5Plasma soluble C-type lectin-like receptor-2 is associated with the risk of coronary artery disease显示文摘Accumulating evidence suggests that C-type lectin-like receptor-2(CLEC-2)plays an important role in atherothrombosis.In this case-control study,we investigated the association between CLEC-2 and incidence of coronary artery disease(CAD).A total of 216 patients,including 14 cases of stable angina pectoris(SAP,non-ACS)and 202 cases of acute coronary syndrome(ACS),and 89 non-CAD control subjects were enrolled.Plasma levels of soluble CLEC-2(sCLEC-2)were measured using the enzyme-linked immunosorbent assay(ELISA).Compared with the control group(65.69(55.36–143.22)pg/mL),the plasma levels of sCLEC-2 were significantly increased in patients with CAD(133.67(88.76–220.09)pg/mL)and ACS(134.16(88.88–225.81)pg/mL).The multivariate adjusted odds ratios(95%confidence interval)of CAD reached 2.01(1.52–2.66)(Ptrend<0.001)for each 1-quartile increase in sCLEC-2.Restricted cubic splines showed a positive dose-response association between sCLEC2 and CAD incidence(Plinearity<0.001).The addition of sCLEC-2 to conventional risk factors improved the C statistic(0.821 vs.0.761,P=0.004)and reclassification ability(net reclassification improvement:57.45%,P<0.001;integrated discrimination improvement:8.27%,P<0.001)for CAD.In conclusion,high plasma sCLEC-2 is independently associated with CAD risk,and the prognostic value of sCLEC-2 may be evaluated in future prospective studies.Min Fei Li Xiang Xichen Chai Jingchun Jin Tao You Yiming Zhao Changgeng Ruan Yiwen Hao Li Zhu 2020Frontiers of Medicine2020,14,1:5

8Narrow-band imaging endoscopy with and without magnification in diagnosis of colorectal neoplasia显示文摘AIM:To evaluate the diagnostic efficacies of narrowband imaging(NBI) endoscopy with and without high magnification in distinguishing neoplasia from nonneoplasia colorectal lesions.METHODS:A total of 118 patients with 123 colorectal lesions examined by NBI endoscopy in the Zhejiang Provincial People's Hospital from September 2008 to April 2010 were enrolled in this study.These lesions were classified by pit pattern and capillary pattern,and then assessed by histopathology.RESULTS:Ten lesions not meeting the diagnostic criteria were excuded,the overall diagnostic accuracy of NBI endoscopy in distinguishing neoplasia from non-neoplasia colorectal lesions was 91.2%(103/113),and that of NBI endoscopy with and without high magnification was 93.0%(40/43) and 90.0%(63/70),respectively.Both were significantly higher than that of conventional colonoscopy reported in the literature(P < 0.05),but there was no significant difference between the two groups(P > 0.05).CONCLUSION:Besides NBI magnifying endoscopy,NBI endoscopy without magnification may also be used to distinguish neoplasia from non-neoplasia colorectal lesions.Qing-Jie Zhou Jian-Min Yang Bao-Ying Fei Qi-Shun Xu wei-Quan wu Hong-Jun Ruan 2011World Journal of Gastroenterology2011,17,5:4
9Research progress of space non-pyrotechnic low-shock connection and separation technology(SNLT):A review显示文摘Space missions have become diversified in recent years, where connection and separation devices play a crucial role as key components of various spacecraft. Traditional pyrotechnic devices have the advantages of large carrying capacity, rapid motion and functional reliability. However,their shortcomings such as great release shock, poor safety, unrepeatability and other prominent defects make them unsuitable for new generation spacecraft such as microsatellites to separate at low shock or lock repeatedly, etc. Therefore, it is necessary to develop space non-pyrotechnic low-shock connection and separation devices(SNLD) which are required for advanced aerospace missions. In this paper, the progress of the research on space non-pyrotechnic low-shock connection and separation technology(SNLT) is summarized and reviewed. Proceed from the principle of reducing shock for non-pyrotechnic devices, present studies are classified from the perspective of actuating technology and systematic designing methods. For non-pyrotechnic actuating techniques,according to different driving sources, the separation devices are classified into several main categories: electric, magnetic, gas and thermal actuating devices. The actuation principle and application prospect of separation techniques are introduced and the working process, dimension and mechanical properties of typical devices are compared and evaluated. For the systematic designing method, the common mechanism types of SNLDs are summarized according to the designing concept of reducing shock. Then connection configurations are classified according to the structural forms of connection devices, of which the principles, bearing capacities and general applications are discussed. 16EDOA: an efficient delay optimization approach for mixed-polarity Reed-Muller logic circuits under the unit delay model显示文摘Delay optimization has recently attracted signif-icant attention. However, few studies have focused on the delay optimization of mixed-polarity Reed-Muller (MPRM) logic circuits. In this paper, we propose an efficient delay op-timization approach (EDOA) for MPRM logic circuits under the unit delay model, which can derive an optimal MPRM logic circuit with minimum delay. First, the simplest MPRM expression with the fewest number of product terms is ob-tained using a novel Reed-Muller expression simplification approach (RMESA) considering don't-care terms. Second, a minimum delay decomposition approach based on a Huffman tree construction algorithm is utilized on the simplest MPRM expression. Experimental results on MCNC benchmark cir-cuits demonstrate that compared to the Berkeley SIS 1.2 and ABC, the EDOA can significantly reduce delay for most cir-cuits. 18Rational application of gefitinib in NSCLC patients with sensitive EGFR mutations based on pharmacokinetics and metabolomics显示文摘Gefitinib has been available in the market for 20 years,but its pharmacokinetic mechanism of response is little known.In this study,we examined the pharmacokinetic and metabolomic profiles in non-small cell lung cancer(NSCLC)patients with sensitive EGFR mutations.A total of 216 advanced NSCLC patients were enrolled,and administered gefitinib at the standard dosage of 250 mg/day,which was established in heterogeneous subjects with non-sensitive mutations.We identified and quantified three main metabolites(named as M1,M2 and M3)in the plasma of patients,the correlations between the concentration of gefitinib/metabolites and efficacy were analyzed.In exploratory and validation set,gefitinib concentration was not correlated with clinical effects.Considering the result that the therapeutic effects of 250 mg/2-day was better than that of 250 mg/day in a multiple center clinical trial,the standard dose might be higher than that for maximal efficacy according to the hypothetical dose-response curve.Among the three metabolites,the IC50 of M2 in HCC827 and PC9 cell lines was significantly lower,and Conc.brain/Conc.plasma of M2 in mice was significantly higher than those of gefitinib,suggesting its higher potential to penetrate blood–brain barrier and might be more effective in the treatment of brain metastatic tumor than gefitinib.Consistently and attractively,higher M2 plasma concentration was found to be correlated with better clinical outcome in patients with brain metastases(the median PFS of CM2<12 ng/mL and CM2≥12 ng/mL were 17.0 and 27.1 months,respectively,P=0.038).The plasma concentration of M2≥12 ng/mL was a strong predictor of the PFS of NSCLC patients.In conclusion,for NSCLC patients with EGFR sensitive mutations,the standard dose is suspectable and could be decreased reasonably.M2 plays an important role in efficacy and may be more effective in the treatment of metastatic tumor than gefitinib.Wei Feng Xi Chen Shao-xing Guan Hong-lian Ruan Yan Huang Hui-zhen Zhang Yun-peng Yang Wen-feng Fang Hong-yun Zhao Wei Zhuang Shuang Xin You-hao Chen Fei Wang Yue Gao Min Huang Xue-ding Wang Li Zhang 2022Acta Pharmacologica Sinica2022,43,7:1
