1世界家庭医生组织(WONCA)研究论文摘要汇编——高度连续的全科医疗服务可降低患者医疗费用和住院率显示文摘目的连续性服务是全科医疗的基本特征之一,其可以降低医疗成本、改善医疗公平性、提高医疗服务质量。但目前以医学专业角度,包括利用联邦医疗质量支付项目,对全科医疗服务的连续性价值进行评估的研究较为缺乏。本研究采用常用的以全科医师视角评估全科医疗服务连续性的4项指标(特定照护者连续性指数、BiceBoxerman照护连续性指标、修订连续性指标、赫芬达尔指数),评估医保参保患者接受的全科医疗服务的连续性,并分析其与医疗费用和住院率的关系。方法数据来源于经6 551例全科医师诊治的1 448 952例医保参保患者的医保报销资料。采用上述4项指标对患者接受的全科医疗服务的连续性进行评估。将接受诊疗的每例患者评价的连续性得分加权平均后,得到该例医师的连续性得分。以患者特征、全科医师特征、农村服务范围建立模型。将患者花费的可被医保A&B(住院保险和补充性医保)报销的实际医疗费用取自然对数后,采用多元线性回归分析全科医疗服务连续性与医疗费用的关系。采用Logistic回归分析全科医疗服务连续性与住院率的关系。结果对4项指标的评估结果均显示,全科医疗服务连续性与医疗费用和住院率呈高度相关,相关系数为0.86~0.99,即高度连续的全科医疗服务可以降低患者的医疗费用和住院率。在调整患者特征和全科医师特征后,将Bice-Boxerman照护连续性得分5等分,得分最高组患者较最低组患者的医疗费用降低了14.1%[($8 092比$6 958,β=-0.151,95%CI(-0.186,-0.116)],住院率降低了16.1%[OR=0.839,95%CI(0.787,0.893)]。结论高度连续的全科医疗服务可以降低医疗费用和住院率。本研究采用的4项评估指标可能与联邦医疗质量支付项目一样有效,鉴于较高的医疗资源利用率有利于降低医疗费用,该4项指标也可以用来评估医疗资源的利用情况。本刊编辑部 BAZEMORE A PETTERSON S PETERSON L E 2019中国全科医学2019,22,1:2
