| 15 | The Immunome of Colon Cancer: Functional In Silico Analysis of Antigenic Proteins Deduced from IgG Microarray Pro?ling显示文摘Characterization of the colon cancer immunome and its autoantibody signature from differentially-reactive antigens(DIRAGs)could provide insights into aberrant cellular mechanisms or enriched networks associated with diseases.The purpose of this study was to characterize the antibody pro?le of plasma samples from 32 colorectal cancer(CRC)patients and 32 controls using proteins isolated from 15,417 human c DNA expression clones on microarrays.671 unique DIRAGs were identi?ed and 632 were more highly reactive in CRC samples.Bioinformatics analyses reveal that compared to control samples,the immunoproteomic IgG pro?ling of CRC samples is mainly associated with cell death,survival,and proliferation pathways,especially proteins involved in EIF2 and m TOR signaling.Ribosomal proteins(e.g.,RPL7,RPL22,and RPL27A)and CRC-related genes such as APC,AXIN1,E2F4,MSH2,PMS2,and TP53 were highly enriched.In addition,differential pathways were observed between the CRC and control samples.Furthermore,103 DIR-AGs were reported in the SEREX antigen database,demonstrating our ability to identify known and new reactive antigens.We also found an overlap of 7 antigens with 48‘‘CRC genes.'These data indicate that immunomics pro?ling on protein microarrays is able to reveal the complexity of immune responses in cancerous diseases and faithfully re?ects the underlying pathology. | Johana A. | 16 | Escitalopram-induced hepatitis:A case report显示文摘BACKGROUND The antidepressant escitalopram is widely prescribed for the treatment of depression.It is generally well-tolerated,and cholestasis is not mentioned in its summary of product characteristics(Sm PC).We present a case of cholestatic and cytolysis liver injury due to escitalopram and a Vigi Base?study.CASE SUMMARY A 68-year-old man was admitted to our emergency unit due to clinical jaundice associated with hepatitis,pruritus and dark urine.We tested the patient for the most common etiologies of jaundice,including hemolysis,viral hepatitis,cirrhosis,carcinoma,cholangitis,cholelithiasis and intrahepatic or extrahepatic obstruction.The etiological study was negative,and an adverse drug reaction was the sole possible explanation.The patient was receiving treatment with escitalopram.Two days after its withdrawal,pruritus was resolved.Ten days after withdrawal,clinical jaundice disappeared.It took a month and three weeks after withdrawal for the patient to have normalized liver function tests.To our knowledge,this is the first reported case of cholestasis where treatment with escitalopram was the only possible cause,with a highly probable causality.In addition,we determined whether escitalopram is associated with hepatotoxicity and cholestasis by performing a disproportionality analysis.All cases of hepatobiliary disorders induced by escitalopram and reported in the World Health Organization pharmacovigilance database(Vigi Base?)were analyzed to characterize this toxicity.We found that patients treated with escitalopram had an increased risk of hepatitis[odds ratio(OR)=1.938(1.186-3.166)]and cholestasis[OR=1.866(1.279-2.724)][OR(95%confidence interval)].The median duration between the introduction of escitalopram and the occurrence of acute hepatitis and/or cholestasis was ten days+/-seven days.CONCLUSION Although extremely rare,this case report,the review of the literature and the pharmacovigilance update confirm that escitalopram can cause drug-induced hepatotoxicity and cholestasis,generally within a week after initiation.Thus,escitalopram should be withdrawn immediately if an iatrogenic cause cannot be excluded.If its responsibility is ascertained,escitalopram should be consequently contraindicated.In addition,serotoninergic antidepressants in patients with nonsevere depression are ineffective and harmful.Finally,the Sm PC of escitalopram should be updated to alert for this risk and give clear clinical guidelines. | Guillaume Wabont Laurie Ferret Nicolas Houdre Antoine Lepied Johana Bene Etienne Cousein | 2022 | World Journal of Clinical Cases2022,10,8: | 0 |

| 18 | Numerical investigation of tunneling induced surface movement:A case study of MRT line 1,Dhaka显示文摘The proposed underground tunnel for Mass Rapid Transit Line 1,Dhaka brings immense attention to the engineers and experts not only because of its construction challenges through a densely-built city,but also for the potential tunneling-induced ground settlements.This very preliminary study investigates the ground surface movement due to the progression of the tunnel boring machine(TBM)using numerical analysis.A series of finite element(FE)models have been developed using PLAXIS 3D,in which Mohr–Coulomb(MC),modified Cam-Clay(MCC),and hardening soil(HS)have been considered.The in-field data of Mashhad Metro Line-2 have been compared to verify PLAXIS 3D’s efficacy in tunnel modeling.Subsequently,the outcomes of the FE analyses are compared with the existing empirical formulas.The PLAXIS 3D analysis considering the MCC soil model exhibits strong agreement with the real monitored data,with a variance of only 3.85%.After simulating different stages of the tunnel construction,results are reported in terms of the distance of the inflexion point from the center,the settlement trough pattern,the maximum transverse settlements,and the vertical settlements.They are also compared with the established empirical formula.In order to comprehend the surface settlement with various tunnel depths and diameters,a parameter dependency study has been carried out.The analysis findings showed that increasing the TBM’s depth and radius causes the inflexion point’s distance from the center of the tunnel to increase by 4%and decrease by 5%,respectively.It is also observed that as tunnel depth increases,the overall settlement of the tunnel lowers by 11%for every additional 5 meters of depth.The MCC model,out of the three,exhibits the most accurate value of the settlement compared to that obtained from the empirical solutions,and also the best-fit form to the Gaussian curve. | Khondaker Sakil Ahmed Johana Sharmin Mehedi Ahmed Ansary | 2023 | Underground Space2023,,5: | 0 |
