| 1 | Rubber band ligation for 750 cases of symptomatic hemorrhoids out of 2200 cases显示文摘AIM: To study the results for the treatment of symptomatic hemorrhoids using rubber band ligation (RBL) method. METHODS: A retrospective study for 750 patients who came to the colorectal unit from June, 1998 to September, 2006, data was retrieved from archived fi les. RBL was performed using the Mc Gown applicator on an outpatient basis. The patients were asked to return to out-patient clinic for follow up at 2 wk, 1 mo, 6 mo and through telephone call every 6 mo for 2 years). RESULTS: After RBL, 696 patients (92.8%) were cured with no difference in outcome for second or third degree hemorrhoids (P = 0.31). Symptomatic recurrence was detected in 11.04% after 2 years. A total of 52 patients (6.93%) had 77 complications from RBL which required no hospitalization. Complications were pain, rectal bleeding and vaso-vagal symptoms(4.13%, 4.13% and 1.33% of patients, respectively). At 1 mo there were a significant improvement in mean SF-36 scores over baseline in five items, while after 2 years there were improvement in all items over baseline, but not significant. No significant manometeric changes after band ligation. CONCLUSION: RBL is a simple, safe and effective method for treating symptomatic second and third degree hemorrhoids as an out patient procedure with signifi cant improvement in quality of life. | 5 | Systematic design method for PI controller with Virtual Resistor-based Active Damping of LCL filter显示文摘The Virtual Resistor based Active Damping(VR-AD) is widely employed in converters connected to the grid via LCL filters in order to mitigate the inherent resonance of the filters. Nevertheless, in digitally controlled systems, the PWM and the calculating delays modify the system characteristics in terms of frequency and phase, thus destabilizing the system and degrading the VR-AD performances, mainly in low switching frequencies. Moreover, the stability of the system is greatly affected under weak grid operation characterized by large grid impedance variation. This paper solves these problems by proposing a systematic, robust and optimized design procedure of voltage oriented PI control(VOC) with VRAD. The considered design procedure ensures robust control(sufficient stability margins) and high quality of grid current(reduced steady-state error and minimized THD value) despite the negative impact of digital time delay, grid impedance variation and filter parameters change. | 6 | Sentinel lymph node biopsy in clinically detected ductal carcinoma in situ显示文摘AIM:To study the indications for sentinel lymph node biopsy(SLNB) in clinically-detected ductal carcinoma in situ(CD-DCIS).METHODS:A retrospective analysis of 20 patients with an initial diagnosis of pure DCIS by an image-guided core needle biopsy(CNB) between June 2006 and June 2012 was conducted at King Faisal Specialist Hospital.The accuracy of performing SLNB in CD-DCIS,the rate of sentinel and non-sentinel nodal metastasis,and the histologic underestimation rate of invasive cancer at initial diagnosis were analyzed.The inclusion criteria were a preoperative diagnosis of pure DCIS with no evidence of invasion.We excluded any patient with evidence of microinvasion or invasion.There were two cases of mammographically detected DCIS and 18 cases of CDDCIS.All our patients were diagnosed by an imageguided CNB except two patients who were diagnosed by fine needle aspiration(FNA).All patients underwent breast surgery,SLNB,and axillary lymph node dissection(ALND) if the SLN was positive.RESULTS:Twenty patients with an initial diagnosis of pure DCIS underwent SLNB,2 of whom had an ALND.The mean age of the patients was 49.7 years(range,35-70).Twelve patients(60%) were premenopausal and 8(40%) were postmenopausal.CNB was the diagnostic procedure for 18 patients,and 2 who were diagnosed by FNA were excluded from the calculation of the underestimation rate.Two out of 20 had a positive SLNB and underwent an ALND and neither had additional non sentinel lymph node metastasis.Both the sentinel visualization rate and the intraoperative sentinel identification rate were 100%.The false negative rate was 0%.Only 2 patients had a positive SLNB(10%) and neither had additional metastasis following an ALND.After definitive surgery,3 patients were upstaged to invasive ductal carcinoma(3/18 = 16.6%) and 3 other patients were upstaged to DCIS with microinvasion(3/18 = 16.6%).Therefore the histologic underestimation rate of invasive disease was 33%.CONCLUSION:SLNB in CD-DCIS is technically feasible and highly accurate.We recommend limiting SLNB to patients undergoing a mastectomy. | Ahmed Yahia Al-Ameer Sahar Al Nefaie Badria Al Johani Ihab Anwar Taher Al Tweigeri Asma Tulbah Mohmmed Alshabanah Osama Al Malik | 2016 | World Journal of Clinical Oncology2016,7,2: | 3 |

| 10 | Effect of catalyst layer mesoscopic pore-morphology on cold start process of PEM fuel cells显示文摘Water transport is of paramount importance to the cold start of proton exchange membrane fuel cells(PEMFCs).Analysis of water transport in cathode catalyst layer(CCL)during cold start reveals the distinct characteristics from the normal temperature operation.This work studies the effect of CCL mesoscopic pore-morphology on PEMFC cold start.The CCL mesoscale morphology is characterized by two tortuosity factors of the ionomer network and pore structure,respectively.The simulation results demonstrate that the mesoscale morphology of CCL has a significant influence on the performance of PEMFC cold start.It was found that cold-starting of a cell with a CCL of less tortuous mesoscale morphology can succeed,whereas starting up a cell with a CCL of more tortuous mesoscale morphology may fail.The CCL of less tortuous pore structure reduces the water back diffusion resistance from the CCL to proton exchange membrane(PEM),thus enhancing the water storage in PEM,while reducing the tortuosity in ionomer network of CCL is found to enhance the water transport in and the water removal from CCL.For the sake of better cold start performance,novel preparation methods,which can create catalyst layers of larger size primary pores and less tortuous pore structure and ionomer network,are desirable. | Ahmed Mohmed DAFALLA Fangming JIANG | 2021 | Frontiers in Energy2021,15,2: | 2 |
