| 1 | Management of negative pressure wound therapy in the treatment of diabetic foot ulcers显示文摘Diabetic foot(DF) is a common complication of diabetes and the first cause of hospital admission in diabetic patients. In recent years several guidelines have been proposed to reinforce the the management of DF with a notable increase in diabetes knowledge and an overall reduction of amputations. Significant improvements have been reached in the treatment of diabetic foot ulcers(DFUs) and nowadays clinicians have several advanced medications to apply for the best local therapy. Among these, negative pressure wound therapy(NPWT) is a useful adjunct in the management of chronic and complex wounds to promote healing and wound bed preparation for surgical procedures such as skin grafts and flap surgery. NPWT has shown remarkable results although its mechanisms of action are not completely understood. In this paper, we offer a complete overview of this medication and its implication in the clinical setting. We have examined literature related to NPWT concerning human, animal and in vitro studies, and we have summarized why, when and how we can use NPWT to treat DFUs. | 2 | Honey:its medicinal property and antibacterial activity显示文摘Indeed,medicinal importance of honey has been documented in the world's oldest medical literatures,and since the ancient times,it has been known to possess antimicrobial property as well as wound-healing activity.The healing property of honey is due to the fact that it offers antibacterial activity,maintains a moist wound condition,and its high viscosity helps to provide a protective barrier to prevent infection.Its immunomodulatory property is relevant to wound repair too.The antimicrobial activity in most honeys is due to the enzymatic production of hydrogen peroxide.However,another kind of honey,called non-peroxide honey(viz.,manuka honey),displays significant antibacterial effects even when the hydrogen peroxide activity is blocked.Its mechanism may be related to the low pH level of honey and its high sugar content(high osmolality) that is enough to hinder the growth of microbes.The medical grade honeys have potent in vitro bactericidal activity against antibiotic-resistant bacteria causing several life-threatening infections to humans. | 4 | Sodium humate accelerates cutaneous wound healing by activating TGF-β/Smads signaling pathway in rats显示文摘Sodium humate(HA-Na) has been topically used as a wound healing and anti-inflammatory agent in folk medicine. In the present study, HA-Na was investigated for cutaneous wound healing in Sprague–Dawley rats. HA-Na solution(1.0%, w/v) was topically administered to rats undergoing excision wound models. Healing was assessed with a recombinant bovine basic fibroblast growth factor for external use as positive control. Wound healing rates were calculated on Day 3, 6, 9, 14 and 21 after injury, and tissues were also harvested after the same intervals for histological analysis. In addition, tissue hydroxyproline levels were measured. Furthermore, m RNA levels and protein expressions of transforming growth factor-β1, 2, 3(TGF-β1, 2, 3) were determined by RT-PCR and western blot. Protein expression levels of Smad-2,-3,-4 and-7 were also detected by western blot. Our study demonstrates that HA-Na has the capacity to promote wound healing in rats via accelerated wound contraction and increased hydroxyproline content. More importantly, these wound healing effects of HA-Na might be mediated through the TGF-β/Smad signaling pathway. | 5 | Recent advances on the association of apoptosis in chronic non healing diabetic wound显示文摘Generally, wounds are of two categories, such as chronic and acute. Chronic wounds takes time to heal when compared to the acute wounds. Chronic wounds include vasculitis, non healing ulcer, pyoderma gangrenosum, and diseases that cause ischemia. Chronic wounds are rapidly increasing among the elderly population with dysfunctional valves in their lower extremity deep veins, ulcer, neuropathic foot and pressure ulcers. The process of the healing of wounds has several steps with the involvement of immune cells and several other cell types. There are many evidences supporting the hypothesis that apoptosis of immune cells is involved in the wound healing process by ending inflammatory condition. It is also involved in the resolution of various phases of tissue repair. During final steps of wound healing most of the endothelial cells, macrophagesand myofibroblasts undergo apoptosis or exit from the wound, leaving a mass that contains few cells and consists mostly of collagen and other extracellular matrix proteins to provide strength to the healing tissue. | 6 | Deep sternal wound infection after cardiac surgery: Evidences and controversies显示文摘Despite many advances in prevention and perioperative care, deep sternal wound infection(DSWI) remains a pressing concern in cardiac surgery, with a still relevant incidence and with a considerable impact on in-hospital mortality and also on mid- and long-term survival. The permanent high impact of this complication is partially related to the increasing proportion of patients at highrisk for infection, as well as to the many patient and surgical risk factors involved in the pathogenesis of DSWI. The prophylactic antibiotic therapy is one of the most important tools in the prevention of DSWI. However, the choice of antibiotic, the dose, the duration, the adequate levels in serum and tissue, and the timing of antimicrobial prophylaxis are still controversial. The treatment of DSWI ranges from surgical revision with primary closure to surgical revision with open dressings or closed irrigation, from reconstruction with soft tissue flaps to negative pressure wound therapy(NPWT). However, to date, there have been no accepted recommendations regarding the best management of DSWI. Emerging evidence in the literature has validated the efficacy and safety of NPWT either as a single-line therapy, or as a 'bridge' prior to final surgical closure. | 7 | Effect and Mechanism of Ganoderma lucidum Polysaccharides on Human Fibroblasts and Skin Wound Healing in Mice显示文摘Objective: To investigate the effects of Ganoderma lucidum polysaccharides(GL-PS) on human fibroblasts and skin wound healing in Kunming male mice and to explore the putative molecular mechanism. Methods: Primary human skin fibroblasts were cultured. The viability of fibroblasts treated with 0, 10, 20, 40, 80, and 160 μg/mL of GL-PS, respectively were detected by 3-4,5-dimethyl-2-thiazolyl-2,5-diphenyl-2-Htetrazolium bromide(MTT). The migration ability of fibroblasts treated with 0, 10, 20, and 40 μg/mL of GL-PS were measured by transwell assay. The secretion of the C-terminal peptide of procollagen type Ⅰ(CICP) and transforming growth factor-β1(TGF-β1) in the cell supernatant was tested by enzyme-linked immunosorbent assay. The expression of β-catenin was detected by Western blot. Furthermore, the Kunming mouse model with full-layer skin resection trauma was established, and was treated with 10, 20, and 40 mg/mL of GL-PS, respectively as external use. The size of the wound was measured daily, complete healing time in each group was recorded and the percentage of wound contraction was calculated. Results: Compared with the control group, 10, 20, and 40 μg/mL of GL-PS significantly increased the viability of fibroblasts, promoted the migration ability of fibroblasts, and up-regulated the expressions of CICP and TGF-β1 in fibroblasts(P<0.05 or P<0.01). The expression of β-catenin in fibroblasts treated with 20 and 40 μg/mL of GL-PS was significantly higher than that of the control group(P<0.01). Furthermore, after external use of 10, 20, and 40 mg/mL of GL-PS, the rates of wound healing in mice were significantly higher and the wound healing time was significantly less than the control group(P<0.05 or P<0.01). | 9 | The wound healing potential of collagen peptides derived from the jellyfish Rhopilema esculentum显示文摘Purpose: Wound represents a major health challenge as they consume a large amount of healthcare resources to improve patient's quality of life. Many scientific studies have been conducted in search of ideal biomaterials with wound-healing activity for clinical use and collagen has been proven to be a suitable can didate biomaterial. This study in tended to investigate the wound healing activity of collagen peptides derived from jellyfish following oral administration. Methods: In this study, collagen was extracted from the jellyfish-Rhopilema esculentum using 1% pepsin. Sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and fourier transform infrared (FTIR) were used to identify and determine the molecular weight of the jellyfish collagen. Collagenase II, papain and alkaline proteinase were used to breakdown jellyfish collagen into collagen peptides. Wound scratch assay (in vitro) was done to determine migration potential of human umbilical vein endothelial cells (HUVEC) covering the artificial wound created on the cell monolayer following treatment with collagen peptides. In vivo studies were con ducted to determine the effects of collagen peptides on wound healing by examining wound contraction, re-epithelialization, tissue regeneration and collagen deposition on the wounded skin of mice. Confidence level (p < 0.05) was considered significant using GraphPad Prism software. Results: The yield of collagen was 4.31%. The SDS-PAGE and FTIR showed that extracted collagen from jellyfish was type I. Enzymatic hydrolysis of this collagen using collagenase II produced collagen peptides (CPi) and hydrolysis with alkaline proteinase/papain resulted into collagen peptides (CP2). Tricine SDSPAGE revealed that collagen peptides consisted of protein fragments with molecular weight <25 kDa. Wound scratch assay showed that there were significant effects on the scratch closure on cells treated with collagen peptides at a concentration of 6.25 μg/mL for 48 h as compared to the vehicle treated cells. Overall treatment with collagen peptide on mice with full thickness excised wounds had a positive result in wound contraction as compared with the control. Histological assessment of peptides treated mice models showed remarkable sign of re-epithelialization, tissue regeneration and increased collagen deposition. Immunohistochemistry of the skin sections showed a significant increase in β-fibroblast growth factor (β-FGF) and the transforming growth factor-βi (TGF-βi) expression on collagen peptides treated group. | 10 | In vitro and in vivo investigation of bacterial cellulose dressing containing uniform silver sulfadiazine nanoparticles for burn wound healing显示文摘Silver sulfadiazine(SSD) particles in homogeneous dispersion state were prepared by an ultrasonic method and then nano- and microparticles were separated using centrifugation. SSD particles with narrow size distribution were impregnated with bacterial cellulose(BC) to produce BC–SSD composite membrane used as burn wound dressing. A scanning electron microscope(SEM) was used to examine the surface morphology of BC–SSD membranes. The incorporation of SSD in BC–SSD was confirmed by X-ray diffraction(XRD). Antimicrobial tests in vitro indicated that BC–SSD showed excellent antibacterial activity against Staphylococcus aureus, Pseudomonas aeruginosa and Escherichia coli. The effects of BC–SSD on burn wound healing were assessed by rat models. The comparative study confirmed that the wound treated with BC–SSD showed high healing rate. The bacteria count in BC–SSD group was far less than control group. Histological analysis showed that epithelialization progressed better in wound treated with BC–SSD. | 12 | Effect of Yupingfeng granules on the skin barrier in atopic dermatitis mice models显示文摘OBJECTIVE: To investigate effect of Yupingfeng granules, prepared with Chinese Medicines, on the wound healing and on the expression of aquaporin3(AQP3) and the skin barrier in the animal models of atopic dermatitis(AD).METHODS: Acute skin lesions of AD models were prepared using 2,4-dinitrochlorobenzo(DNCB) in mice and animals were treated with either Yupingfeng granules or placebo for two weeks. Skin wound healing outcome was assessed by measuring skin thickness, weight(quality) of the skin, and trans-epidermal water loss(TEWL). Expression of AQP3 mRNA and protein was assessed by reverse transcriotion polymerase chain reaction(RT-PCR)and immunoblotting, respectively.RESULTS: Yupingfeng granule treatment resulted in significant acceleration of wound healing with63.64% efficiency, which was significantly higher than that of placebo granule treatment(31.82%,P < 0.01 by Wilcoxon Rank-sum test). Skin thickness, weight of the wounded skin, and TEWL were significantly higher in the AD models compared to that of normal animals. Treatment with Yupingfeng granules resulted in significant decrease in skin thickness [(937 ± 31) vs(360 ± 21) μm, P < 0.01],weight of the wounded skin [(42 ± 4) vs(24 ± 5)mg, P < 0.01], and TEWL [(30 ± 4) vs(13 ± 4) g·h^(-1)·m-2, P < 0.01]. | 14 | Mesenchymal stem cell-derived small extracellular vesicles mitigate oxidative stress-induced senescence in endothelial cells via regulation of miR-146a/Src显示文摘Senescent endothelial cells (ECs) could impair the integrity of the blood vessel endothelium, leading to vascular aging and a series of diseases, such as atherosclerosis, diabetes. Preventing or mitigating EC senescence might serve as a promising therapeutic paradigm for these diseases. Recent studies showed that small extracellular vesicles (sEV) have the potential to transfer bioactive molecules into recipient cells and induce phenotypic changes. Since mesenchymal stem cells (MSCs) have long been postulated as an important source cell in regenerative medicine, herein we investigated the role and mechanism of MSC-derived sEV (MSC-sEV) on EC senescence. In vitro results showed that MSC-sEV reduced senescent biomarkers, decreased senescence-associated secretory phenotype (SASP), rescued angiogenesis, migration and other dysfunctions in senescent EC induced by oxidative stress. In the In vivo natural aging and type-2 diabetes mouse wound-healing models (both of which have senescent ECs), MSC-sEV promoted wound closure and new blood vessel formation. Mechanically, miRNA microarray showed that miR-146a was highly expressed in MSC-sEV and also upregulated in EC after MSC-sEV treatment. miR-146a inhibitors abolished the stimulatory effects of MSC-sEV on senescence. Moreover, we found miR-146a could suppress Src phosphorylation and downstream targets VE-cadherin and Caveolin-1. | 15 | Impact of Helicobacter pylori on the healing process of the gastric barrier显示文摘AIM To determine the impact of selected well defined Helicobacter pylori(H. pylori) antigens on gastric barrier cell turnover.METHODS In this study,using two cellular models of gastric epithelial cells and fibroblasts,we have focused on exploring the effects of well defined H. pylori soluble components such as glycine acid extract antigenic complex(GE),subunit A of urease(Ure A),cytotoxin associated gene A protein(Cag A) and lipopolysaccharide(LPS) on cell turnover by comparing the wound healing capacity of the cells in terms of their proliferative and metabolic activity as well as cell cycle distribution. Toxic effects of H. pylori components have been assessed in an association with damage to cell nuclei and inhibition of signal transducer and activator of transcription 3(STAT3) phosphorylation. RESULTS We showed that H. pylori GE,Cag A and Ure A promoted regeneration of epithelial cells and fibroblasts,which is necessary for effective tissue healing. However,in vivo increased proliferative activity of these cells may constitute an increased risk of gastric neoplasia. In contrast,H. pylori LPS showed a dose-dependent influence on the process of wound healing. At a low concentration(1 ng/m L) H. pylori LPS accelerated of healing epithelial cells,which was linked to significantly enhanced cell proliferation and MTT reduction as well as lack of alterations in cell cycle and downregulation of epidermal growth factor(EGF) production as well as cell nuclei destruction. | 17 | Effects of extracts from Chuanwu(Aconitum Carmichaelii) and Banxia(Rhizoma Pinelliae) on excisional wound healing in a rat's model显示文摘OBJEVTIVE: To investigate the effects of extracts from Chuanwu(Aconitum Carmichaelii) and Banxia(Rhizoma Pinelliae) on the excisional wound healing in a rat's model.METHODS: Rats were performed a surgical lesion with a 2.0 cm resected tissue in the dorsal fascia.Following, animals were randomly divided into model group, YNB group and APE group those were respectively treated with saline, 1 mg/mL of Yunnan Baiyao and 1 mg/mL of Chuanwu(Aconitum Carmichaelii) and Banxia(Rhizoma Pinelliae) extracts. Wound contractions in days 0, 3, 7, 14 and 21 were calculated by an image analyzer. Histological analysis was analyzed using hematoxilin and eosin.Levels of tumor necrosis factor-2(IL-2), IL-4, IL-10, transformαin(TNF-g growα), interleukinth factor-1(TGF-nd basic fibroblast growth factor(bFββ1) a-GF) were determined by real-time quantitative PCR.RESULTS: Compared with that of YNB group and APE gtoup, the skin of rats showed poor re-modeling and re-epithelization characterized by a significant decrease of neovascularization, epithelialization and fibroblast in the model group. | 18 | Randomised controlled trial of triclosan coated vs uncoated sutures in primary hip and knee arthroplasty显示文摘BACKGROUND Triclosan-coated vicryl plus suture(Ethicon, Inc.) was developed to reduce microbial colonisation during surgical procedures. However, its effect on wound healing and surgical site infections remain unclear after hip and knee arthroplasty surgery.AIM To determine the effect of triclosan-coated sutures(TCS) vs non-coated sutures on wound healing, following primary hip and knee arthroplasties.METHODS A single-centred, double-blind randomised controlled trial(RCT) was undertaken. We randomly allocated patients to receive either the triclosan-coated sutures(TCS vicryl plus) or non-coated sutures(NCS vicryl) during the closure of unilateral primary hip and knee arthroplasties. We utilised the ASEPSIS wound scoring system to evaluate wound healing for the first 6 weeks post-operatively.RESULTS One hundred and fifty patients undergoing primary total hip or knee arthroplasty over a one-year period were included. Eighty-one were randomised to the TCS group and 69 to the NCS group. Despite no statistically significant difference in the ASEPSIS scores among the study groups(P = 0.75), sensitivity analysis using the Mann Whitney test(P = 0.036) as well as assessment of the wound complications at 6 weeks follow up, demonstrated significantly higher wound complication rates in the TCS group(8 vs 1, P = 0.03).CONCLUSION No clear advantage was demonstrated for using the TCS. | 19 | Preliminary pharmacological evaluation of Martynia annua Linn leaves for wound healing显示文摘Objective:To evaluate the wound healing potential of fractions from ethanol extract of Martynin annua(M.annua) Linn leaves.Methods:Ethanol extract of Af.annua Linn leaves was fractionate into three different fractions(MAF-A,MAF-B and MAF-C) which were screened for wound healing potential using two models:excision and incision on rats.The thin layer chromatography(TLC) profile of all fractions were analysed and TLC of luteolin was also done.The PovidoneIodine Ointment was used as reference for comparision.Excision and incision wounds were created on dorsal portion of rats for study.Wound contraction,biochemical parameters(protein level and hydroxyproline level) and histopathological study were performed in excision wound model whereas incision model was used for determination of tensile strength.Results:The wound contraction and tensile strength of skin tissues were observed significantly greater in MAF-C fraction treated group than other two fractions(P<0.01).In excision wound method(on day 18) protein content and hydroxyproline were found significantly higher in MAF-C group than control group(P<0.01).Histopathological study also showed better angiogenesis,matured collagen fibres and fibroblast cells as compared with the control group.Conclusions:In conclusion, our findings suggest that fiaction MAF-C from ethanol extract of M.annua leaves is found most effective in wound healing. | Santram Lodhi Singhai AK | 2011 | Asian Pacific Journal of Tropical Biomedicine2011,1,6: | 3 |
