| 1 | Anti-HBc screening in Indian blood donors:Still an unresolved issue显示文摘AIM:To study the seroprevalence of antibody to hepatitis B core antigen (anti-HBc) in healthy blood donors negative for HBsAg and to evaluate whether anti-HBc detection could be adopted in India as a screening assay for HBV in addition to HBsAg. METHODS: A total of 1700 serum samples collected from HBsAg-negative healthy blood donors were tested for the presence of anti-HBc antibody (IgM + IgG). All samples reactive for anti-HBc antibody were then investigated for presence of anti-HBs and for liver function tests (LFTs). One hundred serum samples reactive for anti-HBc were tested for HBV DNA by PCR method. RESULTS: Out of 1700 samples tested, 142 (8.4%) blood samples were found to be reactive for anti-HBc. It was signif icantly lower in voluntary (6.9%) as compared to replacement donors (10.4%, P = 0.011). Seventy- two (50.7%) anti-HBc reactive samples were also reactive for anti-HBs with levels > 10 mIU/mL and 70 (49.3%) samples were non-reactive for anti-HBs, these units were labeled as anti-HBc-only. These 142 anti-HBc reactive units were also tested for liver function test. HBV DNA was detected in only 1 of 100 samples tested. CONCLUSION: Keeping in view that 8%-18% of donor population in India is anti-HBc reactive, inclusion of anti- HBc testing will lead to high discard rate. Anti-HBs as proposed previously does not seem to predict clearance of the virus. | 2 | Massive upper gastrointestinal hemorrhage due to invasive hepatocellular carcinoma and hepato-gastric fistula显示文摘A 36-year-old male Asian immigrant with a history of hepatitis B and hepatitis C related unresectable hepatocellular carcinoma in the left lobe of the liver presented with hematemesis and severe anemia.He was diagnosed with a liver mass that was resected 8 years ago described as a benign tumor in his home country.He had received trans-arterial chemoembolization(TACE)four months ago after subsequent diagnosis of unresectable hepatoma,and currently was receiving chemotherapy with Sorafenib.After resuscitation,a contrast enhanced computerized tomography was performed which showed fistulization of hepatocellular carcinoma into adjacent stomach.This finding was confirmed during endoscopy with direct visualization of the fistulous opening.Hepatocellular carcinoma(HCC)invading the gastrointestinal(GI)tract is rare.We present a case and literature review of HCC with local invasion of the stomach causing massive upper GI bleeding after receiving TACE. | Hari Sayana Osama Yousef Wendell K Clarkston | 2013 | World Journal of Gastroenterology2013，19，42： | 5 |
| 3 | Nitrous oxide emissions in nonflooding period from fallow paddy fields显示文摘The study was conducted to investigate the N2O emissions and dissolved N2O in the leachate during the nonflooding period in nongrowing paddy fields.Three kinds of paddy soils were repacked to soil columns and were supersaturated with water initially and dried gradually in a greenhouse to attain the N2O emissions flux during the incubation.Soils with the texture of silty clay-loam (Q and H) produced cracks during the drying of soil,but soil with the texture of silty loam (X) did not form the cracks.Cracked soils had similar amount of N2O emissions,and the mean N2O flux was 1,280.9 and 1,133.3 μg/(m2·h) from Q and H soil,respectively,during the incubation; whereas the mean N2O flux from noncracked X soil was 426.3 μg/(m2·h),i.e.,significantly different from cracked soils.From cracked soils,the diurnal N2O emissions reached two peaks at 14:00 and 2:00,but such emissions peaked only at 2:00 from noncracked soil.The dissolved N2O concentrations in leachates from noncracked soil columns were greater than those from the cracked soil columns,and it indicated that the preferential flow might not affect the amounts of dissolved N2O in leachates during soil cracking.Supersaturated dissolved N2O in the leachate was potential source of N2O emissions.Fallow paddy fields have big risks of N2O emissions during nonflooding periods. | Hari K Pant | 2008 | Journal of Environmental Sciences2008，20，11： | 2 |
