| 1 | Characteristics of hepatocellular carcinoma in cirrhotic and non-cirrhotic non-alcoholic fatty liver disease显示文摘AIM: To determine characteristics and prognosticpredictors of patients with hepatocellular carcinoma(HCC) in association with non-alcoholic fatty liver disease(NAFLD).METHODS: We reviewed the records of all patients with NAFLD associated HCC between 2000 and 2012. Data collected included demographics; histology; presence or absence of cirrhosis, size and number of HCC, alpha-fetoprotein, body mass index(BMI), and the presence of diabetes, hypertension, or dyslipidaemia.RESULTS: Fifty-four patients with NAFLD associated HCC were identified. Mean age was 64 years with 87% male. Fifteen percent(8/54) were not cirrhotic. 11%, 24% and 50% had a BMI of < 25 kg/m2, 25-29 kg/m2 and ≥ 30 kg/m2 respectively. Fifty-nine percent were diabetic, 44% hypertensive and 26% hyperlipidaemic. Thirty-four percent of the patients had ≤ 1 of these risk factors. Non-cirrhotics had a significantly larger mean tumour diameter at diagnosis than cirrhotics(P = 0.041). Multivariate analysis did not identify any other patient characteristics that predicted the size or number of HCC.CONCLUSION: HCC can develop in NAFLD without cirrhosis. | 2 | Cyclin A2:At the crossroads of cell cycle and cell invasion显示文摘Cyclin A2 is an essential regulator of the cell division cycle through the activation of kinases that participate to the regulation of S phase as well as the mitotic entry. However,whereas its degradation by the proteasome in mid mitosis was thought to be essential for mitosis to proceed,recent observations show that a small fraction of cyclin A2 persists beyond metaphase and is degraded by autophagy. Its implication in the control of cytoskeletal dynamics and cell movement has unveiled its role in the modulation of Rho A activity. Since this GTPase is involved in both cell rounding early in mitosis and later,in the formation of the cleavage furrow,this suggests that cyclin A2 is a novel actor in cytokinesis. | 4 | Impact of the 2010 resuscitation guidelines training on layperson chest compressions显示文摘BACKGROUND: Survival from cardiac arrest is sensitive to the quality of delivered CPR. In 2010, updated international resuscitation guidelines emphasized deeper chest compressions and faster rates, yet it is unknown whether training laypersons using updated guidelines resulted in changed CPR performance. We hypothesized that laypersons taught CPR using the 2010 guidelines performed deeper and faster compressions than those taught using the 2005 materials.METHODS: This work represents a secondary analysis of a study conducted at eight hospitals where family members of hospitalized cardiac patients were trained in CPR. An initial cohort was trained using the 2005 guidelines, and a subsequent cohort was trained using the 2010 guideline materials. Post training, CPR skills were quantified using a recording manikin.RESULTS: Between May 2009 to August 2013, 338 subjects completed the assessment. Among the subjects, 176 received 2005 training and 162 underwent 2010 training. The mean compression rate in the 2005 cohort was 87(95%CI 83–90) per minute, and in the 2010 cohort was 86(95%CI 83–90) per minute(P=ns), while the mean compression depth was 34(95%CI 32–35) mm in the 2005 cohort and 46(95%CI 44–47) mm in the 2010 cohort(P<0.01).CONCLUSIONS: Training with the 2010 CPR guidelines resulted in a statistically significant increase in trainees' compression depth but there was no change in compression rate. | 14 | 超声定位与体表标志定位在儿童髂腹股沟-髂腹下神经阻滞中的比较:罗哌卡因血浆浓度测定的临床指导意义显示文摘背景儿童髂腹股沟-髂腹下神经阻滞常导致血浆局部麻醉药浓度过高。超声引导能为注射局麻药提供准确的解剖定位,从而影响局麻药的血浆浓度变化。为此,我们比较了超声定位与体表标志定位下行神经阻滞对罗哌卡因血浆浓度的影响。方法在全麻诱导下,将66例准备行腹股沟疝修补术的儿童随机分为两组,年龄为8个月-7岁,一组在超声定位下行髂腹股沟-髂腹下神经阻滞(n=35),另一组在体表标志定位下行髂腹股沟-髂腹下神经阻滞(n=31),均使用0.5%的罗哌卡因0.25ml/kg(1.25mg/kg),分别在给药后0、5、10、20、30分钟用高效液相色谱法测罗哌卡因的血浆浓度,观察最大血浆浓度(Cmax),达峰时间(tmax),吸收率常数(ka),在时间0分钟时血浆浓度的上升速度(dC0/d1)和曲线下面积值(AUC)。结果使用超声定位技术阻滞患儿的Cmax(sd)、ka、dC0/dt和AUC的值均高于使用标志定位技术的值,而tmax的值也短于使用标志定位技术的值。[Cmax:1.78(0.62)vs1.23(0.70)μg/ml,P〈0.01;ka:14.4(10.7)vs11.7(11.4)h^-1,P〈0.05;dC0/d1:0.26(0.12)vs0.15(0.03)μg·ml^-1·min^-1,P〈0.01;AUC:42.4(15.9)vs27.2(18.1)μg·30min^-1·ml^-1,P〈0.001;tmax:20.4(8.6)vs25.3(7.6)分钟,P〈0.05)。结论药物代谢动力学数据表明:与体表标志定位相比,在超声定位下行髂腹股沟-髂腹下神经阻滞局麻药的最大血浆浓度更高,药物吸收也更快。因此,在超声定位下行髂腹股沟-髂腹下神经阻滞时需要考虑减少局麻药的用量。 | Marion Weintraud, MD Marit Lundblad, MD Stephan C.
