| 1 | Hepatitis C virus recurrence after liver transplantation:A 10-year evaluation显示文摘AIM: To evaluate the predictors of 10-year survival of patients with hepatitis C recurrence. METHODS: Data from 358 patients transplanted between 1989 and 2010 in two Italian transplant centers and with evidence of hepatitis C recurrence were analyzed. A χ2, Fisher's exact test and Kruskal Wallis' test were used for categorical and continuous variables, respectively. Survival analysis was performed at 10 years after transplant using the Kaplan-Meier method, and a log-rank test was used to compare groups. A P level less than 0.05 was considered significant for all tests. Multivariate analysis of the predictive role of different variables on 10-year survival was performed by a stepwise Cox logistic regression.RESULTS: The ten-year survival of the entire population was 61.2%. Five groups of patients were identified according to the virological response or lack of a response to antiviral treatment and, among those who were not treated, according to the clinical status(mild hepatitis C recurrence, 'too sick to be treated' and patients with comorbidities contraindicating the treatment). While the 10-year survival of treated and untreated patients was not different(59.1% vs 64.7%, P = 0.192), patients with a sustained virological response had a higher 10-year survival rate than both the 'non-responders'(84.7% vs 39.8%, P < 0.0001) and too sick to be treated(84.7% vs 0%, P < 0.0001). Sustained virological responders had a survival rate comparable to patients untreated with mild recurrence(84.7% vs 89.3%). A sustained virological response and young donor age were independent predictors of 10-year survival. CONCLUSION: Sustained virological response significantly increased long-term survival. | 5 | Energy analysis of a wood or pellet stove in a single-family house equipped with gas boiler and radiators显示文摘In the residential sector,biomass appliances are widely used for space heating and often combined with other systems.This work aims at comparing the final and primary energy consumption of different configurations,including a conventional and a ducted pellet stove and a wood log stove using air as heat transfer fluid.A dynamic analysis of the interaction between biomass stoves and conventional heating systems,such as gas boilers and radiators,is carried out within a typical single-family house in a mild climate,using TRNSYS software.In addition,natural ventilation of the building is considered using CONTAM,with a focus on external infiltrations and internal air circulation due to the buoyancy effect.Results show that the biomass device in one room promotes the airflows between adjacent thermal zones,enhancing the heat distribution through door openings,in particular when an air ducted stove is present.The final energy consumption resulting from simulations with wood-burning stoves is 21%higher than pellet stoves.The pellet stove results in similar final energy and a 30%increase in overall primary energy,while the wood stove increases the final energy by 22%and approximately 40%of overall primary energy compared to the case of a traditional gas system coupled to radiators which is considered as reference.Nevertheless,non-renewable primary energy savings are higher than 50%with pellet stoves and 60%with wood-log stoves. | Marco Marigo Fabio Zulli Sara Bordignon Laura Carnieletto Giuseppe Emmi Michele De Carli | 2022 | Building Simulation2022,15,9: | 0 |

| 8 | Antiviral therapy of hepatitis C as curative treatment of indolent B-cell lymphoma显示文摘The association of hepatitis C virus(HCV) and B-cell non-Hodgkin lymphomas(NHL) has been highlighted by several epidemiological and biological insights; however the most convincing evidence is represented by interventional studies demonstrating the capability of antiviral treatment(AT) with interferon(IFN) with or without ribavirin to induce the regression of indolent lymphomas, especially of marginal-zone origin. In the largest published retrospective study(100 patients) the overall response rate(ORR) after first-line IFN-based AT was 77%(44% complete responses) and responses were sustainable(median duration of response 33 mo). These results were confirmed by a recent metaanalysis on 254 patients, demonstrating an ORR of 73%. Moreover this analysis confirmed the highly significant correlation between the achievement of viral eradication sustained virological response(SVR) and hematological responses. Two large prospective studies demonstrated that AT is associated with improved survival and argue in favor of current guidelines' recommendation of AT as preferential first-line option in asymptomatic patients with HCV-associated indolent NHL. The recently approved direct-acting antiviral agents(DAAs) revolutionized the treatment of HCV infection, leading to SVR approaching 100% in all genotypes.
