6Change in neoadjuvant chemotherapy could alter the prognosis of patients with pancreatic adenocarcinoma: A case report显示文摘BACKGROUND Neoadjuvant treatment has become a standard of care for borderline or locally advanced pancreatic cancer and is increasingly considered even for up-front resectable disease.The aim of this article is to present the case of a 62-year-old patient with locally advanced pancreatic adenocarcinoma who was successfully treated with gemcitabine plus nab-paclitaxel after the failure of the first line treatment.CASE SUMMARY Computerized tomography scan and magnetic resonance imaging demonstrated a nodular lesion of ill-defined limits in the body of the pancreas,measuring approximately 4.2 cm×2.7 cm,with an infiltrative aspect.The tumor had contact with the superior mesenteric vein,splenomesenteric junction and the proximal segment of the splenic artery,causing focal reduction of its lumens.Due to vascular involvement,neoadjuvant chemotherapy treatment with eight cycles of“folinic acid,5-fluorouracil,irinotecan and oxa-liplatine”(FOLFIRINOX)were performed.At the end of the cycles,surgery was performed,but the procedure was interrupted due to finding of lesions suspected of metastasis.Gemcitabine plus nab-paclitaxel was then successfully used for neoadjuvant treatment with subsequent R0 surgical resection.CONCLUSION Gemcitabine plus nab-paclitaxel may be effective as an alternative regimen when FOLFIRINOX fails as the first line of treatment,suggesting the need for further studies to identify which patients would benefit from each type of therapeutic approach.Alberto Meyer Bárbara J Carvalho Kayo AA Medeiros Leonardo Z Pipek Fernanda S Nascimento Milena O Suzuki João VT Munhoz Leandro R Iuamoto Luiz A Carneiro-D'Alburquerque Wellington Andraus 2021World Journal of Clinical Cases2021,9,14:0
7热带条件下乳酸菌素对紫花苜蓿青贮饲料的发酵和化学成分的影响显示文摘热带条件下,对不同发酵时间(T)下不同微生物菌剂(MI)处理的紫花苜蓿青贮饲料发酵过程、化学成分和微生物种群进行评估。将新鲜紫花苜蓿随机分配到4×6方阵中,分为无添加组(CTRL)、商业发酵剂组(CIN)、乳酸片球菌组(S1)和戊糖片球菌组(S2)4组,发酵剂添加量为106cfu·g^(-1),发酵时间分别为1、3、7、14、28和56 d,设3个重复。紫花苜蓿在初花期阶段播种后82 d收割,切成1.5 cm长,密封在25×35 cm真空密封塑料袋中,在地窖贮藏。贮存前乳酸菌、肠道菌、霉菌和酵母菌的数量分别为5.42、5.58、4.82和4.8 cfu·g^(-1),56 d后对紫花苜蓿的化学成分进行检测。在T×MI作用下,除乳酸和丙酸的浓度,所有参数均相互影响。1~28 d内S1组和S2组的p H比CTRL组低,56 d时两组p H相近。7或14 d时,明显观察到S1组和S2组乳酸浓度较高。而3、28 d时,S1组和S2组的乳酸浓度低于CTRL组和CIN组。MI不同或MI×T交互作用并未影响微生物种群数量,但肠道菌数量仍有小幅波动,在1~14 d时递减,之后缓慢上升。56 d时,地窖贮存的紫花苜化学成分并没有受到微生物制剂种类的影响。试验结果表明,戊糖片球菌在发酵过程中最有效,能迅速降低p H,增加乳酸浓度,是一种有潜力的青贮饲料发酵剂。Zhang C X Huang K K Lu K L Wang L Song K Zhang L Li P Silva V P Pereira O G Leandro E S Da Silva T C Ribeiro K G Mantovani H C Santos S A 2016饲料博览2016,0,4:0
