| 1 | Up-regulation of mitochondrial chaperone TRAP1 in ulcerative colitis associated colorectal cancer显示文摘AIM:To characterize tumor necrosis factor receptorassociated protein 1(TRAP1)expression in the progression of ulcerative colitis(UC)-associated colorectal cancer.METHODS:Chronic UC is an inflammatory bowel disease that predisposes to colorectal cancer.Immunohistochemical analysis was used to evaluate TRAP1expression on tissue microarrays containing colonic tissues from 42 UC progressors(patients with cancer or dysplasia)and 38 non-progressors(dysplasia/cancer free patients).Statistical analyses of the TRAP1immunohistochemistry staining were performed using Graph Pad Prism.Differences in the TRAP1 level between non-progressors and progressors were tested for statistical significance using the Mann-Whitney test.Receiver operating characteristic curve method was used to quantify marker performance in distinguishing diseased cases from controls.RESULTS:TRAP1 was up-regulated in the colon tissues from UC progressors,but not in the colon tissues from UC non-progressors.Moreover,up-regulation of TRAP1 preceded the neoplastic changes:it was present in both the dysplastic and non-dysplastic tissues of UC progressors.When TRAP1 staining in rectal tissue was used as a diagnostic marker,it could distinguish progressors from non-progressors with 59%sensitivity and 80%specificity.Our study further showed that the increase of TRAP1 expression positively correlated with the degree of inflammation in the colorectal cancer tissues,which could be related to the increased oxidation present in the colonic mucosa from UC progressors.We then investigated the cellular proteome changes underlying oxidative stress,and found that oxidative stress could induce up-regulation of TRAP1 along with several other negative modulators of apoptosis.CONCLUSION:These results suggest that oxidative stress in long standing UC could lead to the increase of cytoprotective protein TRAP1,which in turn could promote cancer progression by preventing or protecting the oxidative damaged epithelial cells from undergoing apoptosis.TRAP1 could be a potential diagnostic marker for UC associated colorectal cancer. | Ru Chen Sheng Pan Keith Lai Lisa A Lai David A Crispin Mary P Bronner Teresa A Brentnall | 2014 | World Journal of Gastroenterology2014,20,45: | 9 |

| 5 | Pituitary carcinoma: Two case reports and review of literature显示文摘BACKGROUND Pituitary carcinoma is a rare type of malignancy that can be very difficult to diagnose and treat.Many cases were diagnosed at autopsy.Delays in diagnosis often adversely impact patients’outcomes.Even with prompt diagnosis,treatment decisions remain challenging in the absence of randomized controlled trials.CASE SUMMARY We report two cases of pituitary carcinoma in men with a history of pituitary adenoma.In the first case,a 55-year-old man was initially diagnosed with pituitary macroadenoma.He underwent subtotal debulking of the tumor followed by adjuvant radiotherapy.Subsequently,he developed relapsed disease and multifocal intracranial metastases and a diagnosis of pituitary carcinoma was rendered.He passed away despite several lines of systemic therapies including temozolomide,lomustine and bevacizumab.Another 52-year-old man was diagnosed with atypical pituitary adenoma with presentation of sudden onset of vision loss in the right eye.He had recurrent pituitary carcinoma with spinal metastases,treated with surgery,radiation and temozolomide.CONCLUSION Pituitary carcinoma is a rare neoplasm with poor prognosis that is difficult to diagnose and treat.The small number of cases restricts our ability to design randomized clinical trials.Management is largely driven by retrospective studies and case series.Establishing molecular biomarkers and comprehensive genomic profiling could help in decisions about diagnosis and management of pituitary carcinoma. | Lai Xu Karam Khaddour Jie Chen Keith M Rich Richard J Perrin Jian Li Campian | 2020 | World Journal of Clinical Oncology2020,11,2: | 1 |

| 12 | A brain structural connectivity biomarker for autism spectrum disorder diagnosis in early childhood显示文摘Background:Autism spectrum disorder(ASD)is associated with altered brain development,but it is unclear which specific structural changes may serve as potential diagnostic markers,particularly in young children at the age when symptoms become fully estab-lished.Furthermore,such brain markers need to meet the requirements of precision medicine and be accurate in aiding diagnosis at an individual rather than only a group level.Objective:This study aimed to identify and model brain-wide differences in structural connectivity using diffusion tensor imaging(DTI)in young ASD and typically developing(TD)children.Methods:A discovery cohort including 93 ASD and 26 TD children and two independent validation cohorts including 12 ASD and 9 TD children from three different cities in China were included.Brain-wide(294 regions)structural connectivity was measured using DTI(fractional anisotropy,FA)together with symptom severity and cognitive development.A connection matrix was constructed for each child for comparisons between ASD and TD groups.Pattern classification was performed on the discovery dataset and the resulting model was tested on the two independent validation datasets.Results:Thirty-three structural connections showed increased FA in ASD compared to TD children and associated with both autistic symptom severity and impaired general cognitive development.The majority(29/33)involved the frontal lobe and comprised five different networks with functional relevance to default mode,motor control,social recognition,language and reward.Overall,clas-sification achieved very high accuracy of 96.77%in the discovery dataset,and 91.67%and 88.89%in the two independent validation datasets.Conclusions:Identified structural connectivity differences primarily involving the frontal cortex can very accurately distinguish novel individual ASD from TD children and may therefore represent a robust early brain biomarker which can address the requirements of precision medicine. | Xi Jiang Xiao-Jing Shou Zhongbo Zhao Yuzhong Chen Fan-Chao Meng Jiao Le Tian-Jia Song Xin-Jie Xu Weitong Guo Xiaoyan Ke Xiao-E Cai Weihua Zhao Juan Kou Ran Huo Ying Liu Hui-Shu Yuan Yan Xing Ji-Sheng Han Song-Ping Han Yun Li Hua Lai Lan Zhang Mei-Xiang Jia Jing Liu Xuan Liu Keith M.Kendrick Rong Zhang | 2023 | Psychoradiology2023,3,1: | 0 |

| 14 | The Hong Kong consensus recommendations on the diagnosis and management of pancreatic cystic lesions显示文摘Background:The finding of pancreatic cystic lesions(PCL)on incidental imaging is becoming increasingly common.International studies report a prevalence of 2.2-44.7%depending on the population,imaging modality and indication for imaging,and the prevalence increases with age.Patients with PCL are at risk of developing pancreatic cancer,a disease with a poor prognosis.This publication summarizes recommendations for the diagnosis and management of PCL and post-operative pancreatic exocrine insufficiency(PEI)from a group of local specialists.Methods:Clinical evidence was consolidated from narrative reviews and consensus statements formulated during two online meetings in March 2022.The expert panel included gastroenterologists,hepatobiliary surgeons,oncologists,radiologists,and endocrinologists.Results:Patients with PCL require careful investigation and follow-up due to the risk of malignant transformation of these lesions.They should undergo clinical investigation and pancreas-specific imaging to classify lesions and understand the risk profile of the patient.Where indicated,patients should undergo pancreatectomy to excise PCL.Following pancreatectomy,patients are at risk of PEI,leading to gastrointestinal dysfunction and malnutrition.Therefore,such patients should be monitored for symptoms of PEI,and promptly treated with pancreatic enzyme replacement therapy(PERT).Patients with poor response to PERT may require increases in dose,addition of a proton pump inhibitor,and/or further investigation,including tests for pancreatic function.Patients are also at risk of new-onset diabetes mellitus after pancreatectomy;they should be screened and treated with insulin if indicated.Conclusions:These statements are an accurate summary of our approach to the diagnosis and management of patients with PCL and will be of assistance to clinicians treating these patients in a similar clinical landscape. | Tan-To Cheung Yuk Tong Lee Raymond Shing-Yan Tang Wong Hoi She Kai Chi Cheng Chin Cheung Cheung Keith Wan Hang Chiu Kenneth Siu Ho Chok Wing Sun Chow Tak Wing Lai Wai-Kay Seto Thomas Yau | 2023 | Hepatobiliary Surgery and Nutrition2023,12,5: | 0 |
