1Surgical approaches of resectable synchronous colorectal liver metastases:Timing considerations显示文摘AIM: To compare the safety and efficacy of simultaneous versus two stage resection of primary colorectal tumors and liver metastases. METHODS: From January 1996 to May 2004, 103 colorectal tumor patients presented with synchronous liver metastases. Twenty five underwent simultaneous colorectal and liver surgery and 78 underwent liver surgery 1-3 mo after primary colorectal tumor resection. Data were retrospectively analyzed to assess and compare the morbidity and mortality between the surgical strategies. The two groups were comparable regarding the age and sex distribution, the types of liver resection and stage of primary tumors, as well as the number and size of liver metastases. RESULTS: In two-stage procedures more transfusions were required (4 ± 1.5 vs 2 ± 1.8, pRBCs, P < 0.05). Chest infection was increased after the two-stage approach (26% vs 17%, P < 0.05). The two-stage procedure was also associated with longer hospitalization (20 ± 8 vs 12 ± 6 d, P < 0.05). Five year survival in both groups was similar (28% vs 31%). No hospital mortality occurred in our series. CONCLUSION: Synchronous colorectal liver metastases can be safely treated simultaneously with the primary tumor. Liver resection should be prioritized over colon resection. 2Managing injuries of hepatic duct confluence variants after major hepatobiliary surgery:An algorithmic approach显示文摘AIM:To investigate injuries of anatomy variants of hepatic duct confluence during hepatobiliary surgery and their impact on morbidity and mortality of these procedures. An algorithmic approach for the management of these injuries is proposed. METHODS:During a 6-year period 234 patients who had undergone major hepatobiliary surgery were retrospectively reviewed in order to study postoperative bile leakage. Diagnostic workup included endoscopic and magnetic retrograde cholangiopancreatography (E/MRCP), scintigraphy and fistulography. RESULTS:Thirty (12.8%) patients who developed postoperative bile leaks were identified. Endoscopic stenting and percutaneous drainage were successful in 23 patients with bile leaks from the liver cut surface. In the rest seven patients with injuries of hepatic duct confluence, biliary variations were recognized and a stepwise therapeutic approach was considered. Conservative management was successful only in 2 patients. Volume of the liver remnant and functional liver reserve as well as local sepsis were used as criteria for either resection of the corresponding liver segment or construction of a biliary-enteric anastomosis. Two deaths occurred in this group of patients with hepatic duct confluence variants (mortality rate 28.5%). CONCLUSION:Management of major biliary fistulaethat are disconnected from the mainstream of the biliary tree and related to injury of variants of the hepatic duct confluence is extremely challenging. 3Bone and high-density lipoprotein:The beginning of a beautiful friendship显示文摘There is a tight link between bone and lipid metabolic pathways.In this vein,several studies focused on the exploration of high-density lipoprotein(HDL)in the pathobiology of bone diseases,with emphasis to the osteoarthritis(OA)and osteoporosis,the most common bone pathologies.Indeed,epidemiological and in vitro data have connected reduced HDL levels or dysfunctional HDL with cartilage destruction and OA development.Recent studies uncovered functional links between HDL and OA fueling the interesting hypothesis that OA could be a chronic element of the metabolic syndrome.Other studies have linked HDL to bone mineral density.Even though at epidemiological levels the results are conflicting,studies in animals as well as in vitro experiments have shown that HDL facilitates osteoblastogensis and bone synthesis and most probably affects osteoclastogenesis and osteoclast bone resorption.Notably,reduced HDL levels result in increased bone marrow adiposity affecting bone cells function.Unveiling the mechanisms that connect HDL and bone/cartilage homeostasis may contribute to the design of novel therapeutic agents for the improvement of bone and cartilage quality and thus for the treatment of related pathological conditions.Dionysios J Papachristou Harry C Blair 2016World Journal of Orthopedics2016,7,2:3

5Elimination of antibiotic resistance genes and control of horizontal transfer risk by UV-based treatment of drinking water: A mini review显示文摘Antibiotic-resistant bacteria (ARB) and antibiotic resistance genes (ARGs) have been recognized as one of the biggest public health issues of the 21 st century. Both ARB and ARGs have been determined in water after treatment with conventional disinfectants. Ultraviolet (UV) technology has been seen growth in application to disinfect the water. However, UV method alone is not adequate to degrade ARGs in water. Researchers are investigating the combination of UV with other oxidants (chlorine, hydrogen peroxide (H2O2), peroxymonosulfate (PMS), and photocatalysts) to harness the high reactivity of produced reactive species (C1-, C1O -, Cl2-,-OH, and SO4-_) in such processes with constituents of cell (e.g., deoxyribonucleic acid (DNA) and its components) in order to increase the degradation efficiency of ARGs. This paper briefly reviews the current status of different UV-based treatments (UV/chlorination, UV/H2O2, UV/PMS, and UV-photocatalysis) to degrade ARGs and to control horizontal gene transfer (HGT) in water. 20Ag-decorated 3D flower-like Bi_(2)MoO_(6)/rGO with boosted photocatalytic performance for removal of organic pollutants显示文摘A series of unique 3D flower-like Bi_(2)MoO_(6)(BMO)/reduced graphene oxide(rGO)heterostructured composites decorated with varying amounts of Ag nanoparticles(NPs)were fabricated.Their morphological characteristics,structural features,energy band structures and photoelectrochemical properties were systematically studied.All the Ag/BMO/rGO ternary composites(AgBGy;y=1%,2%and 3%)demonstrated greater photocatalytic activity towards efficient removal of our selected organic models[methyl orange(MO),rhodamine B(RhB)and phenol],as compared with the BMO/rGO binary composites(BG-x;x=0.25,2,4 and 5).Particularly,AgBG-2%,which was synthesized with the addition of 2 wt% rGO and 2 wt%Ag in BMO,possessed superior photocatalytic activity.The fitted rate constants(k)for the photocatalytic degradation of RhB,MO and phenol using AgBG-2% were estimated to be 0.0286,0.0301 and 0.0165 min^(-1),respectively,which were over one order of magnitude greater than those obtained using pure BMO.Several factors may contribute to the observed enhancement,including greater specific surface area,enhanced light absorption,promoted spatial separation of electronhole(e^(-)-h^(+))pairs and their suppressed recombination,especially benefiting from the synergistic effects among BMO,rGO and Ag NPs.Our work suggests that the rational design of BMO/rGO/Ag ternary composite was an effective strategy to boost the photocatalytic activity of the resulting catalyst towards the highly efficient removal of organic pollutants from water.Xin Liu Wei-Ya Huang Qin Zhou Xi-Rong Chen Kai Yang Dan Li Dionysios D.Dionysiou 2021Rare Metals2021,40,5:1
