| 1 | Anterior resection for rectal carcinoma-risk factors for anastomotic leaks and strictures显示文摘AIM:To determine the incidence and factors responsible for anastomotic leaks and stricture following anterior resection(AR)and its subsequent management.METHODS:Retrospective analysis of data from 108 patients with rectal carcinoma who underwent AR or low anterior resection(LAR)to identify the various preoperative,operative,and post operative factors that might have influence on anastomotic leaks and strictures.RESULTS:There were 68 males and 40 females with an average of 47 years(range 21-75 years).The median distance of the tumor from the anal verge was 8 cm(range 3-15 cm).Sixty(55.6%)patients underwent handsewn anastomosis and 48(44.4%)were stapled.The median operating time was 3.5 h(range2.0-7.5 h).Sixteen(14.6%)patients had an anastomotic leak.Among these,11 patients required reexploration and five were managed expectantly.The anastomotic leak rate was similar in patients with and without diverting stoma(8/60,13.4%with stoma and 8/48;16.7%without stoma).In 15(13.9%)patients,resection margins were positive for malignancy.Ninteen(17.6%)patients developed anastomotic strictures at a median duration of 8 mo(range 3-20 mo).Among these,15 patients were successfully managed with per-anal dilatation.On multivariate analysis,advance age(>60 years)was the only risk factor for anastomotic leak(P=0.004).On the other hand,anastomotic leak(P=0.00),mucin positive tumor(P =0.021),and lower rectal growth(P=0.011)were found as risk factors for the development of an anastomotic stricture.CONCLUSION:Advance age is a risk factor for an anastomotic leak.An anastomotic leak,a mucin-secreting tumor,and lower rectal growth predispose patients to develop anastomotic strictures. | Ashok Kumar Ram Daga Paari Vijayaragavan Anand Prakash Rajneesh Kumar Singh Anu Behari Vinay K Kapoor Rajan Saxena | 2011 | World Journal of Gastroenterology2011,17,11: | 28 |
| 2 | Head mass in chronic pancreatitis: Inflammatory or malignant显示文摘Chronic pancreatitis increases the risk of developing pancreatic cancer. This often presents as a mass lesion in the head of pancreas. Mass lesion in the head of pancreas can also occur secondary to an inflammatory lesion. Recognising this is crucial to avoid unnecessary surgery. This is sometimes difficult as there is an overlap in clinical presentation and conventional computed tomography(CT) abdomen findings in inflammatory andmalignant mass. Advances in imaging technologies like endoscopic ultrasound in conjunction with techniques like fine needle aspiration, contrast enhancement and elastography as well as multidetector row CT, magnetic resonance imaging and positron emission tomography scanning have been shown to help in distinguishing inflammatory and malignant mass. Research is ongoing to develop molecular techniques to help characterise focal pancreatic mass lesions. | 3 | Fabrication of Bi_2O_3/TiO_2nanocomposites and their applications to the degradation of pollutants in air and water under visible-light显示文摘A nanoheterojunction composite photocatalyst Bi2O3/TiO2working under visible-light(λ 420 nm) was prepared by combining two semiconductors Bi2O3 and TiO2 varying the Bi2O3/TiO2molar ratio. Maleic acid was employed as an organic binder to unite Bi2O3 and TiO2 nanoparticles. The SEM, TEM, XRD and diffuse reflectance spectra were utilized to characterize the prepared Bi2O3/TiO2nanoheterojunction. The nanocomposite exhibited unusual high photocatalytic activity in decomposing 2-propanol in gas phase and phenol in aqueous phase and, evolution of CO2 under visible light irradiation while the end members exhibited low photocatalytic activity. The composite was optimized to 5 mol% Bi2O3/TiO2. | 4 | Management of splenic artery aneurysm associated with extrahepatic portal vein obstruction显示文摘BACKGROUND: Splenic artery aneurysms although rare are clinically significant in view of their propensity for spontaneous rupture and life-threatening bleeding. While portal hypertension is an important etiological factor, the majority of reported cases are secondary to cirrhosis of the liver. We report three cases of splenic artery aneurysms associated with extrahepatic portal vein obstruction and discuss their management. METHODS: The records of three patients of splenic artery aneurysm associated with extrahepatic portal vein obstruction managed from 2003 to 2010 were reviewed retrospectively. The clinical presentation, surgical treatment and outcome were analyzed. RESULTS: The aneurysm was >3 cm in all patients. The clinical symptoms were secondary to extrahepatic portal vein obstruction (hematemesis in two, portal biliopathy in two) while the aneurysm was asymptomatic. Doppler ultrasound demonstrated aneurysms in all patients. A proximal splenorenal shunt was performed in two patients with excision of the aneurysm in one patient and ligation of the aneurysm in another one. The third patient had the splenic vein replaced by collaterals and hence underwent splenectomy with aneurysmectomy. All patients had an uneventful post-operative course. CONCLUSIONS: Splenic artery aneurysms are associated with extrahepatic portal vein obstruction. Surgery is the mainstay of treatment. | 7 | Seasonal variations of phytoplankton diversity in the Coleroon coastal waters,southeast coast of India显示文摘The results of an investigation carried out during June 2005 to May 2007 on physico-chemical parameters,species composition and community structure of phytoplankton including Chlorophyll a(Chl-a) at the Coleroon coastal waters(Southeast coast of India) are reported.Air and surface water temperatures(°C) varied from 25.1 to 30.1 and from 24.5 to 28.5 respectively.Salinity values varied from 6 to 28.5 and the pH ranged between 7.0 and 8.3.Variation in dissolved oxygen content was from 3.1 to 7.5 mg/dm-3 while the light extinction coefficient values(LEC) ranged between 3.1 and 10.1 cm.The ranges of inorganic nutrients(μmol/dm-3) viz.,nitrate,nitrite,phosphate and silicate were 10.1-23.4,1.2-8.9,0.2-3.1 and 55-125,respectively.The ranges of Chlorophyll a(μg/dm-3) values was 2.0-7.5.Presently,124 phytoplankton species representing different classes viz:Bacillariophyceae(77),Dinophyceae(19),Cyanophyceae(15),Chlorophyceae(10) and Chrysophyceae(3) were recorded.The phytoplankton cell abundance varied from 0.290 to 111.662 cells/cm-3,with peak diversity(3.38 bits/ind.) during summer season.The maximum abundance was found during summer season coinciding with the stable hydrographical conditions.The seasonal distribution and abundance of phytoplankton are discussed in relation to hydrographical parameters.Canonical Correspondence Analysis(CCA) was applied in this paper for discriminating environmental factors having effect on phytoplankton community at species level.Coleroon coastal water is subjected to long term fluctuations in physico-chemical parameters depending upon the seasonal tidal amplitude and freshwater influx resulting in a continuous exchange of organic,inorganic,plant and animal matters. | Thillai Rajasekar K Rajkumar M SUN Jun Ashok Prabu V Perumal P | 2010 | Acta Oceanologica Sinica2010,29,5: | 3 |
| 8 | Omentum facilitates liver regeneration显示文摘AIM:To investigate the mechanism of liver regeneration induced by fusing the omentum to a small traumatic injury created in the liver. We studied three groups of rats. In one group the rats were omentectomized; in another group the omentum was left in situ and was not activated,and in the third group the omentum was activated by polydextran particles. METHODS:We pre-activated the omentum by injecting polydextran particles and then made a small wedge wound in the rat liver to allow the omentum to fuse to the wound. We monitored the regeneration of the liver by determining the ratio of liver weight/body weight,by histological evaluation (including immune staining for cytokeratin-19,an oval cell marker),and by testing for developmental gene activation using reverse transcription polymerase chain reaction (RT-PCR). RESULTS:There was no liver regeneration in the omentectomized rats,nor was there significant regeneration when the omentum was not activated,even though in this instance the omentum had fusedwith the liver. In contrast,the liver in the rats with the activated omentum expanded to a size 50% greater than the original,and there was histologically an interlying tissue between the wounded liver and the activated omentum in which bile ducts,containing cytokeratin-19 positive oval cells,extended from the wound edge. In this interlying tissue,oval cells were abundant and appeared to proliferate to form new liver tissue. In rats pre-treated with drugs that inhibited hepatocyte growth,liver proliferation was ongoing,indicating that regeneration of the liver was the result of oval cell expansion. | 10 | Breast carcinoma with metastasis to the gallbladder:an unusual case report with a short review of literature显示文摘Gallbladder metastases are very rare and usually arise from malignant melanoma, renal cell carcinoma and cervical carcinoma. Breast carcinoma tnetastatic to the gallbladder is extremely rare and only 4 cases have been reported in the English literature. We hereby report a 54-year-old lady who was diagnosed as having breast carcinoma and underwent modified radical mastectomy. One month after the operation, she developed acute abdomenal pain and underwent cholecystectomy after clinical investigation. Histopathological examination revealed metastasis to the gallbladder. Being considered a patient with tnetastatic breast carcinoma she was subjected to taxane and anthracycline-based palliative chemotherapy.
