1Management of hilar cholangiocarcinoma in the North of England: Pathology, treatment, and outcome显示文摘AIM: To assess the management and outcome of hilar cholangiocarcinoma (Klatskin tumor) in a single tertiary referral center.METHODS: The notes of all patients with a diagnosis of hilar cholangiocarcinoma referred to our unit for over an 8-year period were identified and retrospectively reviewed. Presentation, management and outcome were assessed.RESULTS: Seventy-five patients were identified. The median age was 64 years (range 34-84 years). Male to female ratio was 1:1. Eighty-nine percent of patients presented with jaundice. Most patients referred were under Bismuth classification 3a, 3b or 4. Seventy patients required biliary drainage, 65 patients required 152percutaneous drainage procedures, and 25 had other complications. Forty-one patients had 51 endoscopic drainage procedures performed (15 failed). Of these,36 subsequently required percutaneous drainage. The median number of drainage procedures for all patients was three, 18 patients underwent resection (24%), nine had major complications and three died post-operatively.The 5-year survival rate was 4.2% for all patients, 21%for resected patients and 0% for those who did not undergo resection (P = 0.0021). The median number of admissions after diagnosis in resected patients was two and three in non-resected patients (P<0.05).Twelve patients had external-beam radiotherapy, seven brachytherapy, and eight chemotherapy. 3城乡教育一体化背景下乡村小规模学校布局调整与优化建议——基于陕西省宁强县的案例分析显示文摘我国义务教育正在由优质均衡向城乡一体化发展迈进,作为乡村地区常态化办学形式的小规模学校,其空间布局合理化直接决定着广大乡村儿童能否就近入学、公平享有优质教育的机会。研究基于陕西省宁强县的微观数据,采用GIS技术方法,从地理位置、服务范围、家校距离、上学可达性等维度动态分析小规模学校布局变化和存在的问题:城乡教育一体化发展中公平与效率冲突,布局过于分散,撤并后儿童上学距离加大且承担较高成本,保留的小规模学校与县镇学校办学资源差距依然较大且合作网络缺失。由此,各级教育行政部门应在城乡义务教育布局统筹中坚持公平优先、效率兼顾、质量导向的价值取向, 采用科学的规划程序和调整模式,合理撤并和保留小规模学校, 加强政策支持,补偿偏远地区儿童求学成本, 构建校际合作网络,促进小规模学校共享优质资源。赵丹 郭清扬 Bilal Barakat 2021中国教育学刊2021,,5:10
4Hepatocellular carcinoma in Egypt: A single center study over a decade显示文摘AIM: To identify the trend, possible risk factors and any pattern change of hepatocellular carcinoma (HCC) in Egyptover a decade.METHODS: All HCC patients attending Cairo Liver Center between January 1993 and December 2002, were enrolledin the study. Diagnosis of HCC was based on histopath ological examination and/or detection of hepatic focal lesions by two imaging techniques plus α-fetoprotein level above 200 ng/mL. The duration of the study was divided into two periods of 5 years each; period Ⅰ (1993-1997) and period Ⅱ (1998-2002). Trend, demographic features of patients (age, gender, and residence), risk factors (HBsAg,HCV-Ab, schistosomiasis and others) and pattern of the focal lesions were compared between the two periods.Logistic regression model was fitted to calculate the adjusted odds ratios for the potential risk factors. The population attributable risk percentage was calculated to estimate the proportion of HCC attributed to hepatitis B and C viral infections.RESULTS: Over a decade, 1 328 HCC patients out of 22 450 chronic liver disease (CLD) patients were diagnosed with an overall proportion of 5.9%. The annual proportion of HCC showed a significant rising trend from 4.0% in 1993 to 7.2% in 2002 (P = 0.000). A significant increase in male proportion from 82.5% to 87.6% (P = 0.009); M/F from 5:1 to 7:1 and a slight increase of the predominant age group (40-59 years) from 62.6% to 66.8% (P = 0.387)in periods Ⅰ and Ⅱ respectively, reflecting a shift to younger age group. In the bivariate analysis, HCC was significantly higher in rural residents, patients with history of schistosomiasis and/or blood transfusion. Yet, after adjustment, these variables did not have a significant risk for development of HCC. There was a significant decline of HBsAg from 38.6% to 20.5% (P = 0.000), and a slight increase of HCV-Ab from 85.6% to 87.9% in periods Ⅰ and Ⅱ respectively. HBV conferred a higher risk to develop HCC more than HCV in period Ⅰ (OR 1.9 vs 1.6) and period Ⅱ (OR 2.7 vs 2.0), but the relative contribution of HBV for development of HCC declined in period Ⅱ compared to period Ⅰ (PAR% 4.2%, 21.32%). At presentation,diagnostic α-fetoprotein level (≥200 ng/mL) was demonstrated in 15.6% vs28.9% and small HCC (≤3 cm)represented 14.9% vs 22.7% (P = 0.0002) in periods Ⅰ and Ⅱ respectively.CONCLUSION: Over a decade, there was nearly a twofold increase of the proportion of HCC among CLD patients in Egypt with a significant decline of HBV and slight increase of HCV as risk factors. α-Fetoprotein played a limited role in diagnosis of HCC, compared to imaging techniques.Increased detection of small lesions at presentation reflects increased awareness of the condition.Adbel-Rahman El-Zayadi Hanaa M Badran Eman MF Barakat Mohy El-Deen Attia Sherine Shawky Mostafa K Mohamed Osaima Selim Ahmed Saeid 2005World Journal of Gastroenterology2005,11,33:5
5Recent advances in non-metal doped titania for solar-driven photocatalytic/photoelectrochemical water-splitting显示文摘Photocatalytic (PC) / Photoelectrochemical (PEC) water splitting under solar light irradiation is considered as a prospective technique to support the sustainable and renewable H_(2) economy and to reach the ultime goal of carbon neutral. TiO_(2) based photocatalysts with high chemical stability and excellent photocatalytic properties have great potential for solar-to-H_(2) conversion. To conquer the challenges of the large band-gap and rapid recombination of photo generated electron-holepairs in TiO_(2), non-metal doping turns out to be economic, facile, and effective on boosting the visible light activity. The localized defect states such as oxygen vacancy and Ti^(3+) generated by non-metal doping are located in the band-gap of TiO_(2), which result in the reduction of band-gap, thus a red-shift of the absorption edge. The hetero doping atoms such as B^(3+), I^(7+), S^(4+)/S^(6+), P^(5+) can also act as electron donors or trap sites which facilitate the charge carrier separation and suppress the recombination of electron-hole pairs. In this comprehensive review, we present the most recent advances on non-metal doped TiO_(2) photocatalysts in terms of fundamental aspects, origin of visible light activity and the PC / PEC behaviours for water splitting. In particular, the characteristics of different non-metal elements (N, C, B, S, P, Halogens) as dopants are discussed in details focusing on the synthesis approaches, characterization as well as the efficiency of PC and PEC water splitting. 8Phenolic compounds and hepatoprotective potential of Anastatica hierochuntica ethanolic and aqueous extracts against CCl4-induced hepatotoxicity in rats显示文摘OBJECTIVE:To investigate the effect of Anastatica hierochuntica ethanolic(KEE),aqueous(KAE)extracts,and their combination against CCl4-induced hepatotoxicity in rats.METHODS:The HPLC analysis for KEE and KAE was quantitatively carried out.Biochemical liver markers,antioxidant enzymes,and histopathological alterations were examined then total hepatoprotection potential was calculated.RESULTS:Among 9 identified phenolic compounds(PC)in KEA,sinapic acid was the highest while syringic acid was the highest among 21 identified PC in KAE.Six flavonoids were identified in KEE and two in KAE using HPLC,respectively.Oral administration of KEE,KAE,and KEE+KAE at 250 mg/kg body weight significantly reduced aspartate aminotransferase(AST),alanine aminotransferase(ALT)levels,alkaline phosphatase(ALP),total bilirubin(TBILI),and also attenuated histopathological changes.Additionally,they reduced malondialdehyde(MOD),restored reduced-glutathione(GSH),and enhanced superoxide dismutase(SOD)levels.KEE,KAE,and KEE+KAE protected the liver from CCl4-hepatotoxicity as they mainly attenuating oxidative stress.Total hepatoprotection was about 128.3%,114.5%,and 103.8%for KEE,KAE,and KEE+KAE,respectively.CONCLUSION:Biochemical observations,supplemented by histopathological examination revealed that AH affords extract-depending protection against CCl4-hepatotoxicity.Hassan Barakat Tariq I.Almundarij 2020Journal of Traditional Chinese Medicine2020,40,6:3
9Unhappy triad in limb reconstruction:Management by Ilizarov method显示文摘AIM To evaluate the results of the Ilizarov method in management of cases with bone loss, soft tissue loss and infection.METHODS Twenty eight patients with severe leg trauma complicated by bone loss, soft tissue loss and infection were managed by distraction osteogenesis in our institution. After radical debridement of all the infected and dead tissues the Ilizarov frame was applied, corticotomy was done and bone transport started. The wounds were left open to drain. Partial limb shortening was done in seven cases to reduce the size of both the skeletal and soft tissue defects. The average follow up period was 39 mo(range 27-56 mo). RESULTS The infection was eradicated in all cases. All the soft tissue defects healed during bone transport and plastic surgery was only required in 2 cases. Skeletal defects were treated in all cases. All patients required another surgery at the docking site to fashion the soft tissue and to cover the bone ends. The external fixation time ranged from 9 to 17 mo with an average of 13 mo. The complications included pin tract infection in 16 cases, wire breakage in 2 cases, unstable scar in 4 cases and chronic edema in 3 cases. According to the association for study and application of methods of Ilizarov score the bone results were excellent in 10, good in 16 and fair in 2 cases while the functional results were excellent in 8, good in 17 and fair in 3 cases. 10Manipulating Bloch surface waves in 2D: a platform concept-based flat lens显示文摘At the end of the 1970s,it was confirmed that dielectric multilayers can sustain Bloch surface waves(BSWs).However,BSWs were not widely studied until more recently.Taking advantage of their high-quality factor,sensing applications have focused on BSWs.Thus far,no work has been performed to manipulate and control the natural surface propagations in terms of defined functions with two-dimensional(2D)components,targeting the realization of a 2D system.In this study,we demonstrate that 2D photonic components can be implemented by coating an in-plane shaped ultrathin(l/15)polymer layer on the dielectric multilayer.The presence of the polymer modifies the local effective refractive index,enabling direct manipulation of the BSW.By locally shaping the geometries of the 2D components,the BSW can be deflected,diffracted,focused and coupled with 2D freedom.Enabling BSW manipulation in 2D,the dielectric multilayer can play a new role as a robust platform for 2D optics,which can pave the way for integration in photonic chips.Multiheterodyne near-field measurements are used to study light propagation through micro-and nano-optical components.We demonstrate that a lens-shaped polymer layer can be considered as a 2D component based on the agreement between near-field measurements and theoretical calculations.Both the focal shift and the resolution of a 2D BSW lens are measured for the first time.The proposed platform enables the design of 2D all-optical integrated systems,which have numerous potential applications,including molecular sensing and photonic circuits.Libo Yu Elsie Barakat Tristan Sfez Lubos Hvozdara Joab Di Francesco Hans Peter Herzig 2014Light(Science & Applications)2014,3,1:3
11Outcome of non surgical hepatic decompression procedures in Egyptian patients with Budd-Chiari显示文摘AIM: To evaluate outcome of patients with Budd-Chiari syndrome after balloon angioplasty ± stenting or transjugular intrahepatic portosystemic shunt (TIPS). METHODS: Twenty five patients with Budd-Chiari syndrome admitted to Ain Shams University Hospitals, Tropical Medicine Department were included. Twelve patients (48%) with short segment occlusion were candidates for angioplasty; with stenting in ten cases and without stenting in two. Thirteen patients (52%) had Transjugular Intrahepatic Portosystemic Shunt. Patients were followed up for 12-32 mo. RESULTS: Patency rate in patients who underwent angioplasty ± stenting was 83.3% at one year and at end of follow up. The need of revision was 41.6% with one year survival of 100%, dropped to 91.6% at end of follow up. In patients who had Transjugular Intrahepatic Portosystemic Shunt, patency rate was 92.3% at one year, dropped to 84.6% at end of follow up. The need of revision was 38.4% with one year and end of follow up survival of 100%. Patients with patent shunts showed marked improvement compared to those with occluded shunts. CONCLUSION: Morbidity and mortality following angioplasty ± stenting and TIPS are low with satisfactory outcome. 12Assessment of antifungal activity of herbal and conventional toothpastes against clinical isolates of Candida albicans显示文摘Objective:To detect the anticandidal activity of nine toothpastes containing sodium fluoride, sodium monofluorophosphate and herbal extracts as an active ingredients against 45 oral and non oral Candida albicans(C.albicans) isolates.Methods:The antifungal activity of these toothpaste formulations was determined using a standard agar well diffusion method.Statistical analysis was performed using a statistical package,SPSS windows version 15,by applying mean values using one-way ANOVA with post-hoc least square differences(LSD) method.A P value of less than 0.05 was considered significant.Results:All toothpastes studied in our experiments were effective in inhibiting the growth of all C.albicans isolates.The highest anticandidal activity was obtained from toothpaste that containing both herbal extracts and sodium fluoride as active ingredients, while the lowest activity was obtained from toothpaste containing sodium monofluorophosphate as an active ingredient.Antifungal activity of Parodontax toothpaste showed a significant difference(P<0.001) against C.albicans isolates compared to toothpastes containing sodium fluoride or herbal products.Conclusions:In the present study,it has been demonstrated that toothpaste containing both herbal extracts and sodium fluoride as active ingredients are more effective in control of C.albicans,while toothpaste that containing monofluorophosphate as an active ingredient is less effective against C.albicans.Some herbal toothpaste formulations studied in our experiments,appear to be equally effective as the fluoride dental formulations and it can be used as an alternative to conventional formulations for individuals who have an interest in naturally-based products.Our results may provide invaluable information for dental professionals.Ghaleb Adwan Yousef Salameh Kamel Adwan Ali Barakat 2012Asian Pacific Journal of Tropical Biomedicine2012,2,5:3
13Determination of Polarity Terms of Some Nitrogen-Containing Surfactants and Simulated Hydrophobic Tail Models by Gas Chromatography显示文摘Some polarity terms of two groups of nitrogen-containing surfactants (six alkanolamides and nine polyoxyethylenated long chain amines) are measured through gas chromatography. The apparent methanol carbon number (CMeOH) and polarity index (IP) values are determined on the investigated surfactants as stationary phases in packed columns. Similarly, CMeOH and IP values are determined on simulated hydrophobic tail (SHT) models. The obtained results reveal that the introduction of SHT approach permits the distinction between the polarities of different surfactants and their head groups. The measured polarity terms are discussed as related to hydrophile-lipophile balance (HLB) number and the hydrophobic group carbon number (RCN). 14Hydrophilic bi-functional B-doped g-C_(3)N_(4) hierarchical architecture for excellent photocatalytic H_(2)O_(2) production and photoelectrochemical water splitting显示文摘Graphitic carbon nitride(g-C_(3)N_(4))has attracted great interest in photocatalysis and photoelectrocatalysis.However,their poor hydrophilicity poses a great challenge for their applications in aqueous environment.Here,we demonstrate synthesis of a hydrophilic bi-functional hierarchical architecture by the assembly of B-doped g-C_(3)N_(4)nanoplatelets.Such hierarchical B-doped g-C_(3)N_(4)material enables full utilization of their highly enhanced visible light absorption and photogenerated carrier separation in aqueous medium,leading to an excellent photocatalytic H_(2)O_(2)production rate of 4240.3μM g^(-1)h^(-1),2.84,2.64 and 2.13 times higher than that of the bulk g-C_(3)N_(4),g-C_(3)N_(4)nanoplatelets and bulk B doped g-C_(3)N_(4),respectively.Photoanodes based on these hierarchical architectures can generate an unprecedented photocurrent density of 1.72 m A cm^(-2)at 1.23 V under AM 1.5 G illumination for photoelectrochemical water splitting.This work makes a fundamental improvement towards large-scale exploitation of highly active,hydrophilic and stable metal-free g-C_(3)N_(4)photocatalysts for various practical applications.Yang Ding Soumyajit Maitra Chunhua Wang Runtian Zheng Meiyu Zhang Tarek Barakat Subhasis Roy Jing Liu Yu Li Tawfique Hasan Bao-Lian Su 2022Journal of Energy Chemistry2022,31,7:3

19Preserving apple(Malus domestica var.Anna)fruit bioactive substances using olive wastes extract-chitosan film coating显示文摘Apple fruits have components of therapeutic nature.Such components,to a great extent,decline or decompose during post-harvest that negatively affect fruit shelf-life.Chitosan fruit-filming has proved useful in maintaining these compounds.This study aims,therefore,at enhancing Chitosan coating-film performance by mixing with some olive wastes extracts of leaf and pomace extracts.Apple fruits were sprayed with six different coating formulas including chitosan-water wax coating,in addition to the uncoated fruits.Then,the total phenolic,flavonoids,antioxidants,pigments,weight loss,decay area,and microstructure were assayed.The bioactive substances drastically changed in uncoated rather than coated fruits.Conversely,weight loss and decay area significantly increased in uncoated fruits.Amazingly,the addition of olive leaf extract to chitosan coating films meaningfully reduced the gradual decline in total phenolic,flavonoids and antioxidants.Olive pomace extract recorded the lowest influencing on anthocyanins during storage at 4±1C for 35 d.Also,both olive leaf and pomace extracts enhanced the coating distribution,due to no pores were observed in the fruits’surfaces.Decidedly,incorporation of olive leaf extracts with 2%into chitosan coating solution was the best formula comparable with the others.Thus,olive wastes extracts,incorporated into chitosan fruit coatings;relatively improve the nutritional quality of apple fruits during post-harvest.Ibrahim Khalifa Hassan Barakat Hamdy A.El-Mansy Soliman A.Soliman 2017Information Processing in Agriculture2017,4,1:2
