| 1 | Hematological profile of COVID-19 infected children before and after the spread of the Omicron variant in Istanbul显示文摘Objective:To examine the effect of the COVID-19 virus,especially the Omicron variant,on hematological parameters of hospitalized pediatric patients during the COVID-19 pandemic.Methods:Medical records of pediatric COVID-19 patients hospitalized at Kartal Dr.Lütfi K?rdar City Hospital in Istanbul,Turkey,between March 2020 and May 2022 were retrospectively reviewed to analyze data regarding demographics,SARS-Co V-2 infection polymerase chain reaction(PCR)test results,reversetranscriptase(RT)-PCR for other respiratory agents,duration of hospital stay,and hematological and biochemical laboratory findings.Results:Out of 467 children with a confirmed diagnosis of SARSCo V-2 infection,94(20.1%)had Omicron infection and 373(79.9%)were infected with other variants;the Omicron group had younger patients than the remaining samples(P<0.001).The most frequent clinical symptoms in all children were cough(53.5%)and fever(32.3%),followed by vomiting(20.8%).Lung involvement in the Omicron group(10.6%)was significantly lower than in the remaining samples(29.8%)(P<0.001).Hemoglobin and lymphocyte levels were lower in the Omicron-infected group(both P<0.001),while prothrombin time,activated partial thromboplastin time,international normalized ratio,and D-dimer levels were significantly higher in this group(P<0.001,P<0.001,P<0.001,and P=0.023,respectively).In terms of lung involvement,those with lung involvement were significantly older(P<0.001).Conclusions:Although lung involvement was less common with Omicron infection,this group had greater hematological system involvement,such as anemia,lymphopenia,D-dimer elevation,and coagulation disorders. | Berkin Berk Akbeyaz AyŞe Karaaslan CerenÇetin Mehmet Tolga Köle Zeynep AltaŞ Yasemin Akın | 2022 | Asian Pacific Journal of Tropical Medicine2022,15,12: | 0 |
| 2 | Improvement in renal functions with transcatheter aortic valve implantation显示文摘BACKGROUND & OBJECTIVES:In recent years, emerging transcatheter aortic valve implantation (TAVI) has become an alternative for surgery。 However, with advanced age, several co-morbid factors together with contrast agent usage can cause deterioration in renal function and increase in the risk of acute kidney injury (AKI) with poor prognosis in patients with AKI。 Therefore, many patients cannot benefit from this treatment。 In this study, we aim to examine the effects of TAVI on renal functions。 METHODS AND RESULTS: Seventy patients, mean age of 77.6 years, underwent TAVI between July 2011 and December 2012。 Estimated glomerular filtration rate (eGFR) was calculated by using the Cockcroft and Gault Formula。 Patients were monitored for 48 h for urine output。 Stage 1 AKI, according to the VARC-2 AKIN system, developed in only five (7.1%) of the patients after the procedure。 There was a statistically significant increase between the mean 1(st) month eGFRs before (68。2 vs。 61.0, P < 0.01) and after (68。2 vs。 63.6, P < 0.05) the TAVI in the cohort。 After TAVI (48。5 mL/min, P < 0.01) and the 1(st) month (52.1 mL/min, P < 0.01), the eGFR of the 36 (51.4%) patients diagnosed with chronic kidney disease before the procedure showed a statistically significant increase in renal functions。 The hospital mortality rate was higher in the group which developed AKI (P < 0.01)。 First month eGFR showed a more statistically significant increase than pre-TAVI eGFR (62.8 and 69.8, P < 0.05, respectively) in AKI developing patients and this difference - though statistically not significant - continued into the sixth month。 CONCLUSIONS: In this study, we showed that the treatment of aortic stenosis through TAVI allows improvement of renal functions, and that AKI rates will be lower with careful patient selection, proper pre-procedural hydration, and careful use of contrast agent。 | Telat Keleş Huseyin Ayhan Durmaz Sarı Aslan AN Erdoğan KE Kasapkara Bilen Bayram Akçay Bozkurt | 2013 | Journal of Geriatric Cardiology2013,10,4: | 2 |
| 3 | Quality of life after colorectal surgery:A prospective study of patients compared with their spouses显示文摘BACKGROUND Although radical surgery for colorectal cancer improves the oncological outcomes,a significant portion of patients suffer from alterations in their quality of life(QoL).There are many studies investigating the QoL of patients who have colorectal cancer but none of these focus on the QoL of spouses.AIM To compare the QoL of patients after colorectal surgery to the QoL of spouses.METHODS This prospective study consisted of patients who were married and who underwent surgery at the University of Ankara,Department of Surgery between March 2006 and November 2010.Patients’spouses were also enrolled.The study was approved by the Ethics Committee of the Faculty of Medicine,Ankara University,and all patients provided written informed consent.The study included patients who underwent curative surgery for colorectal carcinoma[n=100;abdominoperineal excision(n=33),low anterior resection(n=33),left hemicolectomy(n=34)]and their spouses(n=100).The patients and spouses completed the Medical Outcome Study 36-item Short Form Survey(SF-36)and the World Health Organization Disability Assessment Schedule II(WHODAS-II)preoperatively and at postoperative months 15 to 18.RESULTS During this 4.5-year study period,273 patients with sigmoid or rectal cancer were admitted to the hospital.Of these patients,119 were eligible and willing to participate.Eleven patients had either systemic or locally inoperable disease,three patients had a severe surgical complication,and five patients were lost to followup.Therefore,a total of 100 patients completed the follow-up period.There was a statistically significant positive correlation between the disability scores of patients and the scores of their spouses for some of the WHODAS-II subscales,such as“self-care,”“life activities,”and“participation in society,”as well as for the total WHODAS-II score.There was also a positive correlation between the QoL of patients and the QoL of their spouses in most of the SF-36 subscales.Statistically significant correlations were observed for the“bodily pain,”“general health,””vitality,”“social function,”“emotion,”“mental health,”and mental component summary score subscales of the SF-36.When gender differences were evaluated,the QoL of male patients’spouses changed more when compared with female patients’spouses for all of the WHODAS-II subscales.Colorectal cancer surgery has a significant effect on the QoL of both patients and their spouses,these effects were more significant among male patients’spouses.CONCLUSION Preoperative counseling regarding potential problems should therefore collectively address patient and their spouse as a couple rather than the patient alone,particularly for patients undergoing low anterior resection and abdominoperineal resection procedures. | Gökçe Aylaz Cihangir Akyol Akın Fırat Kocaay Derya Gökmen Ayşe Burcu Yavuzarslan Ayhan Bülent Erkek Mehmet Ayhan Kuzu | 2021 | World Journal of Gastrointestinal Surgery2021,13,9: | 1 |
| 4 | Human bocavirus infection in children hospitalized with lower respiratory tract infections:Does viral load affect disease course?显示文摘Objective:To examine the effects of human bocavirus type 1(HBoV1)on the course of lower respiratory tract infections in cases of monoinfection and coinfection,and the effects of HBoV1 viral load on the disease in children under six years old hospitalized with a diagnosis of HBoV1-associated lower respiratory tract infections.Methods:Children under six years of age,who were hospitalized with the diagnosis of lower respiratory tract infection due to HBoV1 between 1 January 2021 and 1 January 2022 were included in the study.Laboratory confirmation of the respiratory pathogens was performed using polymerase chain reaction(PCR).Results:Fifty-four(16.4%)children with HBoV1 among 329 children whose PCR was positive with bacterial/viral agent in nasopharyngeal swab samples were included in the study.There were 28(51.9%)males and 26(48.1%)females with a median age 23.4 months[interquartile range(IQR):13.2,30.0 months](min-max:1 month-68 months).HBoV1 was detected as a monoinfecton in 26(48.1%)children,and as a coinfection with other respiratory agents in 28 children(51.9%).In multiple regression analysis,coinfection(P=0.032)was associated with the length of hospitalization(P<0.001;R^(2)=0.166).There was a negative correlation(r=−0.281,P=0.040)between cough and cycle threshold.Fever was found to be positively correlated with C-reactive protein(r=0.568,P<0.001)and procalcitonin(r=0.472;P=0.001).Conclusions:Although we found a higher HBoV1 viral load in children with more cough symptoms in our study,it had no effect on the severity of the disease,such as length of hospital stay and need for intensive care.Coinfection was found to affect the length of hospitalization. | Ayşe Karaaslan CerenÇetin Serap Demir Tekol Ufuk Yükselmiş Mehmet Tolga Köle Yasemin Akın | 2022 | Asian Pacific Journal of Tropical Medicine2022,15,8: | 1 |
| 5 | Crescentic glomerulonephritis in children:a single centre experience显示文摘Background:Crescentic glomerulonephritis(CsGN)is characterized by crescents in 50%or more of glomeruli and clinically by a sudden and progressive decline in renal function.Methods:We evaluated the etiology,clinical features,prognostic factors and long-term outcome of CsGN.Between January 2000 and December 2010,45 children(26 girls,19 boys)with biopsy-proven CsGN(>50%crescents)were investigated retrospectively.Results:The mean age of the patients was 130.86±33.77 months.The mean duration of symptoms prior to diagnosis was 26±12 days(4-40 days).Most of the children had hypertension(62.2%),macroscopic hematuria(73.3%),oligoanuria(44.4%),edema(51.1%)and purpuric rash(40%)at presentation.The final clinical status of the patients was complete remission(n=21),partial remission(n=5)or chronic kidney disease(n=19).Adverse outcomes were significantly associated with a long duration between the onset of symptoms and treatment(P=0.038),the presence of oligoanuria(P=0.006),a severe decreased glomerular filtration rate(GFR<30 mL/min/1.73m^(2))and the need for dialysis(P=0.003)on admission,the ratio of crescents(>75%)(P=0.03),and the ratio of fi brous crescents(P=0.015).Conclusion:The outcome of CsGN in children continues to be poor,and it should be treated as a renal emergency. | Sare GülfemÖzlü AysunÇaltık Özlem Aydoğ Mehmet Bülbül Gülay Demircin EvrimÇakıcı Nilüfer Arda AyşeÖner | 2016 | World Journal of Pediatrics2016,12,2: | 0 |
