5Repurposing vitamin D for treatment of human malignancies via targeting tumor microenvironment显示文摘Tumor cells along with a small proportion of cancer stem cells exist in a stromal microenvironment consisting of vasculature, cancer-associated fibroblasts, immune cells and extracellular components.Recent epidemiological and clinical studies strongly support that vitamin D supplementation is associated with reduced cancer risk and favorable prognosis. Experimental results suggest that vitamin D not only suppresses cancer cells, but also regulates tumor microenvironment to facilitate tumor repression. In this review, we have outlined the current knowledge on epidemiological studies and clinical trials of vitamin D. Notably, wesummarized and discussed the anticancer action of vitamin D in cancer cells, cancer stem cells and stroma cells in tumor microenvironment, providing a better understanding of the role of vitamin D in cancer. 7Emission characteristics of dioxins,furans and polycyclic aromatic hydrocarbons during fluidized-bed combustion of sewage sludge显示文摘Pre-dried sewage sludge with high sulfur content was combusted in an electrically heated lab-scale fluidized-bed incinerator. The emission characteristics of polychlorinated dibenzo-p-dioxins (PCDDs), polychlorinated dibenzofurans (PCDFs) and polycyclic aromatic hydrocarbons (PAHs) were studied. Coal and calcium oxide (CaO) were added during the sewage sludge combustion tests to optimize combustion conditions and control SO2 emission. The results indicated that the flue gases emitted during mono-combustion of sewage sludge were characterized by relatively high concentrations of SO2, NOx and organic pollutants, due to the high sulfur, nitrogen, and volatile matter content of sewage sludge. The total 16 USEPA priority PAHs and 2,3,7,8-substituted PCDD/Fs produced from sewage sludge combustion were found to be 106.14 μg/m3 and 8955.93 pg/m3 in the flue gas, respectively. In the case of co- combustion with coal (msludge/mcoal = 1:1), the 16 PAHs and 2,3,7,8-substituted PCDD/Fs concentrations were markedly lower than those found during mono-combustion of sewage sludge. 8Epigastric pain syndrome: What can traditional Chinese medicine do? A randomized controlled trial of Biling Weitong Granules显示文摘BACKGROUND Recent research suggests that although prokinetic agents,acid suppressors,and radical treatment for Helicobacter pylori infection may be effective in patients with functional dyspepsia(FD),a large proportion of patients still fail to respond to these treatments or may suffer from severe adverse reactions.Many traditional Chinese medicinal herbs can regulate the status of the entire body and have special advantages in the treatment of functional diseases.The present study was designed to verify the efficacy of Biling Weitong Granules(BLWTG),a traditional Chinese medicinal herbal compound formula,in alleviating epigastric pain syndrome(EPS)in FD patients,in an attempt to provide an effective prescription for the clinical treatment of this disease.AIM To evaluate the clinical efficacy and safety of BLWTG in treating EPS in patients with FD.METHODS In this multicenter,stratified,randomized,double-blind,placebo-controlled,parallel group clinical trial,eligible patients were randomized into the BLWTG and placebo groups who were treated for 6 wk.Efficacy indicators including the severity and frequency of EPS and the time to pain resolution and safety indicators including adverse events were observed and compared.RESULTS The baseline demographic data and clinical characteristics,such as epigastric pain symptoms,pain intensity,and frequency of attacks,were matched between the two groups before randomization.After 6 wk of treatment and after the center effect was eliminated,the epigastric pain was significantly improved in 28.33%and 85.59%of the patients in the placebo and BLWTG groups,respectively(P<0.05).At 6 wk,the resolution rate of epigastric pain was 15%and 69.49%in the placebo and BLWTG groups,respectively(P<0.05).The differences of total FD clinical score between these two groups were significant(P<0.05)at 2,4,and 6 wk(P<0.05).The scores of each item and the total score in the Functional Digestive Disorders Quality of Life Questionnaire showed significant differences between the two groups at 6 wk after both the center and interaction effects were eliminated(P<0.05).There was no significant difference in the incidence of adverse events between the two groups,and no serious adverse event was noted during the observation.CONCLUSION Compared with placebo,BLWTG markedly improved EPS in FD patients without causing serious adverse reactions.Yan-Dong Wen Fang Lu Ying-Pan Zhao Ping Wang Qian Yang Jun-Xiang Li Hui-Zhen Li Li-Li Chi Zheng-Hua Zhou Yan-Ping Tang Jin-Kang Xu Yang Zhao Xu-Dong Tang 2020World Journal of Gastroenterology2020,26,28:16
9An EcoCity model for regulating urban land cover structure and thermal environment: Taking Beijing as an example显示文摘Urban land-use/cover changes and their effects on the eco-environment have long been an active research topic in the urbanization field. However, the characteristics of urban inner spatial heterogeneity and its quantitative relationship with thermal environment are still poorly understood, resulting in ineffective application in urban ecological planning and management.Through the integration of 'spatial structure theory' in urban geography and 'surface energy balance' in urban climatology, we proposed a new concept of urban surface structure and thermal environment regulation to reveal the mechanism between urban spatial structure and surface thermal environment. We developed the EcoCity model for regulating urban land cover structure and thermal environment, and established the eco-regulation thresholds of urban surface thermal environments. Based on the comprehensive analysis of experimental observation, remotely sensed and meteorological data, we examined the spatial patterns of urban habitation, industrial, infrastructure service, and ecological spaces. We examined the impacts of internal land-cover components(e.g., urban impervious surfaces, greenness, and water) on surface radiation and heat flux. This research indicated that difference of thermal environments among urban functional areas is closely related to the proportions of the land-cover components.The highly dense impervious surface areas in commercial and residential zones significantly increased land surface temperature through increasing sensible heat flux, while greenness and water decrease land surface temperature through increasing latent heat flux. We also found that different functional zones due to various proportions of green spaces have various heat dissipation roles and ecological thresholds. Urban greening projects in highly dense impervious surfaces areas such as commercial, transportation, and residential zones are especially effective in promoting latent heat dissipation efficiency of vegetation, leading to strongly cooling effect of unit vegetation coverage. This research indicates that the EcoCity model provides the fundamentals to understand the coupled mechanism between urban land use structure and surface flux and the analysis of their spatiotemporal characteristics. 12Microvascular changes after conbercept therapy in central retinal vein occlusion analyzed by optical coherence tomography angiography显示文摘AIM: To investigate microvascular changes in eyes with central retinal vein occlusion(CRVO) complicated by macular edema before and after intravitreal conbercept injection and evaluate correlations between these changes and best-corrected visual acuity(BCVA) and retinal thickness. METHODS: Twenty-eight eyes of 28 patients with macular edema caused by CRVO were included in this retrospective study. All patients received a single intravitreal conbercept injection to treat macular edema. BCVA and the results of optical coherence tomography angiography(OCTA) automatic measurements of the vessel density in the superficial(SCP) and deep retinal capillary plexus(DCP), the foveal avascular zone(FAZ) area, the FAZ perimeter(PERIM), the vessel density within a 300-μm wide ring surrounding the FAZ(FD-300), the acircularity index(AI), the choriocapillaris flow area, and retinal thickness were recorded before and at one month after treatment and compared with the results observed in age-and sexmatched healthy subjects. RESULTS: The vessel density in the SCP and DCP, the FD-300, and the flow area of the choriocapillaris were allsignificantly lower in CRVO eyes than in healthy eyes, while the AI and retinal thickness were significantly higher(all P<0.05). After treatment, retinal thickness was significantly decreased, and the mean BCVA had markedly improved from 20/167 to 20/65(P=0.0092). The flow area of the choriocapillaris was also significantly improved, which may result from the reduction of shadowing effect caused by the attenuation of macular edema. However, there were no significant changes in SCP and DCP vessel density after treatment. 13Remote sensing-based artificial surface cover classification in Asia and spatial pattern analysis显示文摘Artificial surfaces, characterized with intensive land-use changes and complex landscape structures, are important indicators of human impacts on terrestrial ecosystems. Without high-resolution land-cover data at continental scale, it is hard to evaluate the impacts of urbanization on regional climate, ecosystem processes and global environment. This study constructed a hierarchical classification system for artificial surfaces, promoted a remote sensing method to retrieve subpixel components of artificial surfaces from 30-m resolution satellite imageries(Globe Land30) and developed a series of data products of high-precision urban built-up areas including impervious surface and vegetation cover in Asia in 2010. Our assessment, based on multisource data and expert knowledge, showed that the overall accuracy of classification was 90.79%. The mean relative error for the impervious surface components of cities was 0.87. The local error of the extracted information was closely related to the heterogeneity of urban buildings and vegetation in different climate zones. According to our results, the urban built-up area was 18.18×104 km2, accounting for 0.59% of the total land surface areas in Asia; urban impervious surfaces were 11.65×104 km2, accounting for 64.09% of the total urban built-up area in Asia. Vegetation and bare soils accounted for 34.56% of the urban built-up areas. There were three gradients: a concentrated distribution, a scattered distribution and an indeterminate distribution from east to west in terms of spatial pattern of urban impervious surfaces. China, India and Japan ranked as the top three countries with the largest impervious surface areas, which respectively accounted for 32.77%, 16.10% and 11.93% of the urban impervious surface area of Asia. We found the proportions of impervious surface and vegetation cover within urban built-up areas were closely related to the economic development degree of the country and regional climate environment. Built-up areas in developed countries had relatively low impervious surface and high public green vegetation cover, with 50–60% urban impervious surfaces in Japan, South Korea and Singapore. In comparison, the proportion of urban impervious surfaces in developing countries is approaching or exceeding 80% in Asia. 14Review on fate of chlorine during thermal processing of solid wastes显示文摘Chlorine(Cl) is extensively present in solid wastes, causing significant problems during the thermal conversion of waste to energy or fuels, by combustion, gasification or pyrolysis.This paper introduces the analytical methods for determining the Cl content in solid materials and presents the concentrations of Cl in various types of wastes, as reported in literature. Then, it provides a comprehensive analysis on the Cl emission behavior and Cl species formed during the thermal processing of the inorganic and organic Cl sources. The challenges resulted from the reactions between the formed Cl species and the ferrous metals, the heavy metals and the organic matters are summarized and discussed, e.g., high temperature corrosion, heavy metal evaporation and dioxin formation. The quality degradation of products(oil, char and syngas) by Cl is analyzed. 15Chinese Herbal Medicine Combined with Entecavir for HBeAg Positive Chronic Hepatitis B:Study Protocol for a Multi-Center,Double-Blind Randomized-Controlled Trial显示文摘Background: The domestic prevalence of chronic hepatitis B(CHB) in China is 7.18% in 2006, imposing great societal healthcare burdens. Nucleot(s)ide analogues(NUCs) anti-hepatitis B virus(HBV) therapies are widely applied despite the relatively low rate of seroconversion and high risk of drug-resistant mutation. More effective treatments for CHB deserve further explorations. Combined therapy of NUCs plus Chinese herbal medicine(CHM) is widely accepted in China, which is recognized as a prospective alternative approach. The study was primarily designed to confirm the hypothesis that Tiaogan-Yipi Granule(调肝益脾颗粒, TGYP) or Tiaogan-Jianpi-Jiedu Granule(调肝健脾解毒颗粒, TGJPJD) plus entecavir tablet(ETV) was superior over ETV monotherapy in enhancing HBeA g loss rate. Methods: The study was a nationwide, large-scale, multi-center, double-blind, randomized, placebo-control ed trial with a designed duration of 108 weeks. A total of 16 hospitals and 596 eligible Chinese HBeA g positive CHB patients were enrolled from November 2012 to September 2013 and randomly allocated into 2 groups in 1:1 ratio via central randomization system: experimental group(EG) and control group(CG). Subjects in EG received CM formulae(TGYP or TGJPJD, 50 g per dose, twice daily) plus ETV tablet(or ETV placebo) 0.5 mg per day in the first 24 weeks(stage 1), and CHM granule plus ETV tablet(0.5 mg per day) from week 25 to 108(stage 2). Subjects in CG received CHM Granule placebo plus ETV tablet(0.5 mg per day) for 108 weeks throughout the trial. The assessments of primary outcomes(HBV serum markers and HBV-DNA) were conducted by a third-party College of American Pathologists(CAP) qualified laboratory. Adverse effects were observed in the hospitals of recruitment. Discussion: The study was designed to compare the curative effect of CM plus ETV and ETV monotherapy in respect of HBe Ag loss, which is recognized by the European Association for the Study of the Liver as 'a valuable endpoint'. We believe this trial could provide a reliable status for patients' 'journey' towards durable responses after treatment discontinuation. 16ATF3 contributes to brucine-triggered glioma cell ferroptosis via promotion of hydrogen peroxide and iron显示文摘Ferroptotic cell death is characterized by iron-dependent lipid peroxidation that is initiated by ferrous iron and H_(2)O_(2) via Fenton reaction,in which the role of activating transcription factor 3(ATF3)remains elusive.Brucine is a weak alkaline indole alkaloid extracted from the seeds of Strychnos nux-vomica,which has shown potent antitumor activity against various tumors,including glioma.In this study,we showed that brucine inhibited glioma cell growth in vitro and in vivo,which was paralleled by nuclear translocation of ATF3,lipid peroxidation,and increases of iron and H_(2)O_(2).Furthermore,brucine-induced lipid peroxidation was inhibited or exacerbated when intracellular iron was chelated by deferoxamine(500μM)or improved by ferric ammonium citrate(500μM).Suppression of lipid peroxidation with lipophilic antioxidants ferrostatin-1(50μM)or liproxstatin-1(30μM)rescued brucine-induced glioma cell death.Moreover,knockdown of ATF3 prevented brucine-induced accumulation of iron and H_(2)O_(2) and glioma cell death.We revealed that brucine induced ATF3 upregulation and translocation into nuclei via activation of ER stress.ATF3 promoted brucine-induced H_(2)O_(2) accumulation via upregulating NOX4 and SOD1 to generate H_(2)O_(2) on one hand,and downregulating catalase and xCT to prevent H_(2)O_(2) degradation on the other hand.H_(2)O_(2) then contributed to brucine-triggered iron increase and transferrin receptor upregulation,as well as lipid peroxidation.This was further verified by treating glioma cells with exogenous H_(2)O_(2) alone.Moreover,H_(2)O_(2) reversely exacerbated brucine-induced ER stress.Taken together,ATF3 contributes to brucine-induced glioma cell ferroptosis via increasing H_(2)O_(2) and iron.Shan Lu Xuan-zhong Wang Chuan He Lei Wang Shi-peng Liang Chong-cheng Wang Chen Li Tian-fei Luo Chun-sheng Feng Zhen-chuan Wang Guang-fan Chi Peng-fei Ge 2021Acta Pharmacologica Sinica2021,42,10:10

18Adefovir Dipivoxil plus Chinese Medicine in HBeAg-Positive Chronic Hepatitis B Patients:A Randomized Controlled 48-Week Trial显示文摘Objective:To evaluate the effects of a 48-week course of adefovir dipivoxil(ADV)plus Chinese medicine(CM)therapy,namely Tiaogan Jianpi Hexue(调肝健脾和血)and Tiaogan Jiedu Huashi(调肝解毒化湿)fomulae,in hepatitis B e antigen(HBeAg)-positive Chinese patients.Methods:A total of 605 HBeAg-positive Chinese CHB patients were screened and 590 eligible participants were randomly assigned to 2 groups in 1:1 ratio including experimental group(EG,received ADV plus CM)and control group(CG,received ADV plus CM-placebo)for 48 weeks.The major study outcomes were the rates of HBeAg and HBV-DNA loss on week 12,24,36,48,respectively.Secondary endpoints including liver functions(enzymes and bilirubin readings)were evaluated every 4 weeks at the beginning of week 24,36,and 48.Routine blood,urine,and stool analyses in addition to electrocardiogram and abdominal B scan were monitored as safety evaluations.Adverse events(AEs)were documented.Results:The combination therapy demonstrated superior HBeAg loss at 48 weeks,without additional AEs.The full analysis population was 560 and 280 in each group.In the EG,population achieved HBeAg loss on week 12,24,36,and 48 were 25(8.90%),34(12.14%),52(18.57%),and 83(29.64%),respectively;the equivalent numbers in the CG were 20(7.14%),41(14.64%),54(19.29%),and 50(17.86%),respectively.There was a statistically significant difference between two groups on week 48(P<0.01).No additional AEs were found in EG.Subgroup analysis suggested different outcomes among treatment patterns.Conclusion:Combination of CM and ADV therapy demonstrated superior HBeAg clearance compared with ADV monotherapy.The finding indicates that this combination therapy may provide an improved therapeutic effect and safety profile(ChiCTR-TRC-11001263).LI Xiao-ke ZHANG Ming-xiang SHAO Feng-zhen ZHOU Da-qiao XUE Jing-dong LIU Tie-jun CHI Xiao-ling LU Bing-jiu WANG Xian-bo LI Qin LI Jun MAO De-wen YANG Hua-sheng YANG Hong-zhi ZHAO Wen-xia LI Yong ZHANG Guo-liang ZHAO Yi-ming ZOU Jian-dong LIU Meng-yang ZHANG Ke-ke YANG Xian-zhao GAN Da-nan LI Ying ZHANG Peng LI Zhi-guo LI Shuo YE Yong-an 2020Chinese Journal of Integrative Medicine2020,26,5:9
