1Early precut sphincterotomy and the risk of endoscopic retrograde cholangiopancreatography related complications: An updated meta-analysis显示文摘AIM: To study the cannulation and complication rates of early pre-cut sphincterotomy vs persistent attempts at cannulation by standard approach.METHODS: Systematic search of PubMed, EMBASE, Web of Science, and the Cochrane Library for relevant studies published up to February 2013. The main outcome measurements were cannulation rates and postendoscopic retrograde cholangiopancreatography(ERCP) complications. A comprehensive systematic search of the Cochrane library, PubMed, Google scholar, Scopus, National Institutes of Health, meta-register of controlled trials and published proceedings from major Gastroenterology journals and meetings until February 2013 was conducted using keywords. All Prospective randomized controlled trials(RCT) studies whichmet our inclusion criteria were included in the analysis. Prospective non-randomized studies and retrospective studies were excluded from our meta-analysis. The main outcomes of interest were post-ERCP pancreatitis, overall complication rates including cholangitis, ERCPrelated bleeding, perforation and cannulation success rates. RESULTS: Seven RCTs with a total of 1039 patients were included in the meta-analysis based on selection criteria. The overall cannulation rate was 90% in the pre-cut sphincterotomy vs 86.3% in the persistent attempts group(OR = 1.98; 95%CI: 0.70-5.65). The risk of post-ERCP pancreatitis(PEP) was not different between the two groups(3.9% in the pre-cut sphincterotomy vs 6.1% in the persistent attempts group, OR = 0.58, 95%CI: 0.32-1.05). Similarly, there was no statistically significant difference between the groups for overall complication rate including PEP, cholangitis, bleeding, and perforation(6.2% vs 6.9%, OR = 0.85, 95%CI: 0.51-1.41). CONCLUSION: This meta-analysis suggests that precut sphincterotomy and persistent attempts at cannulation are comparable in terms of overall complication rates. 2Modeling and multi-response optimization of machining performance while turning hardened steel with self-propelled rotary tool显示文摘There are many advanced tooling approaches in metal cutting to enhance the cutting tool performance for machining hard-to-cut materials.The self propelled rotary tool(SPRT) is one of the novel approaches to improve the cutting tool performance by providing cutting edge in the form of a disk,which rotates about its principal axis and provides a rest period for the cutting edge to cool and allow engaging a fresh cutting edge with the work piece.This paper aimed to present the cutting performance of SPRT while turning hardened EN24 steel and optimize the machining conditions.Surface roughness(R_a) and metal removal rate(r_(MMR)) are considered as machining performance parameters to evaluate,while the horizontal inclination angle of the SPRT,depth of cut,feed rate and spindle speed are considered as process variables.Initially,design of experiments(DOEs) is employed to minimize the number of experiments.For each set of chosen process variables,the machining experiments are conducted on computer numerical control(CNC) lathe to measure the machining responses.Then,the response surface methodology(RSM) is used to establish quantitative relationships for the output responses in terms of the input variables.Analysis of variance(ANOVA) is used to check the adequacy of the model.The influence of input variables on the output responses is also determined.Consequently,these models are formulated as a multi-response optimization problem to minimize the R_a and maximize the r_(MMR)simultaneously.Non-dominated sorting genetic algorithmII(NSGA-II) is used to derive the set of Pareto-optimal solutions.The optimal results obtained through the proposed methodology are also compared with the results of validation experimental runs and good correlation is found between them.Thella Babu Rao A.Gopala Krishna Ramesh Kumar Katta Konjeti Rama Krishna 2015Advances in Manufacturing2015,3,1:2

10Drug induced autoimmune hepatitis:An unfortunate case of herbal toxicity from Skullcap supplement:A case report显示文摘BACKGROUND The surge in traditional herbal dietary supplement(HDS)popularity has led to increased drug-induced liver injuries(DILI).Despite lacking evidence of efficacy and being prohibited from making medical claims,their acceptance has risen over sevenfold in the last two decades,with roughly 25%of United States(US)adults using these supplements monthly.An estimated 23000 emergency room visits annually in the US are linked to HDS side effects.NIH-funded research suggests HDS contribute to 7-20%of DILI cases,with similar trends in Europe—Spain reporting 2%and Iceland up to 16%.Patients with acute liver failure from HDS undergo liver transplantation more frequently than those from prescription medicines.Here we describe a case of drug-induced autoimmune hepatitis due to Skullcap supplements,this association appears to be the first documented instance in literature.CASE SUMMARY A middle-aged Caucasian woman,previously healthy,presented with sudden jaundice.Four months earlier,her liver enzymes were normal.She mentioned recent use of Skullcap mushroom supplements.Tests for chronic liver disease were negative.The first liver biopsy indicated severe resolving drug-induced liver injury.Despite treatment,she was readmitted due to worsening jaundice.Followup tests raised concerns about autoimmune hepatitis.A subsequent biopsy confirmed this diagnosis.The patient responded as expected to stopping the medication with improvement in liver enzymes.CONCLUSION This scenario highlights an uncommon instance of DILI caused by Skullcap supplements.It's crucial for hepatologists to recognize this connection due to the increasing prevalence of herbal supplements.Nimish Thakral Venkata Rajesh Konjeti Fady William Salama 2023World Journal of Hepatology2023,15,12:0
