1Therapeutic and prophylactic thalidomide in TNBS-induced colitis: Synergistic effects on TNF-α, IL-12 and VEGF production显示文摘AIM: To evaluated the therapeutic and prophylactic effect of thalidomide on 2, 4, 6-trinitrobenzene sulfonic acid (TNBS)-induced colitis. Thalidomide has been reported to downregulate the expression of tumor necrosis factor a (TNF-α), IL-12, and vascular endothelial growth factor (VEGF), hallmarks of intestinal inflammation in Crohn's disease (CD). METHODS: Male Wistar rats were divided in five groups of ten animals each. Four groups received a rectal infusion of TNBS in ethanol. The first group was sacrificed 7 d after colitis induction. The second and third groups received either thalidomide or placebo by gavage and were sacrificed at 14 d. The fourth group received thalidomide 6 h before TNBS administration, and was sacrificed 7 d after induction. The fifth group acted as the control group and colitis was not induced. Histological inflammatory scores of the colon were performed and lamina propria CD4+ T cells, macrophages, and VEGF+ cells were detected by immunohistochemistry. TNF-a and IL-12 were quantified in the supernatant of organ cultures by ELISA. RESULTS: Significant reduction in the inflammatory score and in the percentage of VEGF+ cells was observed in the group treated with thalidomide compared with animals not treated with thalidomide. Both TNF-αand IL-12 levels were significantly reduced among TNBS induced colitis animals treated with thalidomide compared with animals that did not receive thalidomide. TNF-αlevels were also significantly reduced among the animals receiving thalidomide prophylaxis compared with untreated animals with TNBS-induced colitis. Intestinal levels of TNF-αand IL-12 were significantly correlated with the inflammatory score and the number of VEGF+ cells. CONCLUSION: Thalidomide significantly attenuates TNBS-induced colitis by inhibiting the intestinal production of TNF-α, IL-12, and VEGF. 2Thiopurine-methyltransferase variants in inflammatory bowel disease:Prevalence and toxicity in Brazilian patients显示文摘AIM:To analyze the prevalence of thiopurine-methyltransferase(TPMT)genotypes and their associationwith drug toxicity in inflammatory bowel disease(IBD)patients from southeastern Brazil.METHODS:A total of 219 consecutive patients with IBD,of which 146 had Crohn’s disease and 73 had ulcerative colitis,regularly seen at the outpatient unit of the Division of Gastroenterology at the University Hospital Pedro Ernesto of the State University of Rio de Janeiro,a tertiary referral center,were enrolled in this study from February 2009 to January 2011.We analyzed the presence of major TPMT genetic variants(TPMT*2,*3A,*3C)in IBD patients by means of a specific allele and RFLP-PCR.Genomic DNA was isolated from peripheral blood leukocytes by proteinase-K/Sodium Dodecyl Sulfate digestion and phenol-chloroform extraction.TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes were detected by real-time polymerase chain reaction followed by direct sequencing with specific primers.Clinical data were systematically recorded,and correlated with the genotype results.RESULTS:The distribution of the selected TPMT gene polymorphism TPMT*2(C238G),TPMT*3A(G460A/A719G),and TPMT*3C(A719G)genotypes was 3.6%,5.4%,and 7.7%of the patients,respectively.Among the side effects recorded from patients taking azathioprine,14 patients presented with pancreatitis and/or an elevation of pancreatic enzymes,while 6 patients had liver toxicity,and 2 patients exhibited myelosuppression/neutropenia.TPMT polymorphisms were detected in 37/219 patients(8 heterozygous for*2,11 heterozygous for*3A,and 18 heterozygous for*3C).No homozygotic polymorphisms were found.Despite the prevalence of the TPMT*3C genotype,no differences among the genotype frequencies were significant.Although no association was detected regarding myelotoxicity or hepatotoxicity,a trend towards the elevation of pancreatic enzymes was observed for TPMT*2 and TPMT*3C genotypes.CONCLUSION:The prevalence of TPMT genotypes was high among Brazilian patients.Variants genes*2and*3C may be associated with azathioprine pancreatic toxicity in a IBD southeastern Brazilian population.Ana Teresa P Carvalho Barbara C Esberard Renata S B Fróes Davy C M Rapozo Ana B Grinman Tatiana A Simo Juliana C V C Santos Antonio José V Carneiro Luis Felipe Ribeiro-Pinto Heitor S P de Souza 2014World Journal of Gastroenterology2014,20,12:3

5An approach to measure infill matric suction of irregular infilled rock joints under constant normal stiffness shearing显示文摘Rock joints infilled with sediments can strongly influence the strength of rock mass. As infilled joints often exist under unsaturated condition, this study investigated the influence of matric suction of infill on the overall joint shear strength. A novel technique that allows direct measurement of matric suction of infill using high capacity tensiometers(HCTs) during direct shear of infilled joints under constant normal stiffness(CNS) is described. The CNS apparatus was modified to accommodate the HCT and the procedure is explained in detail. Joint specimens were simulated by gypsum plaster using threedimensional(3D) printed surface moulds, and filled with kaolin and sand mixture prepared at different water contents. Shear behaviours of both planar infilled joints and rough joints having joint roughness coefficients(JRCs) of 8-10 and 18-20 with the ratios of infill thickness to asperity height(t/a)equal to 0.5 were investigated. Matric suction shows predominantly unimodal behaviour during shearing of both planar and rough joints, which is closely associated with the variation of unloading rate and volumetric changes of the infill material. As expected, two-peak behaviour was observed for the rough joints and both peaks increased with the increase of infill matric suction. 8Impact of COVID-19 pandemic on the ocular surface显示文摘BACKGROUND There have been increased reports of dry eyes in the coronavirus disease 2019(COVID-19) pandemic era.AIM To analyze the differences in tear film properties from pre-and post-pandemic of the COVID-19 era.METHODS It was a retrospective comparative study.Patients were divided into three groups according to the data of multimodal ocular surface evaluation:(1) Group 1 if it was before Portugal lockdown decision(from August 2019 to March 2020);(2) Group 2 if it was after Portugal lockdown decision but without mask mandate(from April 2020 to October 2020);and(3) Group 3 if it was after Portugal lockdown but with mask mandate in health public highway(from November 2020 to April 2021).The following variables were analyzed:Lipid layer thickness,blink rate,Schirmer test,tear meniscus height,tear osmolarity,non-invasive break-up time,and loss area of the meibomian glands.RESULTS The study included 548 eyes of 274 patients,aged 18 years to 89 years,with a mean age of 66.15 ± 13.40 years at the time of multimodal ocular surface evaluation.Compared to group 1:(1) Mean lipid layer thickness was better in group 2(P = 0.001) and group 3(P < 0.001);(2) Schirmer test was similar in group 2(P = 0.576) and better in group 3(P = 0.002);(3) Tear osmolarity and loss area of the meibomian glands were worse in group 2(P = 0.031 and P < 0.001,respectively) and in group 3(both with P < 0.001);(4) Blink rate and tear meniscus height were similar in group 2(P = 0.821 and P = 0.370,respectively) and worse in group 3(P < 0.001 and P = 0.038,respectively);and(5) Non-invasive break-up time was worse in group 2(P = 0.030) and similar in group 3(P = 0.263).CONCLUSION Our study demonstrated that differences existed in tear film properties comparing data from the pre-and post-pandemic of the COVID-19 era.Ana Marta Joao Heitor Marques Daniel Almeida Diana José Paulo Sousa Irene Barbosa 2022World Journal of Clinical Cases2022,10,27:0
