| 1 | 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents显示文摘Syncope belongs to the transient loss of consciousness(TLOC), characterized by a rapid onset, short duration, and spontaneous complete recovery. It is common in children and adolescents, accounting for 1% to 2% of emergency department visits.Recurrent syncope can seriously affect children's physical and mental health, learning ability and quality of life and sometimes cardiac syncope even poses a risk of sudden death. The present guideline for the diagnosis and treatment of syncope in children and adolescents was developed for guiding a better clinical management of pediatric syncope. Based on the globally recent development and the evidence-based data in China, 2018 Chinese Pediatric Cardiology Society(CPCS) guideline for diagnosis and treatment of syncope in children and adolescents was jointly prepared by the Pediatric Cardiology Society, Chinese Pediatric Society, Chinese Medical Association(CMA)/Committee on Pediatric Syncope, Pediatricians Branch, Chinese Medical Doctor Association(CMDA)/Committee on Pediatric Cardiology, Chinese College of Cardiovascular Physicians, Chinese Medical Doctor Association(CMDA)/Pediatric Cardiology Society, Beijing Pediatric Society, Beijing Medical Association(BMA). | 3 | Effectiveness and safety of splenectomy for gastric carcinoma:A meta-analysis显示文摘AIM:To evaluate the impact of splenectomy on longterm survival,postoperative morbidity and mortality of patients with gastric cancer by performing a metaanalysis.METHODS:A search of electronic databases to identify randomized controlled trials in The Cochrane Library trials register,Medline,CBMdisc(Chinese Biomedical Database) and J-STAGE,etc was performed.Data was extracted from the studies by 2 independent reviewers.Outcome measures were survival,postoperative morbidity and mortality and operation-related events.The meta-analyses were performed by RevMan 4.3.RESULTS:Three studies comprising 466 patients were available for analysis,with 231 patients treated by gastrectomy plus splenectomy.Splenectomy could not increase the 5-year overall survival rate [RR = 1.17,95% confidence interval(CI) 0.97-1.41].The postoperative morbidity(RR = 1.76,95% CI 0.82-3.80) or mortality(RR = 1.58,95% CI 0.45-5.50) did not suggest any significant differences between the 2 groups.No significant differences were noted in terms of number of harvested lymph nodes,operation time,length of hospital stay and reoperation rate.Subgroup analyses showed splenectomy did not increase the survival rate for proximal and whole gastric cancer.No obvious differences were observed between the 2 groups when stratified by stage.Sensitivity analyses indicated no significant differences regarding the survival rates(P > 0.05) .CONCLUSION:Splenectomy did not show a beneficial effect on survival rates compared to splenic preservation.Routinely performing splenectomy should not be recommended. | Kun Yang Xin-Zu Chen Jian-Kun Hu Bo Zhang Zhi-Xin Chen Jia-Ping Chen | 2009 | World Journal of Gastroenterology2009,15,42: | 33 |

| 5 | An atomic force microscopy study of coal nanopore structure显示文摘Coal nanopore structure is an important factor in understanding the storage and migration of absorbed gas in coal. A new method for studying coal nanopore structures is proposed. This idea is based on the nano-level resolution of atomic force microscopy, which can be employed to observe the structural features of coal nanopores clearly, conduct quantitative three-dimensional measurements and obtain structural parameters. Analysis results show that coal nanopores are mainly metamorphic pores and intermolecular pores. The metamorphic pores are commonly rounded and elliptical, increasing quantitatively with the coalification degree. The forms of intermolecular pores change markedly. The average pore size of low-rank coal is bigger than high-rank coal, and the number of intermolecular pores decreases as the coal rank increases. Section analysis effectively characterizes the coal pore microstructure, bearing analysis is a vital approach to measure microporosity, and grain analysis can be employed to study the pore size distribution. | 9 | Complete chloroplast genome sequence of Magnolia grandiflora and comparative analysis with related species显示文摘Magnolia grandiflora is an important medicinal,ornamental and horticultural plant species.The chloroplast(cp) genome of M.grandiflora was sequenced using a 454 sequencing platform and the genome structure was compared with other related species.The complete cp genome of M.grandiflora was 159623 bp in length and contained a pair of inverted repeats(IR) of 26563 bp separated by large and small single copy(LSC,SSC) regions of 87757 and 18740 bp,respectively.A total of 129 genes were successfully annotated,18 of which included introns.The identity,number and GC content of M.grandiflora cp genes were similar to those of other Magnoliaceae species genomes.Analysis revealed 218 simple sequence repeat(SSR) loci,most composed of A or T,contributing to a bias in base composition.The types and abundances of repeat units in Magnoliaceae species were relatively conserved and these loci will be useful for developing M.grandiflora cp genome vectors.In addition,results indicated that the cp genome size in Magnoliaceae species and the position of the IR border were closely related to the length of the ycf1 gene.Phylogenetic analyses based on 66 shared genes from 30 species using maximum parsimony(MP) and maximum likelihood(ML) methods provided strong support for the phylogenetic position of Magnolia.The availability of the complete cp genome sequence of M.grandiflora provides valuable information for breeding of desirable varieties,cp genetic engineering,developing useful molecular markers and phylogenetic analyses in Magnoliaceae. | LI XiWen GAO HuanHuan WANG YiTao SONG JingYuan HENRY Robert WU HeZhen HU ZhiGang YAO Hui LUO HongMei LUO Kun PAN HongLin CHEN ShiLin | 2013 | Science China(Life Sciences)2013,56,2: | 17 |
| 10 | Cost-effectiveness analysis of chemotherapy for advancedgastric cancer in China显示文摘AIM: To assess the economics of various chemotherapeutic regimens for advanced gastric cancer (AGC), and to select the best cost-effective regimen for the common Chinese patients. METHODS: Data source used in this study was the Chinese Biomedical Disk Database. Patients were diagnosed as AGC and any regimen was eligible. Outcome measures included median survival time (MST) and percentage of complete and partial response (CR+PR). Economic statistics was per capita direct medical cost (DMC) of a single cycle. TreeAge Pro Healthcare 2007 software was used to carry out cost-effectiveness and incremental cost-effectiveness analysis. Sensitivity analyses were applied by altering willingness-to-pay and annual discount rate, and also re-analyzed by excluding the studies with apparent heterogeneity. RESULTS: Seven retrospective economics studies on 760 patients were included. 5-fluorouracil-based regimens were universal, and also some new agents were involved, such as docetaxel, paclitaxel, and oxaliplatin. By processing analysis, we could recommend etoposide, leucovorin and 5-fluorouracil (ELF) regimen as preference, with a DMC/MST ratio of 2543 RBM/11.7 mo and a DMC/CR+PR ratio of 2543 RMB/53.3%. Uracil-tegafur, etoposide and cisplatin (FEP) or 5-fluorouracil, adrimycin/epirubin and mitomycin (FAM) regimens could be regarded as optional first-line chemotherapy for AGC in common Chinese patients. With no regard for willingness-to-pay, the docetaxel, cisplatin and 5-fluorouracil (DCF) regimen could be chosen as either a first- or a second-line chemotherapy, with a DMC/CR+PR ratio of 9979 RMB/56.3%. CONCLUSION: 5-fluorouracial regimens are still considered the mainstream for AGC, while new agents such as taxanes are optional. | 13 | Magnolol additive as a replacer of antibiotic enhances the growth performance of Linwu ducks显示文摘Magnolol rich in Magnolia officinalis is a bioactive polyphenolic compound. The aim of this study was to examine the effects of magnolol additive(MA) on growth performance, expression levels of antioxidantrelated genes, and intestinal mucosal morphology of Linwu ducks aged from 49 to 70 days, comparing with that of an antibiotic additive(colistin sulfate [CS]). A total of 275,49-day-old ducks were assigned to5 groups with 5 cages of 11 ducks each and fed diets supplemented with 0,100, 200 and 300 mg of MA/kg and 300 mg of CS/kg for 3 weeks, respectively. The results showed that the average daily body weight gain(ADG) was increased significantly in MA-fed groups(200 and 300 mg/kg), compared with the basal diet(BD) group(P < 0.05). The mRNA levels of superoxide dismutase-1(SOD1), manganese superoxide dismutase-2(MnSOD2) and catalase(CAT) were also increased significantly in MA groups(P < 0.05). In addition, hematoxylin and eosin staining revealed that Linwu ducks fed the diets with MA had more intact intestinal mucosa than those fed the BD and CS diets. In addition, ileal villus height, ileal villus height/crypt depth ratio(V/C) and duodenal V/C were also improved significantly(P < 0.05). | 14 | A Study of Sr, Nd and O Isotopes of the K-rich Melanocratic Dykes in the Late Mesozoic Gold Field in the Jiaodong Peninsula显示文摘Geologic, petrographic and petrochemical studies of the lateMesozoic K-rich melanocratic dykes, including lamprophyres, andesite porphyrites and dacite-porphyry in the gold field system in the Jiaodong Peninsula, China, have shown that these dykes are characterized by rich potassium and alkali but poor titanium. They belong to an ultra-high potassic, shoshonitic and high potassic calc-alkaline rock series. The parental magma has relatively high initial strontium ratios ((87Sr/86Sr)i-0.70895-0.71140) and low (143Nd/144Nd)i ratios (varying from 0.51135 to 0.51231); and its δ18OsMow. whole rock values vary from +5.8‰ to +10.6‰ with a mean of +7.1‰. These features suggest that the source region of the magma is an enriched mantle wedge transformed from a continental lithosphere mantle which has experienced me tasomatism by mantle-derived fluids with H2O-dominated fluids that were provided during the underthrusting of an ocean crust. The initial magma was generated by low-degree partial melting of the enriched mantle in its mature stage in the back-arc spreading environment. The evolution of magmas is associated with two trends, i.e., fractional crystallization and mixing with or intensive contamination by palaeo-crust materials or metamorphic rocks. The former process is evident in the gold field system of quartz-vein type, whereas the latter is dominated in the gold field system of the altered-rock type. | 15 | Numerical investigation on the sensitivity of jointed rock mass strength to various factors显示文摘The mechanical properties of jointed rock masses, such as strength, deformation and the failure mechanism, can be understood only by studying the sensitivity of jointed rock mass strength (both the peak and residual strengths) to the factors that affect it. An orthogonal design of uniaxial compression tests was simulated on eighteen groups of jointed rock specimens having different geometric and mechanical properties using RFPA2D (Rock Failure Process Analysis) code. The results show that the peak strength is controlled by the geometric parameters of the joints, but that the residual strength is controlled by the mechanical prop- erties of the joint interfaces. The failure mode of jointed rock specimens is mainly shear failure. | 16 | Extracellular vesicles from human urine-derived stem cells prevent osteoporosis by transferring CTHRC1 and OPG显示文摘Osteoporosis is a debilitating bone disease affecting millions of people. Here, we used human urine-derived stem cells(USCs),which were noninvasively harvested from unlimited and easily available urine, as a 'factory' to obtain extracellular vesicles(USCEVs) and demonstrated that the systemic injection of USC-EVs effectively alleviates bone loss and maintains bone strength in osteoporotic mice by enhancing osteoblastic bone formation and suppressing osteoclastic bone resorption. More importantly, the anti-osteoporotic properties of USC-EVs are not notably disrupted by the age, gender, or health condition(with or without osteoporosis) of the USC donor. Mechanistic studies determined that collagen triple-helix repeat containing 1(CTHRC1) and osteoprotegerin(OPG) proteins are enriched in USC-EVs and required for USC-EV-induced pro-osteogenic and anti-osteoclastic effects. EORTC trial 40954 has recently reported that NAC plus surgery without postoperative adjuvant chemotherapy could not benefit the locally AGC patients in their overall survival. We performed a meta-analysis of 10 studies including 1518 gastric cancer patients. Stratified subgroups were NAC plus surgery and NAC plus both surgery and adjuvant chemotherapy (AC), while control was surgery alone. The results showed that NAC plus surgery did not benefit the patients with locally AGC in their overall survival [odds ratio (OR) = 1.20, 95% CI 0.80-1.80, P = 0.37] and the number needed to treat (NNT) was 74. However, the NAC plus both surgery and AC had a slight overall survival benefit (OR = 1.33, 95% CI 1.03-1.71, P = 0.03) and NNT was 14, which is superior to the NAC plus surgery. | 18 | Comparing the effects of depression,anxiety,and comorbidity on quality-of-life,adverse outcomes,and medical expenditure in Chinese patients with acute coronary syndrome显示文摘Background:Depression and anxiety have been correlated with elevated risks for quality-of-life (QOL),adverse outcomes,and medical expenditure in patients with acute coronary syndrome (ACS).However,the relevant data are lacking for Chinese ACS populations,especially regarding different effects of major depression,anxiety,and comorbidity.The objective of this study was to evaluate the dynamic changes of depression and/or anxiety over 12 months and examine the effects of depression,anxiety,and comorbidity on QOL,adverse outcomes,and medical expenditure in Chinese patients with ACS.Methods:For this prospective longitudinal study,a total of 647 patients with ACS were recruited from North China between January 2013 and June 2015.Among them,531 patients (82.1%) completed 12-month follow-ups.Logistic regression model was utilized for analyzing the association of baseline major depression,anxiety,and comorbidity with 12-month all-cause mortality,cardiovascular events,QOL,and health expenditure.Results:During a follow-up period of 12 months,7.3% experienced non-fatal myocardial infarction (MI) and 35.8% cardiac rehospitalization.Baseline comorbidity,rather than major depression/anxiety,strongly predicted poor 12-month QOL as measured by short-form health survey-12 (odds ratio [OR]:1.77,95% confidence interval [CI]:1.22–2.52,P = 0.003).Regarding 12-month non-fatal MI and cardiac re-hospitalization,baseline anxiety (OR:2.83,95% CI:1.33–5.89,P<0.01;OR:4.47,95% CI:1.50–13.00,P<0.01),major depression (OR:2.58,95% CI:1.02–6.15,P<0.05;OR:5.22,95% CI:1.42–17.57,P<0.03),and comorbidity (OR:6.33,95% CI:2.96–13.79,P<0.0001,OR:14.08,95% CI:4.99–41.66,P<0.0001) were all independent predictors,and comorbidity had the highest predictive value.Number of re-hospitalization stay,admission frequency within 12 months and medical expenditure within 2 months were the highest in patients with ACS with comorbidity.Conclusions:Major depression and anxiety may predict 12-month non-fatal MI and cardiac re-hospitalization.However,comorbidity has the highest predictive value with greater medical expenditure and worse QOL in Chinese patients with ACS.And depression with comorbid anxiety may be a new target of mood status in patients with ACS. | Kun Xia Le-Feng Wang Xin-Chun Yang Hong-Yan Jiang Li-Jing Zhang Dao-Kuo Yao Da-Yi Hu Rong-Jing Ding | 2019 | Chinese Medical Journal2019,,9: | 13 |
| 19 | A population-based survey of gastroesophageal reflux disease in a region with high prevalence of esophageal cancer in China显示文摘Background: The exact relationship between gastroesophageal reflux disease (GERD) and esophageal squamous cell cancer (ESCC) is far from clarification. The aim of this study was to investigate the epidemiology of GERD in a region with high prevalence of ESCC in China. Methods: A population-based, cross-sectional study was conducted in a high ESCC prevalent area, Anyang, Henan, China. All subjects fulfilled questionnaires and underwent gastroendoscopy with routine esophageal biopsy. The subjects were divided into GERD subtypes (reflux esophagitis [RE] and non-erosive reflux disease [NERD]) and controls. Odds ratios (ORs) and 95% confidence intervals (Cis) were calculated to examine risk factors for RE and NERD. Results: A total of 2844 subjects were finally enrolled. The prevalence of GERD (RE + NERD) was 17.3%. Among them, 271 (9.53%) adults were diagnosed with RE. The prevalence of RE increased with age (7.09% in 45-50 years, 8.00% in 51-60 years, and 9.53% in 61-69 years,x^2 = 62.216, P < 0.001). Sixty-seven (2.36%) subjects were diagnosed with the silent RE. A total of 221 (7.77%) subjects were diagnosed with NERD. Frequent liquid food consumption (OR [95% CI]: 1.502 [1.076-2.095]) was independent risk factor for RE as well as age, male gender, high body mass index (BMI), ever smoking. Age was independent risk factor for NERD. For silent RE, age, male gender, and frequent liquid food consumption were risk factors. Conclusions: In the population with high prevalence of ESCC, a high prevalence of GERD and inverted proportion of RE/NERD were presented. Age was an independent risk factor for GERD.
