| 1 | Recurrence after endoscopic piecemeal mucosal resection for large sessile colorectal polyps显示文摘AIM: To evaluate the safety and outcomes of endoscopic piecemeal mucosal resection (EPMR) for large sessile colorectal polyps. METHODS: The patients enrolled in this study were 47 patients with 50 large sessile polyps (diameter, 2 cm or greater) who underwent EPMR using a submucosal saline injection technique between December 2002 and October 2005. All medical records, including characteristics of the patients and polyps, complications, and recurrences, were retrospectively reviewed. The first follow-up endoscopic examination was performed at 3-6 mo after initial endoscopic resection, and the second at 12 mo postEPMR. Subsequent surveillance colonoscopic examinations were individualized, taking risk factors into account. RESULTS: The patients were 23 men and 24 women,with a mean age of 60 years. Mean polyp size was 30.1 mm. Of 50 polyps identified, 34 (68%) were benign and 16 (32%) were malignant. There were 6 (12%) cases with EPMR-related bleeding: 5 intra-procedural and 1 early post-procedural bleeding. All bleeding episodes were managed by endoscopic clipping or argon beam coagulation. There were no perforations. Recurrence was identified in 5 cases (12.2%): 4 local recurrences detected at 3 mo post-EPMR and 1 local recurrence detected at 14 mo post-EPMR. The recurrence rate after EPMR was 3.1% for benign polyps and 33.3% for malignant polyps (P < 0.05). Median follow-up time was 37 mo. CONCLUSION: EPMR is safe, but should be applied carefully in malignant polyps. | 2 | Evaluation of esophageal function in patients with esophageal motor abnormalities using multichannel intraluminal impedance esophageal manometry显示文摘AIM: To evaluate the functional aspect of esophageal motility in healthy subjects and in patients who were referred for esophageal function testing using multichannel intraluminal impedance-esophageal manometry (MII-EM), and to assess the clinical utility of MII-EM. METHODS: From September 2003 to January 2004, we performed the MII-EM on healthy volunteers and all the patients who were referred for esophageal function testing. Each patient received 10 liquid and 10 viscous swallows. We analyzed the results, the impedance and the manometric findings. Some of the subjects had additional ambulatory 24-h pH study performed to diagnose gastroesophageal reflux disease (GERD). RESULTS: Among 89 studied subjects, the MII-EM findings showed normal esophageal motility in 50 (56.17%), ineffective esophageal motility (IEM) in 17 (19.10%), nutcracker esophagus in 7 (7.86%), achalasia in 4 (4.49%), and scleroderma esophagus in 11 (12.35%) cases. The completeness and the speed of bolus transit were in the order of nutcracker esophagus, normal manometry and IEM. Some of the swallows showing normal manometry and IEM had incomplete transit. In the achalasia and scleroderma esophagus, almost all the swallows had incomplete transit. The body amplitudes were higher for the swallows with complete transit than for the swallows with incomplete transit. There was not a significant difference in the manometric and impedance findings between the subjects with and without GERD. CONCLUSION: MII-EM is a useful tool in assessing theesophageal function in the patients having esophageal motility abnormality. | 7 | A case of hypereosinophilic syndrome presenting with intractable gastric ulcers显示文摘We report a rare case of hypereosinophilic syndrome(HES)presenting with intractable gastric ulcers.A 71-year-old man was admitted with epigastric pain.Initial endoscopic findings revealed multiple,active gastric ulcers in the gastric antrum.He underwent Helicobacter pylori(H pylori)eradication therapy followed by proton pump inhibitor(PPI)therapy.However,follow-up endoscopy at 4,6,10 and 14 mo revealed persistent multiple gastric ulcers without significant improvement.The proportion of his eosinophil count increased to 43%(total count:7903/mm 3).Abdominal-pelvic and chest computed tomography scans showed multiple small nodules in the liver and both lungs.The endoscopic biopsy specimen taken from the gastric antrum revealed prominent eosinophilic infiltration,and the liver biopsy specimen also showed eosinophilic infiltration in the portal tract and sinusoid.A bone marrow biopsy disclosed eosinophilic hyperplasia as well as increased cellularity of 70%.The patient was finally diagnosed with HES involving the stomach,liver,lung,and bone marrow.When gastric ulcers do not improve despite H pylori eradication and prolonged PPI therapy,infiltrative gastric disorders such as HES should be considered. | Tae Young Park Chang Hwan Choi Suh Yoon Yang InSoo Oh In-Do Song Hyun Woong Lee Hyung Joon Kim Jae Hyuk Do Sae Kyung Chang Ah Ra Cho Young Joo Cha | 2009 | World Journal of Gastroenterology2009，15，48： | 2 |
