biosynthesized Silver Nanoparticles on Staphylococcus aureus Biofilm Quenching and Prevention of Biofilm Formation显示文摘The development of green experimental processes for the synthesis of nanoparticles is a need in the field of nanotechnology. The synthesis of silver nanoparticles was achieved using Bacillus cereus supernatant and1 m M silver nitrate. 100 m M glucose was found to quicken the rate of reaction of silver nanoparticles synthesis.UV-visible spectrophotometric analysis was carried out to assess the synthesis of silver nanoparticles. The synthesized silver nanoparticles were further characterized by using Nanoparticle Tracking Analyzer(NTA),Transmission Electron Microscope and Energy Dispersive X-ray spectra. These silver nanoparticles showed enhanced quorum quenching activity against Staphylococcus aureus biofilm and prevention of biofilm formation which can be seen under inverted microscope(40 X). The synergistic effect of silver nanoparticles along with antibiotics in biofilm quenching was found to be effective.
